1 05/11/15 04: STATE ROUTE 2, FREEMAN On 05/11/2015 at approximately 0433 hours, I was dispatched to E State Route 2 highway in reference to a check the welfare. My investigation revealed that a male subject was suicidal and was transported by emergency medical services to Cass Regional Medical Center. 05/11/15 10: MECHANIC ST, HARRISONVILLE On About 1000 hours I made contact with Garrison, Dustin in DIV IV court room. I was advised Mr. Garrison did have an active Cass County warrant 14CA-CR01166 for FTA Driving while revoked or suspended with a bond of $25, cash only. Mr. Garrison was given a court date of at 0900 hours in DIV IV. 05/11/15 10:56 JACKSON COUNTY JAIL, KANSAS CITY On 05/11/15, at 1056 hours, I responded to the Jackson County Jail reference a warrant service. I then took Anthony Jackson into custody on Cass County warrant # 15CA-CR00377,15CA-CR00376 and 15CA-CR00361 for Theft, with a total bond of $17,000. Mr. Jackson was transported to the Cass County Jail. 05/11/15 10:56 JACKSON COUNTY JAIL, KANSAS CITY On 05/11/15, at 1056 hours, I responded to the Jackson County Jail reference a warrant service. I then took Jason R Foundoukis into custody on Cass County warrant # 14CA-CR00450 for Fail To Pay fines, with a bond of $ cash only. Mr. Foundoukis was transported to the Cass County Jail. 05/11/15 16: WARD RD, RAYMORE On May 11, 2015 I received information in regards to forged procurement of U.S. currency by internet. Ms. Leslie Goodwin reported that an unknown suspect had used her identity to obtain a large sum of U.S. currency from an online money loan store found on the internet. 05/11/15 16: JAMAR DR, PECULIAR On May 11, 2015, I responded to 151 Jamar Street in Peculiar, Missouri to assist the Peculiar Police Department with a residence check. A runaway juvenile was taken into custody by the Peculiar Police Department. 05/11/15 20:53 267/RODIER, FREEMAN On May 11, 2015, I was dispatched to Rodier road north of 267th, reference an abandoned vehicle, which had been reported stolen in Raymore, Mo. Upon arrival I located a 1997 Dodge grey and black pickup truck. They vehicle was removed by GT towing at the request of Raymore Police Department. 05/11/15 22: PROSPECT AVE, BELTON On Monday May 11, 2015 at approximately 2208 hours I was dispatched to S. Prospect Ave Belton, MO, In reference to a stealing. Suspects took a miter saw and power washer from a detached garage. Garage door was partially left open by homeowner for their dogs. No suspects have been identified. 05/11/15 23: ST ST, GARDEN CITY On May 11, 2015 I was dispatched to E 291st Street in Rural Garden City for a property damage. I contacted Jim J Morgan who said that someone had damaged his mailbox. No suspects have been identified. 05/12/15 07: MECHANIC ST, HARRISONVILLE On 05/12/2015, at approximately 1750 hours, I made contact with Johnson County, Ks Sheriff's Office who advised me that they had an active warrant on Jackson, Anthony for Felony Theft.

2 05/12/15 07: STATE ROUTE P, PLEASANT HILL On May 12, 2015 I responded to the Helt's Turf Farm at State Rt P, Pleasant Hill, Missouri in reference to property damage. Mr. Chris Richardson reported damage to approximately five acres of bluegrass turf. Suspect information is not available at this time. 05/12/15 09: MECHANIC ST, HARRISONVILLE On , at approximately 0915 Hours, I made contact with Jody Ruth Allbrook, at the Cass County Sheriff's Office on a Cass County warrant. I served Cass County warrant for FTA- Failure to register Mtr Veh. 05/12/15 10: TH ST, PECULIAR On May 12, 2015 at approximately 1028 hours, I was dispatched to East 235th Street, Peculiar, Missouri in reference to a theft of a trailer. My investigation revealed that an enclosed trailer was stolen from the property some time this past weekend. 05/12/15 11: RD ST, GARDEN CITY On May 12, 2015 I responded to E. 323rd Street, Garden City, Missouri in assistance with Department of Family Services, as a follow up interview was needed to be conducted. 05/12/15 14: MECHANIC ST, HARRISONVILLE On 05/12/15 at approximately 1033 hours, I was notified by Cpl. J. Wise #661 that there was a subject in the front lobby of the Sheriff's Office, that had an active warrant out of another agency. I made contact with Daniel Peter Vitti and took him into custody on a Jackson County warrant with a $ % cash or secured bond. 05/12/15 15: STATE ROUTE 2, HARRISONVILLE On May 12, 2015 at approximately 1539 hours, I was dispatched to East State Route 2 Highway, Harrisonville, Missouri in reference to a physical disturbance in progress. My investigation revealed that a physical assault took place between family members. 