1 Holy Trinity Catholic Community Men s Club & Women s Guild Educational Scholarship Deadline First Sunday in April Criteria: (1) Applicant must be baptized (2) Applicant must be confirmed (3) 9 th 12 th grade school cumulative grade point average of 2.5/80 or higher (4) Involved in Holy Trinity parish and /or youth activities (5) Applicant must be a registered member of Holy Trinity Catholic Church of Azle (6) Applicant should exhibit Christian values and attitudes (7) Applicant must be a graduating senior enrolled in a post high school institution within the same calendar year of school graduation (8) Turn in this application form to the church office by the above date Your name (PRINT) Applicant Instructions: (1) Pages 1 3 of this document are to be filled out in their entirety by you. (2) Pages 4 and 5: Lines 1 4 (your personal information) are to be filled out by you before giving those pages to the Holy Trinity youth director and the church secretary. (3) Page 4 of this document will be filled out by the church secretary. (4) Page 5 is to be filled out by the current Holy Trinity Youth coordinator/director. That person will fill out the form and that person will return their portion of the form to the Holy Trinity church office. (5) Be sure to print when asked to do so. (6) Attach a picture of you with permission to publish to page 1. (7) When answering the essay questions, please write carefully so that the committee can have a clear idea of whom you are and what your future goals in life are. Questions #12 16 are the questions most studied by the scholarship committee in determining the recipients of a scholarship. (8) Please try very hard to limit yourself to the lined space provided for you to write about yourself. (9) Please take care to give pages 4 and 5 to the correct people at least 2 weeks prior to the application deadline. (10) If you misplace this document, please ask for another one from the church office. We do not care what happened to the first one you received. Maybe in the future you will be able to download this form, but for now, just ask for another one if you misplace the first one you received.

Write this number wherever there is a year with dash located (i.e ) (2) Keep all applications, with the respective church secretary form and youth director form (related forms), together in as secure of a place as possible. (3) The scholarship committee chair will need up to five (5) copies of applications and related forms by the first Sunday of May. (4) For reasons of maintaining applicant anonymity, the pages to copy and given to the selection committee are only pages 2 & 3, with pages 4 & 5, with all personal information of the applicant removed, given only to the chairperson. Keep the original applications and related documents in the church office until the selection process is completed for the current applicants. (5) If the applicant wishes to correct their own information on their own application and related documents they may do so only in your presence. (6) If the applicant wishes to withdraw their application, tell the applicant that they can have their application back but they will not be given further consideration for scholarship from the committee once the applicant s application leaves the church office. -4-
