For a full list of excipients, see section 6.1. בסעיף 4.2 Posology and method of administration עודכן המידע הבא: Long-term remission and relapse rates are more closely related to the total dose administered than to either duration of treatment or daily dose. It has been shown that no substantial additional benefit is to be expected beyond a cumulative treatment dose of mg/kg. The duration of treatment will depend on the individual daily dose. Patients with severe renal insufficiency In patients with severe renal insufficiency treatment should be started at a lower dose (e.g. 10 mg/day). The dose should then be increased up to 1 mg/kg/day or until the patient is receiving the maximum tolerated dose (see section 4.4). Children Roaccutane is not indicated for the treatment of prepubertal acne and is not recommended in patients less than 12 years of age due to a lack of data on efficacy and safety. Patients with intolerance In patients who show severe intolerance to the recommended dose, treatment may be continued at a lower dose with the consequences of a longer therapy duration and a higher risk of relapse. In order to achieve the maximum possible efficacy in these patients the dose should normally be continued at the highest tolerated dose. בסעיף 4.3 Contraindications עודכן המידע הבא: Isotretinoin is contraindicated in women who are pregnant or breastfeeding. Isotretinoin is contraindicated in women of childbearing potential unless all of the conditions of the Pregnancy Prevention Programme are met (see section 4.4). Roaccutane contains soya oil, partially hydrogenated soya oil, and hydrogenated soya oil. Therefore, Roaccutane is contraindicated in patients allergic to peanut or soya. Isotretinoin is also contraindicated in patients receiving concomitant treatment with tetracyclines (see section 4.5). בסעיף 4.4 Special warnings and precautions for use עודכן המידע הבא: Pregnancy Prevention Programme Isotretinoin is contraindicated in women of childbearing potential unless all of the following conditions of the Pregnancy Prevention Programme are met: She has severe acne (such as nodular or conglobate acne or acne at risk of permanent scarring) resistant to adequate courses of standard therapy with systemic anti-bacterials and topical therapy (see section 4.1). She understands the teratogenic risk. She understands the need for rigorous follow-up, on a monthly basis. 1

3 She understands and accepts the need for effective contraception, without interruption, 1 month before starting treatment, throughout the duration of treatment and 1 month after the end of treatment. At least one and preferably two complementary forms of contraception including a barrier method should be used. Even if she has amenorrhea she must follow all of the advice on effective contraception. She should be capable of complying with effective contraceptive measures. She is informed and understands the potential consequences of pregnancy and the need to rapidly consult if there is a risk of pregnancy. She understands the need and accepts to undergo pregnancy testing before, during and 5 weeks after the end of treatment. She has acknowledged that she has understood the hazards and necessary precautions associated with the use of isotretinoin. These conditions also concern women who are not currently sexually active unless the prescriber considers that there are compelling reasons to indicate that there is no risk of pregnancy. The prescriber must ensure that: The patient complies with the conditions for pregnancy prevention as listed above, including confirmation that she has an adequate level of understanding. The patient has acknowledged the aforementioned conditions. The patient has used at least one and preferably two methods of effective contraception including a barrier method for at least 1 month prior to starting treatment and is continuing to use effective contraception throughout the treatment period and for at least 1 month after cessation of treatment. Negative pregnancy test results have been obtained before, during and 5 weeks after the end of treatment. The dates and results of pregnancy tests should be documented. Contraception Female patients must be provided with comprehensive information on pregnancy prevention and should be referred for contraceptive advice if they are not using effective contraception. As a minimum requirement, female patients at potential risk of pregnancy must use at least one effective method of contraception. Preferably the patient should use two complementary forms of contraception including a barrier method. Contraception should be continued for at least 1 month after stopping treatment