05/12/15 16: MECHANIC ST, HARRISONVILLE On 05/12/2015, at approximately 1630 hours, I made contact with Laquita Renee Hamilton in the Cass County Jail at the booking desk. The vehicle was moved from the intersection without incident. 05/13/15 09: HATFIELD RD, GARDEN CITY On Wednesday April 13th, 2015 at approximately 0909 I responded to Hatfield Road in rural Garden City, Missouri in reference to a stealing. I made contact with the reporting party who stated someone had used his identity to file income taxes. No suspects have been identified in this investigation at this time. 05/13/15 12:53 NEWTON COUNTY JAIL, NEOSHO On 05/13/15, at 0922 hours, I responded to the Jackson County Jail reference a warrant service. I then took Shawn W Mclaughlin into custody on Cass County warrant # 12CA-CR00055 for Probation Violation, with a bond of $4,000 10% cash only. Mr. Mclaughlin was transported to the Cass County Jail. 05/13/15 13: MECHANIC ST, HARRISONVILLE On Wednesday, May 13, 2015, I was dispatched to the Cass County Sheriff s Office Lobby in regards to fraud. Upon my arrival, I spoke Leo G. Andrews and his daughter, Laura J. McVay, whom reported that unknown callers had called and scammed Mr. Andrews out of $ Dollars. 05/13/15 15: SIERRA DR, RAYMORE On Wednesday, May 13, 2015 at approximately 1540 hours, I conducted a warrant arrest at the residence of 110 West Sierra Drive in Raymore, Missouri in regards to an active felony warrant for probation violation. Justin D. Newkirk, white male was placed in to custody and transported to the Cass County jail where he was given a bond of $ Cash or Surety and a court date of June 01, 2015 at 0900 hours in Cass County courts. 05/13/15 15: HIGHWAY/58 HWY, RAYMORE On Wednesday at approximately 1715 hours I made contact with Cooper, Leonard L w/m. A on the corner of State route 291 and 58. A computer check revealed that he had a warrant for his arrest out of Clinton Missouri. Mr. Cooper was taken into custody and transported to the Cass County Jail. 05/13/15 16: MECHANIC ST, HARRISONVILLE On at about 1638 hours, I made contact at the Cass County jail and served a Cass County warrant. A suspect has been identified. 05/13/15 19: MECHANIC ST, HARRISONVILLE On 05/13/2015, at approximately 2000 hours, I arrested Shane Harmonica Lockhart, W/M at the Cass County Jail regarding a Cass County warrant. 05/13/15 20: TH ST, DREXEL On 05/13/2015 at approximately 2014 hours, I was dispatched to 8703 E 327th St in Drexel Missouri in reference to a burglary that had occured. My investigation revealed that family members had made entry into the house and taken property not yet declared to a owner in a divorce settlement. 05/14/15 07:13 SCHOOL RD N OF 195TH, RAYMORE On Thursday, May 14, 2015 at approximately 0732 hours, I conducted an investigation near the residence of South School Road in rural Raymore, Missouri in regards to a reported motor vehicle accident. No injuries were reported by any of the occupants of the vehicles. Both vehicles involved were able to be driven away from the scene. 05/14/15 08:29 WYANDOTTE COUNTY KS, KANSAS CITY On at 0829 hours, I responded to the Wyandotte County Kansas Jail reference a warrant service. I then took Torin White into custody on Cass County warrant # 10CA-CR for Fail To Appear, bond $16,000, Mr. White was transported to the Cass County Jail. 05/14/15 10: MECHANIC ST, HARRISONVILLE On at about 1035 hours, I made contact at the Cass County jail and served a Cass County warrant. I served Raymond Richardson warrant# 15CA-CR00464 for Violation of order of protection 1st offense, with a $2, cash/ surety bond and a court date of at 0900 hours. 05/14/15 10: EVERS RD, GARDEN CITY On Thursday May 14th, 2015 at approximately 1049 hours I was dispatched to 2501 W. Mechanic Street in reference to stealing. Upon arrival I made contact with Johnson, Dennis W/M and gathered information for the report. No suspects have been identified in this case. 05/14/15 10:40 235TH & LAKE RD, HARRISONVILLE On May 14, 2015 I responded to the area of E. 235th and S. Lake Rd in reference to property damage to the security fence around the Phillips 66 pipeline. It appears that a vehicle left the roadway and struck the fence. No suspects have been identified at this time. 05/14/15 10:48 TANEY COUNTY, FORSYTH On at 1048 hours, I responded to the Taney County Jail reference a warrant service. Tieman 641, responded to the Belton High in reference to providing K9 assistance to the Belton High School administration. 