3 MISSION OVERVIEW UGANDA December 5-13, 2015 Description: Participants will teach Bible doctrine to pastors, men, women, children and youth. Children s ministry will be much like a Backyard Bible Club. The team will work through local pastor, Alfred Adundo of Nile Baptist Church in Jinja in church planting activities, teaching children, men, women, youth and pastors. We will work in Jinja, Masindi, and possibly other locations as needed. Our team must be flexible, but in every location there is much to do. Training will be provided and in every location the goal will be to teach the Good news of the Gospel. Travel Details: Tentatively, we will depart from Columbus, Ohio and connect to a major international airport to continue our journey, landing in Entebbe, Uganda the following afternoon or early evening (depending on lay-overs). We will travel to Jinja where we will spend the first day and begin our ministry immediately. Anticipated Cost: Expense Breakdown: These are estimates only! This cost hinges on plane ticket prices. Airline tickets (Round trip), lodging, accidental death/ evacuation insurance, food, and travel (in country) $2, Every effort is being made to reduce this cost through a bidding process for airline tickets. Passports, inoculation process, souvenirs, refreshments while traveling or in-country are not included. If you are not self-funded, funds may be raised using the methods found on pages entitled Raising Short-Term Mission Support. Commitment Date: 9/20/15 Turn in application, $ deposit and the Participant Testimony form to the Team Leader. If you are raising funds, please commit as early as possible with your deposit and forms before sending support letters. Begin passport application process by mid- August so we can send away for Visas as soon as possible. Visas will be sent away the beginning of October. Begin inoculations as soon as possible. Health conditions that could prevent a participant from accepting this assignment: Inability to walk short distances. Factors that could influence the participant s ability to do this job: Participants should be in good health. Living conditions will be primitive. We will be staying in third-world hotels or guest houses. Health care facilities available: IMB Missionary doctors are available. 3

4 Items/Materials to Bring to the Field: A packing checklist specific to short-term missions is included on page 12. Possible Visa/Passport Restrictions: You will need to have a passport, which you will turn into your Team Leader so that we can send them away as a group to get our visa stamp. This will be at no additional cost to you. Training Requirements: Participants will be required to complete Child Protection Training, Safe Travel Training and other meetings as required by the Team Leader. 4

5 SHORT-TERM MISSION PROJECT POLICIES Living Hope Church Marysville, Ohio Purpose of Short-term Mission Projects To lead people to faith in Jesus Christ through the sharing of the Gospel. To impact the world through the living testimonies of participants and through the Word of God. To assist and encourage the ministry of the local missionaries and pastors. To assist your local church in being obedient to God s command expressed in the Great Commission (Matthew 28:19-20) To develop long-term, strategic relationships throughout the world. Short-Term Participant Selection Criteria All participants must have a clear testimony of salvation through Jesus Christ and be able to complete the Participant Testimony Form, as well as verbally express this testimony as needed. The participant should be at least 16 years of age unless otherwise approved by the Team Leader. Parental or guardian permission is required for participants who are under 18 years of age. Parents will be provided with a form that will need to be signed and notarized before the minor may travel. All participants 18 years and older will be required to complete a background check every 3 years. This is a requirement of the IMB and can be processed through Living Hope Church. If you have already had a background check within the past 3 years due to serving in another ministry area, other church, or work, you may submit a copy of the results to your Team Leader. Application for the Team Participants must complete the Living Hope Church Short-term Mission Team Application (included) and Participant Testimony Form and submit them to the Team Leader by the prescribed dates for review. The Team Leader may require interviews with potential participants in addition to the application. Short-term mission participants must agree to and comply with Living Hope Church Financial Policies. (see next section) Participant Requirements Participants on an Living Hope Church sponsored short-term mission trip are reminded that they are ambassadors for Christ. As teams go overseas, they not only represent Him, but their local church, Living Hope Church, the United States, and the supporting mission agency. This is a tremendous responsibility. For this reason, Living Hope Church asks that each participant seeks to be above reproach in his/her actions and attitudes. Participants should not have current issues in their life that would disqualify them from service in the church. The participant must submit to the Team Leader's authority. 5