with isotretinoin, even in patients with amenorrhea. Pregnancy testing According to local practice, medically supervised pregnancy tests with a minimum sensitivity of 25mIU/mL are recommended to be performed in the first 3 days of the menstrual cycle, as follows. Prior to starting therapy: In order to exclude the possibility of pregnancy prior to starting contraception, it is recommended that an initial medically supervised pregnancy test should be performed and its date and result recorded. In patients without regular menses, the timing of this pregnancy test should reflect the sexual activity of the patient and should be undertaken approximately 3 weeks after the patient last had unprotected sexual intercourse. The prescriber should educate the patient about contraception. A medically supervised pregnancy test should also be performed during the consultation when isotretinoin is prescribed or in the 3 days prior to the visit to the prescriber, and should have been delayed until the patient had been using effective 2

4 contraception for at least 1 month. This test should ensure the patient is not pregnant when she starts treatment with isotretinoin. Follow-up visits Follow-up visits should be arranged at 28 day intervals. The need for repeated medically supervised pregnancy tests every month should be determined according to local practice including consideration of the patient s sexual activity and recent menstrual history (abnormal menses, missed periods or amenorrhea). Where indicated, follow-up pregnancy tests should be performed on the day of the prescribing visit or in the 3 days prior to the visit to the prescriber. End of treatment Five weeks after stopping treatment, women should undergo a final pregnancy test to exclude pregnancy. Prescribing and dispensing restrictions Prescriptions of isotretinoin for women of childbearing potential should be limited to 30 days of treatment and continuation of treatment requires a new prescription. Ideally, pregnancy testing, issuing a prescription and dispensing of isotretinoin should occur on the same day. Dispensing of isotretinoin should occur within a maximum of 7 days of the prescription. Male patients: The available data suggest that the level of maternal exposure from the semen of the patients receiving isotretinoin, is not of a sufficient magnitude to be associated with the teratogenic effects of isotretinoin. Male patients should be reminded that they must not share their medication with anyone, particularly not females. Additional precautions Patients should be instructed never to give this medicinal product to another person, and to return any unused capsules to their pharmacist at the end of treatment. Patients should not donate blood during therapy and for 1 month following discontinuation of isotretinoin because of the potential risk to the foetus of a pregnant transfusion recipient. Educational material In order to assist prescribers, pharmacists and patients in avoiding foetal exposure to isotretinoin the Marketing Authorisation Holder will provide educational material to reinforce the warnings about the teratogenicity of isotretinoin, to provide advice on contraception before therapy is started and to provide guidance on the need for pregnancy testing. Full patient information about the teratogenic risk and the strict pregnancy prevention measures as specified in the Pregnancy Prevention Programme should be given by the physician to all patients, both male and female. Psychiatric disorders Depression, depression aggravated, anxiety, aggressive tendencies, mood alterations, psychotic symptoms, and very rarely, suicidal ideation, suicide attempts and suicide have been reported in patients treated with isotretinoin (see section 4.8). Particular care needs to be taken in patients with a history of depression and all patients should be monitored for signs of depression and referred for appropriate treatment if necessary. However, discontinuation of isotretinoin may be insufficient to alleviate symptoms and therefore further psychiatric or psychological evaluation may be necessary. 3