05/14/15 14: MECHANIC ST, HARRISONVILLE On about 1355 hours I made contact with Couffer, Amanda in DIV IV court room. Upon arrival I made contact with Jason E. Arnold w/m on a active Cass County warrant and transported him to the Cass County Jail. 05/14/15 17: BROOKRIDGE DR, HARRISONVILLE On Thursday may 14th, 2015 at approximately 1745 hours I was dispatched to 203 Brookside Drive in Harrisonville, Missouri in reference to a child custody issue. Upon arrival I made contact with Savvedra, Melissa W/F and gathered information for the report. 05/14/15 22: MECHANIC ST, HARRISONVILLE On May 14, 2015 I served Ryan Smith, w/m, Cass County Weekened Warrant for Forgery at 2501 West Mechanic Street, Harrisonville, Missouri. A court date of June 4, 2015 at 0900 hours was given with bond set at $25,000 cash or surety. 05/15/15 01:52 OUTER RD, ARCHIE On May 15, 2015, I responded to the Archie Commuter Lot located on the SE Outer Road in Archie, Missouri for a prisoner transfer with the Bates County Sheriff's Office. I took custody of Mike R. Barnhart and transported him to the Cass County Jail for booking. I recovered a red Ford two door vehicle that was reported stolen out of Harrisonville Police Department. 05/15/15 10: ND STREET, CLEVELAND On May 15, 2015, I investigated a report of an Ex Parte Violation that occurred at 15 S. 2nd Street in Cleveland, Missouri. A suspect has been identified in this incident. 05/15/15 10: MECHANIC ST, HARRISONVILLE On at approximately 1015 hours, in Division 2. I was advised by the prosecutor that Darrell Victor who testifying at the time for the state may have a warrant. At 1030 hours, I asked Darrell Victor if he had a warrant, he stated he may have one out of Jackson County. I then ran Darrell Victor though dispatch and was notified Darrell Victor had a lee Summit warrant for forgery, $10,000 ten percent cash or secured bond. Darrell Victor stated he left Center Point hospital to come to the court house to testify for the state and then once finish he was returning back to Center Point hospital. Darrell Victor stated he had a bad heart and his leg was swollen and bright red, Darrell Victor the lifted his pants leg and his right leg was swollen and red. Dispatch stated lee Summit would transport and that they were on their way. I held Darrell Victor until lee Summit PD arrival. At 1145 hour I handed Darrell Victor over to lee Summit PD to officer Hoverston. 05/15/15 11: RD STREET, BELTON On May 15, 2015, I responded to the Belton City Jail located at 7001 E. 163rd Street in Belton, Missouri for a prisoner transfer. I picked up Ryan L. Lamb and transported him to the Cass County Jail for booking. Mr. Lamb has a Cass County assault warrant with a $1, cash or surety bond. 05/15/15 12:41 NB 49/MM 172, RAYMORE On May 15, 2015 at approximately 1245 hours, I was dispatched to Northbound I-49 at Mile Marker 172 in reference to a hit and run motor vehicle accident. My investigation revealed that a vehicle was struck in the rear by a semi truck and the semi truck continued traveling Northbound on the interstate. 05/15/15 23: MECHANIC ST, HARRISONVILLE On , at approximately 2000 hours, I arrested Phillip D. Ward at the Cass County Jail as he turned himself of 2 days of shock time on case #14CA-CR /16/15 01:44 58 W/MILE OUTSIDE OF TOWN, PLEASANT HILL On Saturday May 16, 2015 at approximately 0144 hours, I was dispatched to a vehicle fire on 58 Hwy approximately one mile west of Pleasant Hill, MO. The vehicle was burned down to the frame and no one was found inside the vehicle. It is unknown what the cause of the fire was. I was unable to contact the owner of the vehicle. 05/16/15 02:33 SB 7/E OF GARDEN CITY, GARDEN CITY On May 16, 2015 at about 0233 hours, I stopped a 2000 Ford Crown Victoria traveling south on MO State Highway 7 south of Garden City. After stopping the vehicle for traveling 47 mph in a 70 mph zone the passenger, Mr. Kennith McAllister was arrested on eleven Independence Warrants. 05/16/15 08: MECHANIC ST, HARRISONVILLE On 05/16/2015, at approximately 0915 Hours, I made contact with Hernandez-Montesino, Manuel W/M, at the Cass County Sheriff's Office on a Cass County warrant. I served Cass County warrant 15CA-CR00491 for Statutory Rape 1st degree and Statutory Rape 2nd degree. Bond was set at $50, cash or surety and a court date of 06/01/2015 at 0900 hours was given.