6 Due to instability and anti-american sentiment in various countries around the world, Living Hope Church asks that participants refrain from expressing political opinions while overseas. The participant must adhere to the behavioral guidelines for each specific team as set by the team leadership or the missionary agency, with a mind toward the culture to which the team is going. This will require a servant attitude toward all nationals and participants, appropriate dress as stipulated by the Team Leader, along with the willingness to learn from the host culture. Each participant will need to complete the Safe Travel Training before going on mission. Living Hope Church will schedule a day-long training when participants can attend as a group with their Team Leader. If the participant cannot be present at that training, they will need to watch the entire video program on their own and submit their written work to the Team Leader. This training needs to be repeated every 3 years. Participants 18 years and older will need to complete Child Protection Training as required by the IMB. This consists of viewing a 30-minute video and completing a video response sheet that must be turned in to the Team Leader. This training needs to be repeated every 3 years. We are partnering with others from the International Mission Board, a Southern Baptist sending organization. The use of charismatic gifts such as tongues is strictly prohibited. It creates major problems for their ministry. Use of this prohibition will lead to your early departure at your extra expense. After consultation with the local church pastor and Living Hope Church, the Team Leader reserves the right to ask a problematic participant to return home if that participant s behavior is destructive to the team, the ministry, or the host community. Any additional costs incurred as a result of this action will be at the participant s expense. 6

7 Application for New Passport PASSPORT PROCEDURES Living Hope Church has almost everything you need to know about getting a passport, including information about where to get one and the ability to fill out the application online and print it out. (Do not sign it until you are before the acceptor, usually the Postmaster or Clerk of Court for the signing and witnessing of the application.) Applicant must submit the following along with the application: 2 Passport Photos (2 x 2 ) These photos may be taken at Walgreen s, photo shop, AAA, etc. Application Fee Please visit to see the current fee structure. Evidence of U.S. Citizenship or State Birth Certificate Birth Certificates must be State certified with a raised, embossed seal. The Passport Department will not accept a copy or hospital record of birth. Valid Form of Photo Identification (Driver s License) Photos, application fees, proof of U.S. citizenship, and your completed application will be collected by your local passport acceptor and forwarded to your nearest official passport office. Renewing Your Passport You can renew by mail if: Your most recent passport is available to submit and it is not damaged or altered You received the passport within the past 15 years You were over age 16 when it was issued You still have the same name, or can legally document your name change. Review the information provided by the U.S. Department of State, as well as print the renewal application form at NOTE: Your previous passport will be returned to you with your new passport. Applicant must submit the following: Your Most Recent Passport The issue date must be within fifteen years. 7

8 The passport must have been issued on or after the applicant s 16th birthday. Marriage Certificate or Court Order if Name has Changed Two Identical Passport Photos (2 X 2 ) These photos may be taken at Walgreens, photo shop, AAA, etc. Renewal Fee Please visit to see the current renewal fee structure. Do not apply by mail if: Your passport is older than 15 years old. You were a minor, (under 16 years old) when your passport was issued. Processing Time You should receive your passport within 4-6 weeks of receipt of completed application. If you need your passport more quickly, you may request Expedited Service which will get it to you within 3 weeks. To expedite your passport for guaranteed processing within three business days, there will be an additional fee of $60, plus overnight postage delivery costs for next day air both ways. Passport Agencies assist customers with urgent travel needs (generally if you are traveling within 2 weeks). One option is: What to Do When Your Passport Arrives Once you receive your passport, immediately sign your name in the space provided. It is not valid until you sign it. Keep it in a safe place! Do not pack in your suitcase once you leave for overseas because you must carry it on your person. Remember that a person without a passport is a person without a country. The Visa Process You will turn in your passport to your Team Leader so that we can send them away as a group to get our visa stamp. This is included in the cost of your trip. 8

9 IMMUNIZATIONS AND INOCULATIONS Required Vaccinations Yellow Fever: You will need to find an authorized U.S. yellow fever vaccination clinic! See the website below to find a clinic near you. In Columbus, you can go to: Travel Health Services 7219 Sawmill Road Suite 105C Dublin, Ohio Phone: Recommended Vaccinations The following vaccines may be recommended for your travel to Uganda. Living Hope Church is not responsible for the accuracy of the information below, but has provided it for informational purposes only based on the CDC recommendations. Discuss your travel plans and personal health with a health-care provider to determine which vaccines you will need. Routine Vaccinations, recommended if you are not up-to-date with routine shots such as, measles/mumps/rubella (MMR) vaccine, diphtheria/pertussis/tetanus (DPT) vaccine, etc. Hepatitis A, or immune globulin (IG), recommended for all unvaccinated people traveling to or working in countries with an intermediate or high level of hepatitis A virus infection where exposure might occur through food or water. Cases of travel-related hepatitis A can also occur in travelers to developing countries with "standard" tourist itineraries, accommodations, and food consumption behaviors. Hepatitis B, recommended for all unvaccinated persons traveling to or working in countries with intermediate to high levels of endemic HBV transmission and who might be exposed to blood or body fluids, have sexual contact with the local population, or be exposed through medical treatment, such as for an accident, and for all adults requesting protection from HBV infection. Typhoid vaccine, recommended for all unvaccinated people traveling to or working in East Africa, especially if visiting smaller cities, villages, or rural areas and staying with friends or relatives where exposure might occur through food or water. Meningococcal (meningitis), recommended if you plan to visit countries that experience epidemics of meningococcal disease during December through June. Rabies, recommended for travelers spending a lot of time outdoors, especially in rural areas, involved in activities such as bicycling, camping, hiking, or work. Also, children are considered at higher risk because they tend to play with animals and may not report bites. 9