5 Skin and subcutaneous tissues disorders Acute exacerbation of acne is occasionally seen during the initial period but this subsides with continued treatment, usually within 7-10 days, and usually does not require dose adjustment. Exposure to intense sunlight or to UV rays should be avoided. Where necessary a sun-protection product with a high protection factor of at least SPF 15 should be used. Aggressive chemical dermabrasion and cutaneous laser treatment should be avoided in patients on isotretinoin for a period of 5-6 months after the end of the treatment because of the risk of hypertrophic scarring in atypical areas and more rarely post inflammatory hyper or hypopigmentation in treated areas. Wax depilation should be avoided in patients on isotretinoin for at least a period of 6 months after treatment because of the risk of epidermal stripping. Concurrent administration of isotretinoin with topical keratolytic or exfoliative antiacne agents should be avoided as local irritation may increase (see section 4.5). Patients should be advised to use a skin moisturising ointment or cream and a lip balm from the start of treatment as isotretinoin is likely to cause dryness of the skin and lips. There have been post-marketing reports of severe skin reactions (e.g. erythema multiforme (EM), Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN)) associated with isotretinoin use. As these events may be difficult to distinguish from other skin reactions that may occur (see section 4.8), patients should be advised of the signs and symptoms and monitored closely for severe skin reactions. If a severe skin reaction is suspected, isotretinoin treatment should be discontinued. Eye disorders Dry eyes, corneal opacities, decreased night vision and keratitis usually resolve after discontinuation of therapy. Dry eyes can be helped by the application of a lubricating eye ointment or by the application of tear replacement therapy. Intolerance to contact lenses may occur which may necessitate the patient to wear glasses during treatment. Decreased night vision has also been reported and the onset in some patients was sudden (see section 4.7). Patients experiencing visual difficulties should be referred for an expert ophthalmological opinion. Withdrawal of isotretinoin may be necessary. Musculo-skeletal and connective tissue disorders Bone changes including premature epiphyseal closure, hyperostosis, and calcification of tendons and ligaments have occurred after several years of administration at very high doses for treating disorders of keratinisation. The dose levels, duration of treatment and total cumulative dose in these patients generally far exceeded those recommended for the treatment of acne. Benign intracranial hypertension Cases of benign intracranial hypertension have been reported, some of which involved concomitant use of tetracyclines (see section 4.3 and section 4.5). Concomitant treatment with tetracyclines should therefore be avoided. Signs and symptoms of benign intracranial hypertension include headache, nausea and vomiting, visual disturbances and papilloedema. Patients who develop benign intracranial hypertension should discontinue isotretinoin immediately. 4

6 Hepatobiliary disorders Liver enzymes should be checked before treatment, 1 month after the start of treatment, and subsequently at 3 monthly intervals unless more frequent monitoring is clinically indicated. Transient and reversible increases in liver transaminases have been reported. In many cases these changes have been within the normal range and values have returned to baseline levels during treatment. However, in the event of persistent clinically relevant elevation of transaminase levels, reduction of the dose or discontinuation of treatment should be considered. Renal insufficiency Renal insufficiency and renal failure do not affect the pharmacokinetics of isotretinoin. Therefore, isotretinoin can be given to patients with renal insufficiency. However, it is recommended that patients are started on a low dose and titrated up to the maximum tolerated dose (see section 4.2). Lipid Metabolism Serum lipids (fasting values) should be checked before treatment, 1 month after the start of treatment, and subsequently at 3 monthly intervals unless more frequent monitoring is clinically indicated. Elevated serum lipid values usually return to normal on reduction of the dose or discontinuation of treatment and may also respond to dietary measures. Isotretinoin has been associated with an increase in plasma triglyceride levels. Isotretinoin should be discontinued if hypertriglyceridaemia cannot be controlled at an acceptable level or if symptoms of pancreatitis occur (see section 4.8). Levels in excess of 800mg/dL or 9mmol/L are sometimes associated with acute pancreatitis, which may be fatal. Fructose intolerance Roaccutane contains sorbitol. Patients with rare hereditary problems of fructose intolerance should not take this medicine. High Risk Patients In patients with diabetes, obesity, alcoholism or a lipid metabolism disorder or familial history of lipid metabolism undergoing treatment with isotretinoin, more frequent checks of serum values for lipids and/or blood glucose may be necessary. Elevated fasting blood sugars have been reported, and new cases of diabetes have been diagnosed during isotretinoin therapy. Microdosed progesterone preparations (minipills) may be an inadequate method of contraceptive during Roaccutane therapy. Impaired hearing has been reported in patients taking Roaccutane; in some cases, the hearing impairment has been reported to persist after therapy has been discontinued. Mechanism(s) and causality for this event have not been established. Patients who experience tinnitus or hearing impairment should discontinue Roaccutane treatment and be referred to specialized care for further evaluation (see Undesirable effects: Sensory disorders). 5