7 05/16/15 10: TH ST, HARRISONVILLE On May 16, 2015 at approximately 1101 hours, I was dispatched to East 275th Street located in Harrisonville, Missouri in reference to a check the welfare. I investigated the death of a Dale Koske. His body was released to the Jackson County Medical Examiner's Office. 05/16/15 11: BEECH ST, GARDEN CITY On Saturday, May 16, 2015 at approximately 1151 hours I was dispatched to 2501 W. Mechanic, Harrisonville, MO for a walk in report of fraud. I contacted Michael Storiale who said that he had sent money transfers to an organization specializing in grant money. No grants were provided and no suspects have been identified. 05/16/15 13: MECHANIC ST, HARRISONVILLE On 05/16/2015, at approximately 1306 hours, I observed two inmates in a physical altercation in D-block of the Cass County Jail while manning secondary control. Both suspects have been identified. 05/16/15 14: MECHANIC ST, HARRISONVILLE On 05/16/2015, at approximately 1247 Hours, I made contact with Huey, Nicholas W/M, at the Cass County Sheriff's Office on court ordered arrest. Subject is to serve 48 hours shocktime. 05/16/15 14: TH STREET, CREIGHTON On May 16, 2015, I responded to Cass Regional Medical Center to investigate a reported dog bite that occurred in Creighton, Missouri. The victim was treated and released from the Emergency Department. 05/16/15 16:19 251/HARPER, PECULIAR On May 16, 2015, I investigated a motor vehicle collision on 251st Street at Harper Road in rural Peculiar, Missouri. A black Toyota Corolla struck a white Ford F350 as it turned in front of them. There were no reported injuries. 05/16/15 16: WHIPPOORWILL HILL DR, LAKE ANNETTE On May 16, 2015 I responded to 8311 E. Whippoorwill Hill Road, Lake Annette, Missouri in reference to a disturbance. Upon arrival the subjects were separated and agreed to remain separated with out further incident or criminal actions reported or observed. 05/16/15 20: TH ST, PECULIAR On Saturday May 16, 2015 at approximately 2039 hours I was dispatched to 2501 West Mechanic Harrisonville, MO in reference to an exparte violation. Subject has been receiving texts and phones calls from the respondent. Suspect has been identified. 05/16/15 20: ROLLING HILLS RD, BELTON On May 16, 2015 I was dispatched to Rolling Hills Road in Rural Belton Missouri for a report of a stealing. I contacted Dominic P. Ismert who said that someone had stolen his power washer from his shed. No suspects have been identified. 05/17/15 00: MECHANIC ST, HARRISONVILLE On 05/17/2015, at approximately 0035 Hours, I made contact with Steven McIntire at the Cass County Sheriff's Office on a Cass County warrant. I served Cass County warrant #15CA-CR00453 for Failure to Appear, and warrant # 15CA-CR00488 for Possession of a controlled substance, except 35 grams or less of marijuana. Bond was set at $4,000.00, 10% cash allowed, and $7,500.00, cash or surety, with a court date of 05/26/2015 at 0900 hours, and 06/01/2015 at 0900 hours were given. Lyne Avenue, located in Raymore Missouri in reference to a burglary in progress. My investigation revealed that no burglary took place but one of the persons at the residence was arrested for active warrants. 05/17/15 15:13 253RD/OVERFELT, FREEMAN On May 17, 2015 at approximately 1501 hours, I conducted a traffic stop on a gray Saab bearing Missouri license, at East 253rd Street and South Overfelt Road. Prior to the stop I recognized the subject driving to have a revoked driving status through the State of Missouri. A subject was arrested for driving while revoked and transported to the Cass County Jail. 05/17/15 16: TH ST, HARRISONVILLE On May 17, 2015 I responded to E. 289th Street, Harrisonville, Missouri in reference to a domestic assault. A male subject, John Stradford, stated that he had been assault. One subject was placed into custody and transported to the Cass County Jail. 05/17/15 18: MECHANIC ST, HARRISONVILLE On , at approximately 1820 Hours, I made contact with George L Chittim, at the Cass County Sheriff's Office on a Cass County warrant. I served Cass County warrant 13CA-CR00264 for Non-Support. 05/17/15 19: MECHANIC ST, HARRISONVILLE On 05/17/15 at approximately 1949 hours, I made contact with a subject at the Cass County Sheriff s Office who had an active warrant out of Cass County Missouri. I then placed the subject under arrest for the warrant. 05/17/15 21: MECHANIC ST, HARRISONVILLE On 05/17/15 at approximately 2130 hours I made contact with a subject that was ordered into custody by the Cass County Court. I then took the subject into custody per the Cass County Drug Court. 05/17/15 22: MECHANIC ST, HARRISONVILLE On , at approximately 2205 hours, I served multiple warrants to Jade Waller at the Cass County Jail. Bond was set at $510.00, cash only, and a court date of , at 0900 hours was given. 05/17/15 22:52 WARD 58HWY, RAYMORE On Sunday May 17, 2015 at approximately 2252 hours I was dispatched to Ward road just north of 58 Hwy, in reference to copper stripped and street lights not working. Also street lights were out. Wires were cut and a male subject was near the scene and taken into custody. Suspect has been identified.