10 Polio, recommended for adult travelers who have received a primary series with either inactivated poliovirus vaccine (IPV) or oral polio vaccine (OPV). They should receive another dose of IPV before departure. Malaria: Your risk of malaria may be high in all of Uganda, including cities. See your health care provider or health clinic for a prescription anti-malarial drug. In Uganda, Chloroquine is NOT an effective antimalarial drug in Uganda and should not be taken to prevent malaria in this region! 11 TRAVEL INSURANCE POLICY 1. Every participant is required to be covered with health insurance while traveling overseas as a missionary with Living Hope Church. Many domestic insurance carriers will not cover overseas travel. Check with your insurance company to make sure you will be covered while in an underdeveloped country. If your health insurance provider does not cover you out of the country, there are short-term policies that can be purchased relatively inexpensively. 2. Each participant will be provided with a $100,000 Accidental Death/Medical Evacuation policy through Gallagher, which is the provider that has a relationship with the IMB. 3. Living Hope Church will not be responsible for the participant s health care expenses at any time. Living Hope Church will not be responsible for medical or political evacuation to the U.S. or any other country. 4. Travel insurance is available for an additional charge and needs to be purchased within 10 days of the ticket purchase. Please see the insurance policy prior to purchasing to confirm valid cancellation reasons, such as illness, death in the family, act of terrorism, etc. Living Hope Church will assist the ill participant in any way we can, but we cannot assume monetary responsibility. 11

12 SHORT-TERM MISSION PACKING LIST Luggage: 2 Suitcases (Each filled with 1/2 personal items and 1/2 ministry supplies; 50lb. limit each) 1 Carry-On (approximately 22"x14"x9"; approx. 40lb limit check with the airline) Pack 2 ministry outfits and essentials (including medications) as luggage may arrive late! 1 Personal Item (i.e. large purse or briefcase) Ministry Items: Bible, Journal Small Backpack/Daypack (this is essential for carrying tracts, Bibles, water, etc.) Fanny Pack Teaching/Study materials Gifts for interpreters, pastors, friends (Suggestions: Bibles, bookmarks, devotional guides, Bible highlighters, pins, etc.) GIVE THEM DIRECTLY TO THE MISSIONARY! Clothing Items: Ministry clothing: shirts & slacks (men) Ministry clothing: shirts & skirts, shorts for under skirts (women) Pajamas/sweats 2 Travel outfits Underwear/socks Cushioned tennis shoes for lots of walking! (Hiking boots where appropriate) Toiletries/Medications: Over-the-counter medicines (pain reliever, vitamins, Imodium AD, Pepto-Bismol tablets) Prescription medications (plus Anti-Malarial, Cipro if needed) Antiseptic mouthwash (also good for rinsing toothbrush) Glasses and/or extra contact lenses, saline solution Deodorant Clothes line, clothes pins, Woolite (can also use large plastic bags to wash in) Razor, shaving cream Hair brush, comb Shampoo/conditioner (sealed in zip lock bag) Soap (in plastic soap container) Thongs/flip flops to protect feet in shower Toothbrush w/case, toothpaste Tissue packets or wet wipes, feminine products General Items: Breath mints/chewing gum Camera, extra film and batteries Small flashlight & batteries, travel alarm clock Electrical converter First aid kit (travel size), large safety pins 12