7 בסעיף 4.6 Pregnancy and lactation עודכן המידע הבא: Pregnancy is an absolute contraindication to treatment with isotretinoin (see section 4.3). If pregnancy does occur in spite of these precautions during treatment with isotretinoin or in the month following, there is a great risk of very severe and serious malformation of the foetus. The foetal malformations associated with exposure to isotretinoin include central nervous system abnormalities (hydrocephalus, cerebellar malformation / abnormalities, microcephaly), facial dysmorphia, cleft palate, external ear abnormalities (absence of external ear, small or absent external auditory canals), eye abnormalities (microphthalmia), cardiovascular abnormalities (conotruncal malformations such as tetralogy of Fallot, transposition of great vessels, septal defects), thymus gland abnormality and parathyroid gland abnormalities. There is also an increased incidence of spontaneous abortion. If pregnancy occurs in a woman treated with isotretinoin, treatment must be stopped and the patient should be referred to a physician specialised or experienced in teratology for evaluation and advice. בסעיף 4.7 Effects on ability to drive and use machines נוסף המידע הבא: A number of cases of decreased night vision have occurred during isotretinoin therapy and in rare instances have persisted after therapy (see section 4.4 and section 4.8). Because the onset in some patients was sudden, patients should be advised of this potential problem and warned to be cautious when driving or operating machines. Drowsiness, dizziness and visual disturbances have been reported very rarely. Patients should be warned that if they experience these effects, they should not drive, operate machinery or take part in any other activities where the symptoms could put either themselves or others at risk. בסעיף 4.8 Undesirable effects עודכן המידע הבא: Some of the side effects associated with the use of isotretinoin are dose-related. With the recommended dosage, the risk/benefit ratio is generally acceptable considering the severity of the disease. The side effects are generally reversible after altering the dose or discontinuation of treatment, however some may persist after treatment has stopped. The following symptoms are the most commonly reported undesirable effects with isotretinoin: dryness of the skin, dryness of the mucosae e.g. of the lips (cheilitis), the nasal mucosa (epistaxis) and the eyes (conjunctivitis). 9 בסעיף 4.9 Overdose עודכן המידע הבא: Isotretinoin is a derivative of vitamin A. Although the acute toxicity of isotretinoin is low, signs of hypervitaminosis A could appear in cases of accidental overdose. Manifestations of acute vitamin A toxicity include severe headache, nausea or vomiting, drowsiness, irritability and pruritus. Signs and symptoms of accidental or deliberate overdosage with isotretinoin would probably be similar. These symptoms would be expected to be reversible and to subside without the need for treatment. Evacuation of the stomach may be indicated in the first few hours after overdosage. בסעיף 5.2 Pharmacokinetic properties עודכן המידע הבא: Metabolism After oral administration of isotretinoin, three major metabolites have been identified in plasma: 4-oxo-isotretinoin, tretinoin, (all-trans retinoic acid), and 4-oxo-tretinoin. These metabolites have shown biological activity in several in vitro tests. 