Undoubedly he general praciioner 1n rural areas does more of his ype emergency work han he urban physician whose paiens are usuallr aken o he hospial emergency room. The needle is hen insered and procaine injeced beneah he incised edge abou he periphery of he wound raher han o cause he producion of muli pie cuaneous puncures. Care n1us be aken no o over infilrae and cause issue disorion wih he re ulan malalignmen of skin edges.!for example, his problem exiss \\'hen infilraing he arseolar issues abou he eye. \X'ih he aneshesia no\\' complee, any hair presen is shaved and a horough clean ing is permied v. ih incure of green soap, saline ]a, age and incure merhiolae 1:1000. 5 negleced if one suspecs any foreign body o be embedded. c. Sururing Technic The axiom, "a man is only as good as his ools" can no be over en1phasized in he reanen of facial laceraions. Many fauly repairs have been he direc resul of a poor choice of in~runens. Wih his in mind, a mosquo needle holder and eye fixaion forceps are he insrumens of choice. The suure maerial should consis of No. 4-0 or No. 5-0 black silk on an araumaic cuing needle. nerruped maress ype suures are now placed in he skin. Dressings and Afer Care Upon compleion of he suuring, he laceraion is sponged wih 1: 1000 ineure of merhiolae. f he paien is o remain non-ambulaory, serile gauze squares held in place by :oiler banda~e may be applied. A 10r r warm bone acid soluion is applied direcly o he wound hrough he bandages a hree hour inervals. Wih he paien remaining ambulaory and coninuing his daily rouine, a serile vaseline gauze square held secure by elasoplas (Duke Laboraory) will suffice. Teanus Anioxin when indicaed is now given. The applicaion of sulpha crysals or powder is conra-indicaed in he managemen of hese condiions, for we n1us avoid excessive granulaion and scarring for cosmeic reasons. Anibioics, if indicaed, may be given by oher roues. n fory-eigh hours every oher suure is removed ~nd he remaining suures are o be removed he following day or seveny-wo hours afer he repar. n renoving he suures, a pair of small scissors and humb forceps are o Page 2 be employed. Excellen resuls hus ~ar ( aained may be compleely los wh careless removal of he suures. A small mediplas (Duke Laboraory) dressing is now sufficien. n he presence of any disconinuiy of issue, incure of benzoine is applied on eiher side of he wound and he issue is reapproximaed wih flame adhesive sraps. The adhesive is hen removed when adequae healing has aken place. Wihin wo o hree weeks, cocoa buer may be lighly massaged wih he finger ips over he cicarix o produce a local eryhema and sofening of he issues involved. Conclusion The five facors considered fundanenal in nanaging facial laceraions are: 1. Anisepsis 2. Adequae aneshesia 3. Proper insrumens and fine suure maerial 4. Good skin approxiinaion 5. The absence of underlying sof issue permi injury of underlying srucures such as joins, Yessels, nerves, bone and endon. Compound hand injuries, when firs seen, n1us be rapidly appraised and judgrnen exercised as o he major repair problem. Tendon division can usually be rapidly deermined and if presen or suspeced, operaing roorn peparaions should be made for furher definiive reamen. The san1e procedure applies o ampuaions, compound fracures, or exensive skin laceraions wih muscle or join involvemen and nerve severance. Under such circurnsances, serile pressure dressings are applied o he enire hand, morphine or some oher suiable analgesic adminisered and x-rays aken if desired. A general anesheic is o be preferred because i allows for he applicaion of a blood pressure cuff for Gonrol of bleeding. The echnique of wound cleansing en<jompasses he washing of he surrounding area of he hand firs, using coon, \Varnl, whie soap soluion, waer, and inennien and final washing \vih saline. Genleness is a all ines exercised when inspecing he recesses, crevices, and exension of he 'vound. A definie plan of repair can be forrnulacd as he cleansing of he wound is carried ou. A careful inspec in his inerval will re\ eal any injuries of deeper lying srucures ha n1igh oherwise be overlooked. The viabiliy of he injured issues can also be ascerained. A clean wound, once obained, is he bes possible insurance of primary healing as well as a finn foundaion for opposiion of injured srucures. Aenion o refinemens of echnique in handling he issues will lend some force o he characer and rae of healing. Such refinemens are only hose of meiculous surgery anywhere, bu ofen hey are no oo closely adhered o and, in hand wounds paricularly, hey are quie imporan. These refinemens include he clamping and ligaion of all bleeding poins including only lhe bleeding vessels and no overuse, of suure n1aerial of sufficien srengh bu fine calibre, painsaking suuring, and sure dressing wih adequae splining. Cerain common injuries should be discussed in some deail, paricularly heir definiive reamen. While oher sies of injury are no o be lighly dismissed, he area of greaes difficuly is he division of flexor endons in he proximal phalanges and paijn. Exension of, or addiional incision, is usually required and care mus be exercised in following naural skin folds and creases in making hem. The proximiy of nerves, blood vessels, and endons in he palm muliplies he number of injured srucures, adds o he confusion of idenificaion, and increases he amoun of fibrosis in healing ha one wishes o avoid. The commones error in repair in his area is he aemp o suure all of he endons severed. The flexor sublimis and he flexor profundus are raher snugly enclosed in a common endon sheah. When severed hey are divided a he same poin. f suured, he bulk of issue presen prevens covering hem adequaely wih endon sheah maerial. Subsequen swelling furher increases he conen of he endon sheah wih a likelihood ha he sheah will separae even if suured. The endons are likely o become adheren and he end resul is funcionally a poor one. Therefore, one endon, preferably he flexor sublimis, should be sacrificed by cuing i as far back as possible. The flexor profundus is suured and adequae sheah proecion provided. A saisfacory endon suure will usually resul in a good funcional finger. Silk suures have given good resuls, in he hands of mos surgeons, for endon repair and heir coninued use can be recommended. Two suures placed as visualized will usually suffice for approximaion. Exensor endons of he hand deserve equally imporan care in reamen. However, wih he same principles in suuring, he repair will be compleed wih greaer faciliy and ease on he dorsal surface of he hand because of he comparaive greaer ease of exposure and idenificaion. The primary endon and wound closure is usually possible if he wound is less han welve hours old, excep in such insances as gross conaminaion which would make i unwise. n such case, he wound is lef open and imporan srucures may be agged wih suures. Tendon suuring is done in hree weeks bu if wound infecion occurs, hree monhs should elapse before repair is aemped. (Operaive Procedure-Ehicon Suure Division of J & J. P. 93).
