2 About the organization This logo symbolizes the objectives of the organization. Ÿ Ÿ Ÿ The words in the outer circle are from the great Indian epic "Mahabharat", saying that "nothing is above a Human". This is also the motto of the organization. The triangle in the inner circle symbolizes the hands of three people and stands for community development through participation. The light from the lamp in the small hut in the centre symbolizes the development of the weakest and poorest person of the community. Founder trustees of the organization were inspired by Gandhian thinking and work of great men like Albert Schweitzer. They felt deeply the agony and hopelessness of poor villagers. They saw the plight of villagers and felt a need of medical services in these villages. Hence they brought like minded friends together and founded Gram Seva Trust, an organization dedicated to rural health and development. In 1994 the trust started a 30 bedded hospital with 5 staff members in an old dilapidated building, given by another trust. As the need arose the hospital was expanded to accommodate more patients and better services. Today after 20 years the hospital can accommodate 80 patients and has all basic facilities required in a rural hospital providing health services at affordable rates and sometimes free of charge to the needy from nearly 200 surrounding villages of Navsari and Dang districts. The organization also wanted to improve health of the surrounding villages hence as and when need was identified different community projects were started in the surrounding villages with main focus on health and development of women and children. Immense efforts are put in to decrease malnutrition. Gram Seva Trust also reaches out to the tribals in deep interior villages of Vansda and Dang through satellite centers and general medical mega camps arranged every year. The organization has also started different projects for development of villages like Self Help Group project (a micro saving and micro lending project for women), trainings for income generation activities, adolescent girl project, tutorial classes to strengthen primary education and a Child Education and Development Centre to improve standard of education in children of landless labourers. 3 Annual Report : Gram Seva Trust, Kharel Editorial The organization has completed 21 years and entered its 22nd year of health and development services. It is very heartening to present to you, the annual activities and financial report for the year This report is concise as the 20 year report was also published this year. The Service NGO project for maternal and child services in interior villages of Vansda was started with support from National Health Mission. The organization has also continued the sanitation project to build toilets in surrounding villages. Our satellite centre at Mahal was started five years ago in an office belonging to the District Water Department. This year we had to close the centre as the department needed their office for internal use. The behaviour of the government officials was disheartening but such experiences are a part of public services. Behaviour Change Communication 2 SNGO Mankunia Project After successfully completing SNGO project in Chikhli block for 6 years, District Health Department asked us to adopt few villages in Vansda block as they were lacking in maternal and child health indicators. The 13 villages under Mankuniya and 2 under Ankalacch PHC were selected in this project because they were the most interior and remote villages of Navsari district. The villages were situated on the border of Gujarat and Maharashtra; houses were scattered over a large geographical area made up of hills and forest, and hence this population remained underserved and services were underutilized. Poverty, illiteracy, poor hygiene & sanitation, water scarcity during summer, large family size, less use of contraception, teenage marriages and pregnancies, superstitions and strong hold of traditional healers, etc contributed to poor health indicators in this area. Core issues identified by base line survey, focus group discussions, secondary data from Anganwadi workers, PHC and Field NGOs were as described below: 1. High maternal mortality and infant mortality 2. Low percentage of complete Ante-natal Care 3. Low couple protection rate 4. High prevalence of reproductive tract infections 5. High percentage of under-nutrition in 0-5 years of children 6. High proportion of LBW in newborn babies Keeping these issues in view, GST proposed to provide preventive and curative services to these villages with the following objectives 1. To increase complete and qualitative Ante-Natal Care from 28% to 60% 2. Early detection of all high risk pregnant women and 100% safe institutional delivery of them 3. To decrease malnutrition in 0-3 year children by 10 % The following strategies have been selected to achieve the above mentioned objectives 1. Service delivery 2. Regular follow up of ANC, PNC and 0 to 3 year 5. Human Resource development and Capacity building 6. Monitoring and Evaluation 7. Reporting Thus this project was started to improve the maternal and child health in 13 villages of Mankuniya and 2 villages of Ankalacch PHC by providing clinical health services to remote interior areas and social development activities in collaboration with community (through village committees and PRIs), district/block level government health staff, ICDS department, traditional and private health care providers and other community based organizations. The project was inaugurated on 2nd December 2014 by the Navsari District Collector Smt.Remya Mohan, District Development Officer Shri Chandanani and the District Health Department in presence of local community leaders and villagers. Sanitation Project (Phase 2) Health and Hygiene always go hand in hand. Yet 46% of the rural households don t have any toilets. The organization started helping families to build toilets as a part of its sanitation project. This year 38 toilets were built by raising funds from the government schemes, families themselves, funding agencies and voluntary donors. Thus this project is based upon collective participation of different stakeholders. Permanent Projects Hospital Services Hospital The hospital continues to serve a population of over 300 villages of Gandevi, Vansda, Chikhli, Mahuwa and Dang in medicine, paediatrics, gynaecology, ophthalmology, surgery, orthopaedic, skin, ent, nephrology, dental, physiotherapy services. Annapurna Bhojnalay Our Annapurna Bhojnalay provides free food to all

