The PREVENTION OF CRUELTY TO ANIMALS LAW (EXPERIMENTS ON ANIMALS) experiment Law, , regulates the issue of experiment on animals. The execution of the law is given in the hands of the Ministry of Health through the Council for Animal Experimentation. B. In the State of Israel, it is allowed to conduct experiments on animals only under this law and the regulations of the Council. C. You are requested to read the law and confirm this by your signature. D. The law states that no animal experiment be conducted if there are suitable alternatives for these experiments. You should confirm by your signature that there are no alternatives to animals in the proposed experiment. E. If the experiment involves the use of hazardous substances (biological, chemical or physical), you should obtain the approval of the Institutional Safety Committee. F. You should commit to conduct the experiments only in facilities approved by the Committee for Animal Experimentation (IACUC) at your Institution. G. Submission of an ethics application can be made only by the Principal Investigator. H. The user name and personal code of the Principal Investigator are personal and are not transferable. The Principal Investigator is responsible for submitting this application, even if someone else used his user name and code. By your signature on this form, you undertake to abide by the law, and conduct the experiment only in facilities approved by the Committee for Animal Experimentation (IACUC) in your institution and according with the Council s guidelines.

5 Rationale for choosing an animal species and strain. 1. The name of each strain should be specifically noted in this paragraph and in the upper table (especially mutants and genetically modified). 2. 7 Monitoring the animals during the experiment and definitions of Humane End Points (HEP) The measures taken for the examination and treatment of the animals during the experiment (frequency of clinical monitoring, weighing s, supportive care etc.) should be noted.