13 Hand wipes and sanitizer Snack foods (Granola bars/protein bars, nuts, trail mix) Zip Lock bags (lots of them!) Luggage locks (TSA approved) Money belt or other travel case for money, passport, etc... 14 PRAYER: THE KEY TO THE MISSION PROJECT Prayer is the key to the success of our mission endeavors, for unless the Lord goes before us to prepare the way, our way will be very difficult. Indeed, unless the Lord is within each of us, controlling us, our way is the wrong way. Consider this Prayer: Continue to seek God, but go beyond seeking Him, expect Him, and when He comes in power, yield to Him. - Martin Lloyd Jones Holy Father, bless this mission team. Let every promise of Your Word be abundant in their life as they prepare to personally fulfill your call to go to the ends of the earth. Prepare also the people You will bring into their lives as they walk obediently, in faith, to Your call. Spend Time in Prayer: Keep a prayer journal! (It will amaze you!) Fast for a specific period of time at regular intervals. (Check with your physician first.) Pray regularly for: Those who make up your prayer support team. Those God has appointed you to minister to on the field. 14

15 Those who make up your financial support team and for general fund raising. The other members of your team. Your field contact, missionary, or other local church pastor and for their ministry and encouragement. Intimacy with God so as to hear and learn all He has for you during this project. The whole team, especially yourself, to keep devotional time as a priority amidst hectic schedules. Your Team Leader and the wisdom he or she needs to make any necessary decisions. 15

16 RAISING SHORT-TERM MISSION SUPPORT: THE TEAM-BUILDING APPROACH Building a Support Team Each individual participant will be responsible for developing his/her own Support Team. The Support Team consists of: Prayer Supporters Encouragers Financial Contributors The concept is to develop a team, not merely to raise funds. The most important need for the participant is for the people to pray for him/her before and during the project. The Team The purpose of a team is to involve as many people as possible in the mission experience. The Team is a ministry through you to the team. (TEAM = goers + prayer support + senders) is a means by which others can share in the blessings of the ministry. (Phil. 4:17) is a Biblical approach. (2 Cor. 8:4) is a means by which others become world Christians through giving. You should seek people you want on your team, not those you think will contribute financially. Remember, if the prayer support is in place, everything else will fall into place. Raising Funds If an individual cannot pay for their trip, the most effective, proven method to raise funds is to write a letter for distribution to friends, family, co-workers, and church-members. Support should be raised both from within and outside of your local church. Professional fundraisers for large organizations will tell you that people will give to people they know. Potential contributors are happy to have the opportunity to invest in someone they know personally or to an endeavor they care about. Preparing the Team List Usually between 50 and 100 names Not all members of your local church (50% from outside the church) Friends, relatives, coworkers, companies, Christian contacts Writing the Letter You are asking people to support the ministry of Living Hope Church. If you are asking for financial contributions, you are asking them to help with the costs you incur by conducting the 16

17 ministry such as transportation, meals while on the field, in-country housing, visa, travel insurance, etc. A common testimony for participants is that while they were initially hesitant about the idea of sending letters, the experience turned out to be a highlight of the entire trip. The following guidelines will assist you in writing your letter: There are three types of letters: A letter sent to ask for prayer support only. (If you are paying your own way) A letter sent to ask for prayer and financial support. A letter sent to ask for project support. (If you are paying your own way) Compose your own letter: It is unwise to copy from a sample letter. The last thing a donor wants is four copies of the same letter with four different signatures. People prefer to read a personal letter. Keep the letter brief (one page). Give a copy to your Team Leader before you mail it out. Include the following information in the letter: 1. Information about the country and the culture to which you are going. 2. Information about the sending organizations: church, host organizations, IMB, etc. 3. Description of what the team will be doing and why. 4. The cost of the trip. (The same for each individual member.) 5. The date you need the money. 6. A recommended giving amount. ($25.00, $50.00, or $100.00) 7. A description of why you want to be involved in the short-term trip, including reasons for going about which you feel the strongest. 8. How you would like the recipient to be involved. (prayer, financially, both) 9. Include a copy of the Living Hope Church Donation Policy with every letter. 10. Include a response card for them to return to you. 17