4-oxoisotretinoin has been shown in a clinical study to be a significant contributor to the activity of isotretinoin (reduction in sebum excretion rate despite no effect on plasma levels of isotretinoin and tretinoin). Other minor metabolites includes glucuronide conjugates. The major metabolite is 4-oxo-isotretinoin with plasma concentrations at steady state, that are 2.5 times higher than those of the parent compound. Isotretinoin and tretinoin (all-trans retinoic acid) are reversibly metabolised (interconverted), and the metabolism of tretinoin is therefore linked with that of isotretinoin. It has been estimated that % of an isotretinoin dose is metabolised by isomerisation. Enterohepatic circulation may play a significant role in the pharmacokinetics of isotretinoin in man. In vitro metabolism studies have demonstrated that several CYP enzymes are involved in the metabolism of isotretinoin to 4-oxo-isotretinoin and tretinoin. No single isoform appears to have a predominant role. Isotretinoin and its metabolites do not significantly affect CYP activity. Pharmacokinetics in special populations Since isotretinoin is contraindicated in patients with hepatic impairment, limited information on the kinetics of isotretinoin is available in this patient population. Renal failure does not significantly reduce the plasma clearance of isotretinoin or 4-oxoisotretinoin. 8

10 התווסף סעיף של :5.3 Preclinical safety data Acute toxicity The acute oral toxicity of isotretinoin was determined in various animal species. LD50 is approximately 2000 mg/kg in rabbits, approximately 3000 mg/kg in mice, and over 4000 mg/kg in rats. Chronic toxicity A long-term study in rats over 2 years (isotretinoin dosage 2, 8 and 32 mg/kg/d) produced evidence of partial hair loss and elevated plasma triglycerides in the higher dose groups. The side effect spectrum of isotretinoin in the rodent thus closely resembles that of vitamin A, but does not include the massive tissue and organ calcifications observed with vitamin A in the rat. The liver cell changes observed with vitamin A did not occur with isotretinoin. All observed side effects of hypervitaminosis A syndrome were spontaneously reversible after withdrawal of isotretinoin. Even experimental animals in a poor general state had largely recovered within 1 2 weeks. Teratogenicity Like other vitamin A derivatives, isotretinoin has been shown in animal experiments to be teratogenic and embryotoxic. Due to the teratogenic potential of isotretinoin there are therapeutic consequences for the administration to women of a childbearing age (see section 4.3, section 4.4, and section 4.6). Fertility Isotretinoin, in therapeutic dosages, does not affect the number, motility and morphology of sperm and does not jeopardise the formation and development of the embryo on the part of the men taking isotretinoin. 12 אין לשתות יינות או משקאות חריפים בתקופת הטיפול עם התרופה. תרופה זו עלולה לגרום לרגישות מיוחדת עם חשיפה לשמש (שיזוף מוגבר), על כן המנע מחשיפה לשמש ודאג להגנה מתאימה (בגדים ארוכים, כובע, משחות הגנה וכו'). נוסף סעיף של אזהרות מיוחדות בנשים: רואקוטן הוא תכשיר בעל סבירות גבוהה לפגוע בעובר (בשפה רפואית: התכשיר הוא טרטוגני). בנוסף, הוא מעלה את הסיכון להפלה. ולפיכך: אסור לך ליטול רואקוטן כאשר את בהיריון. אסור לך ליטול רואקוטן אם את מיניקה. ישנה סבירות גבוהה שהתרופה תעבור לחלב האם שלך ותפגע בתינוקך. אסור לך ליטול רואקוטן אם יש סיכוי שתיכנסי להיריון במהלך הטיפול או במשך חודש לאחר הפסקת הטיפול. נשים שיכולות להיכנס להיריון רשאיות ליטול רואקוטן בהינתן מספר תנאים מחמירים, בשל הסיכון למומים מולדים (נזק לעובר טרם היוולדו). להלן התנאים: תוכלי להשתמש ברואקוטן רק במקרים של אקנה חמור, אשר לא השתפר כתגובה לאף אחד מן הטיפולים האחרים באקנה, כולל אנטיביוטיקות וטיפולים עוריים. הרופא שלך הסביר לך על הסיכון למומים מולדים: את מבינה מדוע אסור לך להיכנס להיריון ומה עליך לעשות כדי למנוע כניסה להיריון. דנת עם הרופא המטפל שלך לגבי אמצעים למניעת היריון. הרופא המטפל יספק לך מידע לגבי מניעת היריון וייתכן שיפנה אותך לרופא מומחה אחר לצורך כך. חובה עליך להסכים להשתמש באמצעי יעיל אחד למניעת היריון, או רצוי אף שניים, הכוללים קונדומים או דיאפרגמה בתוספת קוטל זרע, במשך חודש לפני תחילת הטיפול ברואקוטן, במהלך הטיפול ברואקוטן ולמשך חודש אחרי הפסקת הטיפול. לפני תחילת הטיפול ברואקוטן, הרופא שלך יבקש ממך לבצע בדיקת היריון, אשר חייבת לצאת שלילית. חובה עליך להשתמש באמצעים למניעת היריון גם אם אין לך ווסת או אינך פעילה מינית (אלא אם כן הרופא החליט כי אין בכך צורך). חובה עליך להסכים להגיע לרופא אחת לחודש למעקב ולבצע בדיקות היריון נוספות על-פי החלטת הרופא המטפל שלך. ייתכן שתתבקשי לבצע בדיקת היריון 5 שבועות לאחר הפסקת הטיפול ברואקוטן. אסור לך להיכנס להיריון במהלך הטיפול ברואקוטן ובמשך חודש לאחר סיום הטיפול. ייתכן שהרופא שלך יבקש ממך (או מן האפוטרופוס שלך) לחתום על טופס שמאשר כי הוסברו לך הסיכונים, וכי את מוכנה לנקוט באמצעי הזהירות הנדרשים. מרשמים לנשים שיכולות להיכנס להיריון מוגבלים ל- 30 ימי טיפול. לצורך קבלת המשך טיפול נדרש מרשם חדש. אם בכל זאת נכנסת להיריון במהלך הטיפול ברואקוטן, או במהלך חודש לאחר סיום הטיפול, הפסיקי ליטול את התרופה מיד ופני לרופא המטפל. ייתכן שהרופא המטפל שלך יפנה אותך לרופא מומחה אחר להתייעצות. הרופא המטפל יספק לך "חוברת מידע למטופל ברואקוטן", הכוללת הסברים לגבי היריון ואמצעים למניעת היריון. אם החוברת טרם סופקה לך, אנא בקשי אותה מן הרופא המטפל. "רואקיוטן" הוא תכשיר טרטוגני ולפיכך הוא אסור בשימוש לכל אישה בגיל הפוריות. יחד עם זאת, במקרים חמורים של אקנה המשחית את עור הפנים וכאשר אין כל טיפול חלופי, רשאי הרופא לרשום "רואקיוטן" לנשים בגיל הפוריות. אולם, יש לציית לאמצעי הזהירות שלהלן ולשקול את הסיכונים הקיימים כנגד הפוטנציאל החיוני של הטיפול: 1. חובה על כל אישה בגיל הפוריות, העוברת טיפול ב"רואקיוטן" להשתמש ברציפות באמצעי יעיל למניעת היריון, חודש לפני טיפול, בתקופת הטיפול ועד ארבעה שבועות מיום הפסקת הטיפול. יש לבחון בקפידה את יעילות האמצעי למניעת היריון, לגבי כל מטופלת, בייחוד במחזור הראשון של נטילת תכשיר הורמונלי למניעת היריון. 