5 Annual Report : Gram Seva Trust, Kharel Health Education and Access to Services is the need of the community 17 year old Ranjan was resident of Kharoli Village on the borders of Chikhli Block about 15 km away from Kharel. Ranjan s family was poor and she along with her parents worked as farm labourers for their daily wages. While visiting her village our field workers met Ranjan and found her very pale. On further questioning she affirmed that she felt weak, tired easily and was sometimes breathless. Our workers informed Ranjan and her parents that these were signs of Anaemia and she needed immediate medical check up and treatment. Her parents were not able to afford even minimum expenses for Ranjan s treatment. Seeing their financial condition our workers assured them that her check up and treatment shall be done free of charge. They will have to bear only the travel expenses to the Hospital at Kharel. At the Hospital Ranjan s laboratory tests showed that she suffered from severe iron deficiency anaemia due to poor nutritional intake. Her Haemoglobin was just 5gm%. Ranjan was counselled to take iron tablets daily for one year and come every month for checkup at the hospital. She was assured free treatment for the whole year. Ranjan diligently completed her treatment for one year. After one year her Haemoglobin had increased to 12.5gm%. Today she is healthy and happily married. Timely detection by field workers, their counselling and follow up, free treatment for one year by the organization and Ranjan s compliance saved further complication and ill health for a young adolescent girl. patients in general ward and their attendents. Food is also served at low price to other patients and their relatives. This year free dishes were served in all. Fund Relief Our Motto has been that no man should be without health services for lack of money. We treat patients and their illness no matter what caste, community or economic back ground. Even after providing services at very affordable rates, many patients have to be given complete or partial relief as they cannot pay those bills. More than 3600 patients attended these eye camps. Details are given in table below. Dialysis Our Dialysis unit serves chronic kidney disease patients. Many patients require 2 to 3 dialysis sessions pr week. A total of 1399 sessions were conducted this year. Out of which 1103 sessions were free. Mamtaghar Mamtaghar caters to pregnant women who have some risk or who live in interior areas and transportation is a problem. Such women can stay in Mamtaghar 8 to 10 days prior to their deliveries. They and their attendant are provided free breakfast, lunch and dinner. Pregnant women are also given high calorie snack at 4 p.m. This year 1021 pregnant women benefitted from this facility. 6 Gram Seva Trust, Kharel Annual Report : Blind Faith Lataben was a resident of Hond village in Chikhli block of Navsari Dsitrict. Her family was educated and economically well off. Lataben suffered from pain in abdomen one day and noticed that her abdomen was increasing in size. She was taken to a bhagat ( traditional healer) as per the tradition of their family. After treatment by the Bhagat for few days when there was no improvement they consulted a private practitioner. But without completing the treatment they again went back to the Bhagat, who advised branding with hot iron bars as the treatment. Lataben was inhumanly branded on the abdomen. One day Lataben was brought to our hospital in Kharel in severe pain and in critical condition. After treatment for few days she started improving and was later discharged. Lataben was saved but even today our middle class is chained to the old traditions and beliefs leading to preventable complications in patients. Camps We conduct general camps in interior areas every year. This year three camps were conducted as shown in table no.3 Community Health Projects Child Health and Nutrition Project: Malnutrition is a major health problem in children in rural areas.our Child Health and Nutrition Project caters to the health needs of children of 101 anganwadis in surrounding areas. Children are checked routinely by medical officers and followed by field workers during home visits. Health education is given to parents, family members and community regarding child care, nutrition, immunization etc. Hospital provides referral services free of charge to all ill children in this project. Antenatal Care: Though pregnancy is a physiological process, care is required during pregnancy so that both mother and child are healthy. Hence free antenatal services are provided to pregnant women in the hospital thrice a week and then the pregnant women are followed at home by community health workers. They promote regular antenatal check- ups and safe institutional deliveries. All high risk pregnant women are followed regularly to prevent any maternal mortality. Adolescent Health Adolescence is a period of rapid physical and mental changes in our body. The adolescents are prone to many health problems like anaemia, thyroid deficiency, depression, addiction, teenage pregnancy, suicides etc if they are not given proper care and support. We try to conduct meetings with