18 Include the following Donation Policy with each letter: Living Hope Church Donation Policy All contributions to Living Hope Church are non-refundable. To be tax-deductible you must: 1. Make checks and money orders payable to Living Hope Church. 2. Do not write an individual s name on the envelope or the check. (Including the memo line). 3. Enclose a note stating which individual you are supporting. DO NOT attach the note to the check. You may write the project/trip name on the memo line. 4. Contributors with a single gift of $ will receive acknowledgments from Living Hope Church. Other gifts of smaller amounts will be the responsibility of the individual giver to keep track of for their tax deductions. 5. Mail the check directly to the participant you are supporting. IRS regulations prohibit our finance office from giving you tax credit for donations designated for individuals. Contributions received which designate persons will be deposited, but your annual contribution statement will not be credited. All contributions will support the mission ministry of the Living Hope Church Send the Letter: Mail the letter, response card, Living Hope Church Donation Policy, and a self-addressed envelope to the list of potential supporters by the deadline date set by the Team Leader. Living Hope Church does not supply postage, envelopes, or copies of letters. Processing Your Donations: Participants must complete a Donation Summary form and then place all checks/cash in the Missions Envelope with the donor name, check number (if applicable) and amount noted on the outside. This should be given directly to the Team Leader who will count the money and both the participant and the Team Leader will sign their names that they agree upon the amount in the envelope. The Team Leader will then turn in the money to the Living Hope Church Financial Director who will also sign off on the amount. The Missions Envelope (or a copy) will be returned to the Team Leader after processing. It is the participant s responsibility to keep accurate records of financial supporters/donations. The burden of proof will lie with the participant should a discrepancy arise. Follow up: Following the trip, each participant must send a follow-up letter to each person who supported him/her on the trip, whether by prayer, finances or encouragement. The letter should detail what happened on the project and how it affected them. It is discouraging to support someone on a mission trip and never hear from that person again. By sharing your experiences with your support team, you continue to spread the burden and heart for world missions. Please give a copy of your thank you letter to your Team Leader. 18

19 MISSION TRIP FINANCIAL OBLIGATIONS 1. Every participant is responsible for raising 100% of their financial support for their mission trip. Financial checkpoints are set by the Team Leader to give you goals in your fundraising. Though the funds do not always come at the times specified, there are two minimum deadlines that cannot change: You must have enough funds turned in to offset the airline ticket or transportation expense by the time the reservation is made. If you have not turned in enough funds to pay for the ticket, no reservation will be made in your name. You must have all the required mission trip funds turned in 5 days prior to departure. 2. Participants who are partnering with Living Hope Church from another church may make deposits through their local church. The church can then forward the funds to Living Hope Church. It will be up to the participant to keep a record of each support contribution. What Happens If You Do Not Raise Enough Funds? The funds you raise or give are tax-deductible and are given for the mission project. Federal law mandates that they cannot be returned to you or any other donor. The funds can be set aside in your name for a future mission project. Living Hope Church will hold those funds under future trips in your name for a period of one year from the initial mission trip departure date. After that, they will be used for the field ministry that they were originally intended. Unused airline tickets are in the name of the participant and any alternate arrangements must be handled directly with the airline carrier by the participant. Most carriers allow you to reissue the ticket within one year if you cancel your seat before the flight leaves and pay their change fee, but this would be the sole responsibility of the participant. Living Hope Church is not responsible for monies lost due to insufficient trip funds or trip cancellations. What Happens If You Raise More Funds Than Are Needed? Funds raised in excess of the trip cost can either be set aside in your name for a future mission project for a period of one year, directed to another participant that has not raised all the needed funds, or given to the field ministry where the team will be serving. 19

25 PARTICIPATION AGREEMENT Risk Assumption Form for Short-Term Volunteer Missions In consideration for participating in Living Hope Church short-term mission trips, I agree to release, discharge, and hold harmless Living Hope Church; their employees, agents, and members from any and all claims or demands due to personal injury, illness or death as well as any and all property damage sustained of any nature which may be incurred by me, whether in foreign or domestic territory. I am aware of the hazards and risks to my person and property associated with serving in a mission capacity, such hazards and risk including, but not being limited to: death or injury by accident, disease, war, terrorist acts, weather conditions, inadequate medical services and supplies, criminal activity, and random acts of violence. I accept my assignment with full awareness of these risks. I also agree to be directed by and responsible to the designated mission leadership for the project. I agree to accept all risks subject only to any insurance coverage that may be available to me. I attest that I have no medical condition that would prevent me from performing my duties. I have carefully read and understand the contents of this "Participation Agreement" and sign this release as a voluntary act of my free will. I hereby authorize Living Hope Church or its representatives to initiate any medically necessary care on my behalf in the event of my incapability to present myself for such care and agree to be financially responsible to any care provider and authorize the release of any necessary medical or insurance related information pertinent to the circumstances. This is a legal document and I understand that I have the opportunity to consult with an attorney before signing it. This agreement shall remain in force until I rescind it in writing and that rescinding is signed by an officer of the church and is filed at the church office.