2. כמו-כן יש להשתמש ברציפות באמצעים יעילים למניעת הריון במקרה של טיפול מתמשך (עם הישנות המחלה) ולהמשיך בנקיטת אמצעים אלה, עד לארבעה שבועות מיום סיום הטיפול. 3. אולם, אם למרות כל אמצעי הזהירות, התרחש הריון בתקופת הטיפול ב"רואקיוטן" ועד ארבעה שבועות מיום סיום הטיפול, קיים סיכון גבוה למום של העובר (בעיקר במערכת העצבים המרכזית, הלב וכלי הדם הראשיים). 11

13 לפני כל טיפול ב"רואקיוטן", על הרופא לבדוק כל אישה בגיל הפוריות ולוודא שאינה הרה. כמו-כן, יש למסור למטופלת מידע על כל אמצעי הזהירות שעליה לנקוט ועל הסיכונים האפשריים והתוצאות של הריון המתרחש במהלך הטיפול ב"רואקיוטן" או ארבעה שבועות מיום סיום הטיפול. נוסף סעיף של אזהרות מיוחדות בגברים: ככל הנראה, רואקוטן לא פוגע בזרע. ניתן למצוא רמות נמוכות מאוד של איזוטרטינואין (החומר הפעיל של התרופה) בנוזל הזרע של גברים שנוטלים רואקוטן, אולם רמות אלה הינן נמוכות מכדי לפגוע בעובר של בת זוגך. עליך לזכור כי אסור לך לחלוק את התרופה שלך עם אף אדם אחר, בייחוד לא עם נשים. נוסף סעיף של אזהרות לכל המטופלים: הטיפול ברואקוטן צריך להתבצע בהשגחת רופא עור. עליך ליידע את הרופא המטפל אם הינך חש/ה בשינוי מהותי במצבך הנפשי תוך כדי הטיפול ברואקוטן. לעיתים תכופות, רואקוטן מעלה רמות שומנים בדם, כגון: כולסטרול או טריגליצרידים. הרופא המטפל יבצע בדיקות דם למדידת ערכים אלה בדמך לפני תחילת הטיפול ברואקוטן, במהלך הטיפול ואחריו. אם רמות השומנים בדמך יוותרו גבוהות, ייתכן והרופא יקטין את המינון של רואקוטן או יחליט על הפסקת הטיפול. מומלץ לא לשתות משקאות אלכוהולים במהלך הטיפול ברואקוטן או לפחות להקטין את כמות המשקאות האלכוהולים שהינך צורך/ת בדרך-כלל כל עוד הינך מטופל/ת ברואקוטן. רואקוטן עלול לגרום לעלייה ברמות אנזימי כבד. הרופא המטפל יבצע בדיקות דם למדידת רמות אלה לפני תחילת הטיפול ברואקוטן, במהלך הטיפול ואחריו. אם רמות אנזימי הכבד יוותרו גבוהות, יתכן והרופא יקטין את המינון של רואקוטן או יחליט על הפסקת הטיפול. רואקוטן עלול לגרום לעליה ברמות הסוכר בדמך. במקרים נדירים, מטופלים הפכו להיות סוכרתיים. ייתכן והרופא המטפל ינטר את רמות הסוכר בדמך, בייחוד אם הינך כבר חולה בסוכרת, אם הינך סובל/ת מעודף משקל או אם הינך אלכוהוליסט/ית. ככל הנראה עורך יתייבש. מומלץ להשתמש בתכשירי לחות לעור ולשפתיים במהלך הטיפול. כדי להימנע מגירוי בעור, מומלץ להימנע מתכשירי ניקוי המקלפים את העור או מתכשירים נגד אקנה. יש להימנע מחשיפה מוגברת לשמש ומשימוש במנורות ובמיטות שיזוף. ייתכן ועורך יהיה רגיש יותר לשמש. לפני שהינך יוצא/ת לשמש, עליך להשתמש בתכשיר הגנה משמש עם מקדם הגנה גבוה (15 SPF או גבוה יותר). יש להימנע מטיפולים קוסמטיים כלשהם. רואקוטן עלול לגרום לעורך להיות שברירי יותר. יש להימנע מהסרת שיער באמצעות שעווה, מקילוף עור מכאני (שיוף עור) ומטיפולי לייזר בתקופת הטיפול ברואקוטן ובמשך 6 חודשים לפחות לאחר סיום הטיפול. טיפולים אלה עלולים לגרום לצלקות, לגירויים בעור, או במקרים נדירים, לשינויים בצבע העור. יש להפחית בתרגילים מאומצים ובפעילות גופנית. רואקוטן עלול לגרום לכאבי שרירים ומפרקים, בייחוד בילדים ובני נוער. אין ליטול תוספי תזונה המכילים ויטמין A במהלך הטיפול ברואקוטן. השימוש ברואקוטן ובתוספי ויטמין A במקביל עלול להגביר את הסיכון לתופעות לוואי. אין לתרום דם במהלך הטיפול ברואקוטן ובמשך 30 יום לאחר סיום הטיפול. אם אישה בהיריון תקבל את דמך, התינוק עלול להיוולד עם מומים מולדים. לעיתים חלה החמרה במצב האקנה בשבועות הראשונים של הטיפול. המצב משתפר לרוב עם התקדמות הטיפול. לפני תחילת הטיפול ובמשך הטיפול בתרופה זו יש לערוך בדיקות תפקוד הכבד ושומנים בדם. אם הינך חש/ה בשינוי מהותי במצבך הנפשי, עליך לדווח על-כך לרופא המטפל. קילוף עור עמוק והסרת שיער באמצעות שעווה אינם מומלצים בתקופת הטיפול ב"רואקיוטן" ועד לפחות שישה חודשים מיום סיום הטיפול. לעיתים חלה החמרה זמנית באקנה עם תחילת הטיפול. אם הינך רגיש/ה למזון כלשהו או לתרופה כלשהי ובמיוחד לויטמין A או רטינואידים, עליך להודיע על כך לרופא לפני נטילת התרופה. אין לתרום דם אלא לאחר 30 יום מתום הטיפול. 12

