1 PARENT S INFORMATION SHEET EVENT: 2012 CREATION FESTIVAL DATE: JUNE 26 JULY 1, 2012 TIME: Depart 6:00 a.m. (Jun 26) 9:00 p.m. (Jul 1) DESCRIPTION: The CREATION Festival is a time spent in God s Word, in worship, in prayer and a time of encouragement for us to examine our walk with Christ. The festival is held in Mt. Union, Pennsylvania at the Agape Farm. The youth will be challenged by God s Word, introduced to Christian music and encouraged to grow in their faith serving Christ and His bride, the church, at this 4-day music festival. CREATION Festival is the nation s largest Christian Music Festival with upwards of 70,000 people in attendance. The festival offers over 50 of the nation s top Christian music artists and over 15 world-class speakers. Each year thousands of young people place their faith in Jesus as Savior and even more make decisions to re-prioritize their lives and live for Jesus at the CREATION Festival. We will enjoy Christ-centered fellowship, teaching and music together during this event. This is a camping event, in which we will sleep in tents, eat camp food, and may be subject to inclement weather. 2 CREATION Music Festival 2012 DATES: June 26 July 1 COST: $220 (including the $100 deposit) Falmouth Baptist Church is going to the 2012 CREATION Music Festival! This is a 6-day trip to Mount Union, Pennsylvania. CREATION is a camping experience in which we tent overnight for 5 nights. The CREATION Music Festival is the largest music festival in the United States with upwards of 70,000 people attending each year. The festival offers over 50 of the nation s top Christian bands/artists and more than 15 nationally-known speakers. Our group camps together and will enjoy morning devotions, morning and evening worship and messages as well as a speaker of their choice during the daytime. Students & adults will attend speakers and concerts using the buddy system with regularly scheduled check-in times throughout each day. Our purpose in attending the CREATION Music Festival is both to hear from God s word being challenged throughout each day with messages from great speakers that proclaim the Truth and to introduce or encourage our students to listen to Christian music that will help build them up in their faith and encourage a Christian worldview. I further agree to hold Falmouth Baptist Church and its agents harmless and to indemnify them against all losses, liability, claims, or expenses including fines, penalties, and attorney's fees due to any alleged injury to my child incurred in connection with the operation of the trip and transportation thereto and therefrom. I also assume the responsibility of any damages caused or expenses incurred by my child. I grant to Falmouth Baptist Church, its representatives and employees the right to take photographs, video and/or audio recordings of my child(ren) and my property in connection with the above-identified subject. I authorize Falmouth Baptist Church, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Falmouth Baptist Church may use such photographs for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. I understand that no other personal information such as my child s name or other personal identifier will be used and my child s photograph, video, or audio recording will be removed from any form of media immediately upon request. Falmouth Baptist Church values the opportunity to feature our church family in various media/publications; however we respect that some parents may not wish to have themselves or their child appear in any church-related publication. We will honor your instructions when planning our publications. I further give permission for the chaperone(s) to send my child home via bus, taxi or public transportation at my expense if he/she is determined by the chaperone(s) to have disobeyed instructions, or participated in any illegal, immoral, or improper conduct. 6 CREATION 2012 SPONSORSHIP FORM Sponsors may pledge a per-car rate or a flat rate (as seen below). The total cost of the CREATION 2012 trip is $220. The Free Car Wash is on Saturday, May 26, 2012, from 10a.m. till noon, at Murray & MacDonald Insurance Services Inc. 406 Jones Rd. (next to Stop & Shop). Sponsorship is on an individual basis.