14 הסעיף תגובות בין תרופתיות עודכן כדלקמן: אם הינך נוטל/ת תרופה נוספת, כולל תרופות הנמכרות ללא מרשם ותוספי תזונה, או אם סיימת זה עתה טיפול בתרופה אחרת, עליך לדווח לרופא המטפל כדי למנוע סיכונים או אי-יעילות הנובעים מתגובות בין-תרופתיות, במיוחד לגבי תרופות מהקבוצות הבאות: טטראציקלינים (סוג של אנטיביוטיקה), גלולה למניעת הריון המכילה פרוגסטרון במינון נמוך כמרכיב בלעדי,(minipills) ויטמין A, תכשירים אחרים לאקנה או כל תכשיר אחר או מוצר קוסמטי העלול תכשירי ניקוי המקלפים את העור, תכשירי עור נגד אקנה ותכשירים או מוצרים קוסמטיים העלולים לייבש את העור, תכשירים המכילים אלכוהול. יחד עם זאת, מומלץ להשתמש בתכשירי לחות או בתכשירים המרככים את העור (קרמים או תכשירים לעור המונעת איבוד מים מן העור ובעלות אפקט מרכך על העור). הסעיף תופעות לוואי עודכן כדלקמן: בנוסף לפעילות הרצויה של התרופה, בזמן השימוש בה עלולות להופיע השפעות לוואי כגון: יובש בפה ובאף, כאבי שרירים, כאבי פרקים, דילול או נשירת שיער, רגישות לשמש (ראה/י לעיל), יובש בעיניים אשר עלול לגרום לקשיים בהרכבת עדשות מגע, קילוף עור, צרידות. בנוסף לפעילות הרצויה של התרופה, בזמן השימוש בה עלולות להופיע תופעות לוואי, אם כי לא כולם חווים אותן. תופעות אלו דועכות או פוסקות לרוב עם הפסקת הטיפול. הרופא המטפל יכול לסייע לך להתמודד איתן. תופעות לוואי שכיחות מאוד (מתרחשות ביותר מ- 1 ל- 10 מטופלים): בעיות בכבד ובכליות: רמות גבוהות של אנזימי כבד בבדיקות דם. בעיות הקשורות בדם: פציעות, דימומים או הופעת קרישי דם ביתר קלות (במקרים בהם יש השפעה על תאי קרישת הדם); אנמיה חולשה, סחרחורת, עור חיוור (במקרים בהם יש השפעה על תאי הדם האדומים); עלייה בסיכוי ללקות בזיהומים (במקרים בהם יש השפעה על תאי הדם הלבנים). תגובות אחרות: שינויים ברמות השומנים בדם (כולל HDL או טריגליצרידים). תופעות לוואי שכיחות (מתרחשות בשכיחות של עד 1 ל- 10 מטופלים): בעיות במערכת העצבים: כאבי ראש. בעיות אף אוזן גרון: יובש בחלק הפנימי של האף וכיסויו בקרום, תופעה הגורמת לדימום קל מן האף; כאבים או דלקות בגרון ובאף. תגובות אחרות: עלייה ברמות הכולסטרול בדם, הופעת חלבון או דם בשתן. תופעות לוואי נדירות (מתרחשות בשכיחות של עד 1 ל- 1,000 מטופלים): בעיות בעור ובשיער: נשירת שיער (התקרחות). תופעה זו היא בדרך כלל זמנית. שיערך אמור לחזור למצבו הרגיל לאחר סיום הטיפול. תופעות לוואי נדירות מאוד (מתרחשות בשכיחות של עד 1 ל- 10,000 מטופלים): בעיות בעור ובשיער: לעיתים חלה החמרה באקנה בשבועות הראשונים של הטיפול, אך התסמינים אמורים להשתפר עם התקדמות הטיפול; דלקות בעור, נפיחות בעור, עור כהה מן הרגיל, בייחוד באיזור הפנים; הזעת יתר או גרד; עלייה ברגישות לאור; זיהומים חיידקיים בבסיס הציפורניים, שינויים בציפורניים; נפיחות, הפרשות, מוגלה; הצטלקות מעובה לאחר ניתוח; עלייה בשיעור הגוף. בעיות במערכת העצבים: התכווצויות, נמנום, סחרחורות. בעיות הקשורות בדם: נפיחות בבלוטות לימפה. בעיות אף אוזן גרון: יובש בגרון, צרידות; לחץ פתאומי בחזה, קוצר נשימה וצפצופים, בייחוד אם את/ה סובל/ת מאסתמה; קשיים בשמיעה. סוכרת: תחושת צמא מופרזת, צורך תכוף במתן שתן, עלייה ברמות סוכר בבדיקות דם - כל אלו יכולים להיות סימנים לסוכרת. תגובות אחרות: תחושה כללית לא טובה, רמות גבוהות של חומצה אורית בדם, זיהומים חיידקיים, דלקת של כלי הדם (שמלווה לעיתים בפציעות ונגעים אדומים). הסעיף תופעות לוואי המחייבות התייחסות מיוחדת עודכן כדלקמן: קילוף או דלקת השפתיים, דלקת עיניים, דימום אף: הפסק/י הטיפול ופנה לרופא. שינויים בראייה, כאבי ראש חזקים, פריחה בעור (נדיר): הפסק הטיפול ופנה לרופא. דיכאון (נדיר): פנה/י לרופא לייעוץ. בכל מקרה שבו הינך מרגיש/ה תופעות לוואי שלא צוינו בעלון זה, או אם חל שינוי בהרגשתך הכללית עליך להתייעץ עם הרופא מיד. בעיות נפשיות: 13

15 14 תופעות נדירות: דיכאון או הפרעות נלוות. סימנים לכך כוללים תחושת ריקנות או עצבות, שינויים במצב הרוח, חרדה, פרצי בכי, רגזנות, אובדן היכולת ליהנות או לגלות עניין בפעילויות חברתיות או ספורטיביות, שינה מרובה מדי או מעטה מדי, שינויים במשקל או בתיאבון, ירידה בביצועים בבית- הספר או בעבודה, קשיים בריכוז. החמרה של דיכאון קיים. התפתחות של אלימות או תוקפנות. תופעות נדירות מאוד: ישנם מטופלים שפיתחו מחשבות על פגיעה בעצמם או על נטילת חייהם (מחשבות אובדניות), שניסו לסיים את חייהם (ניסו להתאבד), או סיימו את חייהם (התאבדו). ייתכן כי מטופלים אלה לא ייראו בדיכאון. התנהגות לא רגילה. סימנים של פסיכוזה: אובדן קשר עם המציאות, כגון: שמיעת קולות או ראיית דברים שאינם קיימים. אם הינך חווה סימנים של כל אחת מהתופעות הנפשיות המתוארות לעיל, עליך לפנות לרופא המטפל באופן מיידי. יתכן שהרופא יורה לך להפסיק ליטול רואקוטן, ויתכן כי הפסקת הטיפול ברואקוטן לא תביא להיעלמות התופעות: יתכן ותזדקק/י לעזרה נוספת והרופא שלךיכול לסייע בכך. תגובות אלרגיות (נדיר): תגובות חמורות (אנפילקטיות): קשיים בנשימה או בבליעה, הנגרמים כתוצאה מנפיחות פתאומית של הגרון, הפנים, השפתיים והפה; נפיחות פתאומית של הידיים, הרגליים והקרסוליים. תגובות אלרגיות כגון פריחה וגרד. אם פיתחת תגובה אלרגית חמורה, פנה/י מייד לקבלת סיוע רפואי דחוף. אם פיתחת תגובה אלרגית כלשהי, הפסק/י ליטול רואקוטן ופנה/י לרופא המטפל. בעיות בעור ובשיער: תופעות שכיחות מאוד: יובש של העור, בייחוד באיזור השפתיים והפנים; דלקות בעור; שפתיים סדוקות ומודלקות; פריחה; גירוד וקילוף קל בעור; עור שברירי ואדמומי יותר מהרגיל, בייחוד באיזור הפנים. יש להשתמש בתכשירי לחות מתחילת הטיפול. תופעות בשכיחות לא ידועה: פריחות קשות בעור multiforme) Stevens-Johnson,erythema,(toxic epidermal necrolysis,syndrome אשר עלולות לסכן חיים ועל-כן מחייבות התערבות רפואית מיידית. הן מופיעות בהתחלה ככתמים מעוגלים עם שלפוחיות במרכזם, בדרך כלל על-גבי הזרועות והידיים או על הרגליים וכפות הרגליים, כאשר פריחות חמורות יותר עלולות לכלול גם שלפוחיות באיזור החזה והגב. תסמינים נוספים שעלולים להופיע כוללים זיהומים בעיניים או כיבים בפה, בגרון, או באף. פריחות חמורות עלולות להתפתח לקילוף נרחב של העור אשר עלול לסכן חיים. פריחות קשות אלו בדרך-כלל מתקדמות לכאבי ראש, חום וכאבים בגוף (תסמינים דמויי שפעת). אם פיתחת פריחה או תסמינים עוריים כמתואר לעיל, הפסק/י ליטול רואקוטן ופנה/י לרופא המטפל מיד. בעיות בכבד ובכליות (נדיר מאוד): עור צהוב או עיניים צהובות ותחושת עייפות - אלו יכולים להיות סימנים להפטיטיס (צהבת, דלקת של הכבד). קושי במתן שתן, נפיחות בעפעפיים, תחושת עייפות מופרזת - אלו יכולים להיות סימנים לדלקת בכליות. אם פיתחת תסמינים אלה, הפסק/י ליטול רואקוטן ופנה/י לרופא המטפל מיד. בעיות במערכת העצבים (נדיר מאוד): כאב ראש ממושך, המלווה בתחושת חולי (בחילה), הקאות ושינויים בראייה כולל טשטוש בראייה - אלו יכולים להיות סימנים ליתר-לחץ-דם תוך-גולגלתי שפיר( hypertension (benign intracranial, בייחוד אם רואקוטן נלקח בו-זמנית עם אנטיביוטיקות ממשפחת הטטראציקלינים: הפסק/י ליטול רואקוטן ופנה/י לרופא המטפל מיד. בעיות מעיים וקיבה (נדיר מאוד): כאבי בטן חמורים, עם או ללא שלשולים דמיים חמורים, המלווים בבחילות והקאות - אלו יכולים להיות סימנים לבעיות חמורות במעיים: הפסק/י ליטול רואקוטן ופנה/י לרופא המטפל מיד. בעיות עיניים: תופעות שכיחות מאוד: דלקת בעיניים ובאיזור העפעפיים, יובש וגירוי בעיניים. התייעץ/י עם הרוקח לגבי טיפות עיניים מתאימות. אם הינך מרכיב/ה עדשות מגע וסובל/ת מיובש בעיניים, ייתכן שתצטרך/י להרכיב משקפיים במקום העדשות. תופעות נדירות מאוד: פגיעה בראיית לילה; החמרה בעיוורון צבעים ובראיית צבעים; עליה ברגישות לאור- ייתכן ותחוש/י צורך להרכיב משקפי שמש כדי להגן על עיניך מפני אור שמש חזק מדי; בעיות ראייה אחרות, כולל: טשטוש ראייה, עיוות בראייה, ותחושת 'עננות' בעיניים (חוסר שקיפות בקרנית, קטרקט). אם את/ה מפתח/ת טשטוש ראייה, הפסק/י ליטול רואקוטן ופנה/י לרופא המטפל מיד. אם ראייתך נפגמת בכל אופן אחר, עליך לדווח על כך לרופא בהקדם האפשרי. עצמות ושרירים: תופעות שכיחות מאוד: כאבי גב; כאבי שרירים; כאבי מפרקים, בייחוד בילדים ובני נוער. תופעות נדירות מאוד: דלקת מפרקים; הפרעות בעצמות (עיכוב בגדילה, גדילה מופרזת ושינויים בצפיפות העצם); הפסקה בגדילתן של עצמות שעדיין נמצאות בתהליך של גדילה; הצטברות סידן

16 ברקמות רכות, כאבים בגידים, רמות גבוהות בדם של תוצרי פירוק שרירים אם הינך מבצע/ת פעילות גופנית נמרצת. בכדי להימנע מהחמרה של בעיות בעצמות ובשרירים, יש להפחית בפעילות גופנית נמרצת במהלך הטיפול ברואקוטן. בכל מקרה שבו הינך מרגיש/ה תופעות לוואי שלא צויינו בעלון זה, אם הינך מודאג מתופעות לוואי מסויימות, בין אם הן מוזכרות בעלון זה ובין אם לא, או אם חל שינוי בהרגשתך הכללית, עליך להתייעץ עם הרופא המטפל. הסעיף מינון עודכן כדלקמן: מינון לפי הוראות הרופא בלבד. אין לעבור על המנה המומלצת. המנה ההתחלתית המקובלת היא 0.5 מ"ג לק"ג משקל גוף ביום (0.5 מ"ג/ק"ג/יום), כך שאם הינך שוקל/ת 70 ק"ג, הטיפול שלך יתחיל בדרך-כלל במנה של 35 מ"ג ביום. יש להשתמש בתרופה זו בזמנים קצובים כפי שנקבע על-ידי הרופא המטפל. אם שכחת ליטול תרופה זו בזמן הקצוב, יש ליטול אותה מיד כשנזכרת, אולם אם מועד נטילת המנה הבאה קרוב - יש לדלג על המנה שנשכחה ולהמשיך בטיפול מן המנה הבאה ללא כל שינוי. בשום אופן אין ליטול שתי מנות ביחד כדי לפצות על מנה שנשכחה. שבועות מספר לאחר תחילת הטיפול, ייתכן שהרופא ישנה את המינון שלך, כתלות בתגובתך לטיפול. אם את/ה סבור/ה כי המינון של התרופה גבוה מדי או נמוך מדי עבורך, פנה/י לרופא המטפל. אם הינך סובל/ת מבעיות חמורות בכליות, תטופל/י כנראה במנה התחלתית נמוכה יותר (כמו: 10 מ"ג ביום), שתעלה באופן הדרגתי עד למנה הגבוהה ביותר שעימה גופך יכול להתמודד. אם גופך לא יכול להתמודד עם המנה המומלצת, ייתכן שתטופל/י במנה נמוכה יותר: משמעות הדבר היא שהטיפול יהיה ממושך יותר ושיש סיכוי גבוה יותר שהאקנה שלך יחזור. קורס טיפולי ברואקוטן נמשך בדרך-כלל בין 16 ל- 24 שבועות. מרבית המטופלים זקוקים לקורס טיפול אחד בלבד. ייתכן כי מצב האקנה שלך ימשיך להשתפר במשך עד 8 שבועות לאחר סיום הטיפול ברואקוטן, ולכן לרוב לא מתחילים קורס טיפולי נוסף עד חלוף 8 שבועות מתום הטיפול. לעיתים חלה החמרה במצב האקנה בשבועות הראשונים של הטיפול. המצב משתפר לרוב עם התקדמות הטיפול. הסעיף אופן השימוש עודכן כדלקמן: אין ללעוס! לבלוע את התרופה עם מים. יש ליטול התרופה עם הארוחה. אין ללעוס! יש לבלוע את הכמוסות בשלמותן, על קיבה מלאה, עם שתייה או עם כמות קטנה של אוכל. ניתן ליטול את הכמוסות פעם או פעמיים ביום. בסעיף כיצד תוכל/י לסייע להצלחת הטיפול? נוסף המידע הבא: רצוי להימנע משתיית יינות או משקאות חריפים בתקופת הטיפול בתרופה. יש להימנע מחשיפה מוגברת לשמש, להשתמש בתכשירי הגנה משמש עם מקדם הגנה גבוה (15 SPF או גבוה יותר) ולדאוג להגנה מתאימה (בגדים ארוכים, כובע וכדומה). מומלץ להשתמש בתכשירי לחות לעור ולשפתיים במהלך הטיפול ולהימנע מתכשירי ניקוי המקלפים את העור או מתכשירים נגד אקנה. יש להימנע מטיפולים קוסמטיים כלשהם, כגון: הסרת שיער באמצעות שעווה, קילוף עור מכאני (שיוף עור) וטיפולי לייזר בתקופת הטיפול ברואקוטן ובמשך 6 חודשים לפחות לאחר סיום הטיפול. יש להפחית בתרגילים מאומצים ובפעילות גופנית. בסעיף אחסנה עודכן המידע הבא: במקום קריר. יש לאחסן במקום יבש מתחת ל- 30 C ולהגן מאור. יש לשמור את הכמוסות בתוך הבליסטר (אריזת המגש) ובתוך אריזת הקרטון החיצונית, כדי להגן מאור ומלחות. 15