adolescents in villages and talk to them about health care, hygiene, nutrition, effects of addiction, teenage pregnancies etc. Those requiring medical care are referred to the hospital for medical services. Increasing health awareness in the community is one of the main components to improve health in rural areas. The following sessions were conducted in this year. Community Development Projects Education Project To improve the formal education in villages we started tutorial classes in villages for children of farm labourers. These helped strengthen their primary education and decrease the school drop outs. We are having difficulty in finding educated volunteers interested in conducting such tutorials. Last year few tutorials had to be closed. This year efforts are being made to start these tutorials again and some more in other villages. Parodh Child Education and Development Centre( Balawas) Many families in rural areas migrate in search of livelihood to towns and cities for nearly eight months in a year. They take their children them depriving them of education. We started a child education and development centre, a residential centre for children whose parents migrate, orphans or who come from broken families. At present 36 children are staying with us. With the help and support of donors and well wishers we are also building a facility for 100 such children, which will be completed soon. There was sales of Rs.5,29,271/- of handicraft and household toiletries made by the local women. The SHGs started by us are being merged with the sakhi mandals formed by the government in villages so that the groups benefit from the schemes made for sakhi mandals. Gram Seva has been selected to train sakhi mandals in villages and empower them with skills for good management of their group activities. This year trainings to total 84 trainings were conducted in which 3780 members of 252 groups participated. Three trainings were also conducted to train 85 trainers from sakhi mandals of 3 blocks. Satellite Centres Medical services to tribals in deep interior villages of Dang district are provided through our satellite centres. A team of doctors and paramedical staff visit these centres and run medical dispensaries. Patients requiring further management are brought to Kharel. This year the satellite centre at Mahal was closed due to lack of place for the dispensary. The dispensary was started in 2009 in a rented place given by the Water Department. Nearly 4000 patients received services every year. Water Department needed the place for their departmental work and hence we had to vacant the premises. No positive response has been obtained Guidance and Support Members of a Self Help Group of Vanzana village were participating in a workshop organised by us. We keep arranging such small workshops in villages as a part of our women empowerment project under the National Rural Livelihood Mission. The Self Help group members were learning how to handle the different problems encountered in day to day administration of such groups. This group from Vanzana had some problems with their present president and they had stopped their savings since one year. After attending the workshop they realized their mistake. They were also empowered with the knowledge of how to handle such situations. All members conducted a meeting after the workshop and unanimously elected a new president and started their savings again. Thus with little help and guidance the once non-functioning group had started again on the road to their development. Women in rural areas want to participate in the progress of themselves and their families, they just need a helping hand. 5

8 Gram Seva Trust, Kharel Annual Report : Poverty and its vicious cycle Mangiben was in her thirties when she came to our hospital in Kharel for treatment. She was suffering from weakness, inability to work and breathlessness. On examination it was found that she was suffering from severe anaemia and malnutrition. Mangiben was a resident of Dang about 100km away from Kharel. The doctor was curious to know why such a young woman was severely malnourished. Hence she was asked about her family and back ground. Mangiben lived with her husband and four children. They worked as farm labourers to earn their daily bread. Even though both husband and wife worked very hard it was difficult to feed a family of six. Hence sometimes Mangiben sleeps without any food. As the days went by this took a toll on her health and now due to illhealth she cannot even work as labourer. Mangiben and her family were stuck in the vicious cycle of poverty- poor nutrition- illhealth-inability to work-poverty. Seeing her plight the doctors gave her free treatment and over few months her condition is improving and she can go to work now. to several applications sent to various departmental heads for their support and any other premise if available. We continue our efforts to search for a new place for the dispensary. Other Activities De- addiction camps Alcohol addiction is rampant in villages and has been our concern since long. Our de-addiction camps for alcohol addicts in the past have had little success. This time they were organized in collaboration with Alcohol Anonymous group of Navsari and donations from well wishers. One person of the group of 17 has successfully left alcohol for more than one year. The rest were inconsistent. Gram Seva Trust has given a place for Alcohol Anonymous and Al Anone( family members of alcoholics) meetings on every Tuesday and Saturday. But these meetings are not attended by the alcoholic patients as per our expectations. More and more people should attend these meetings. Women s Day Celebrations Every year International Women s Day is celebrated at Gram Seva Trust with different themes for that year. This year our theme was empowerment of families of alcohol addicts to cope with the psychological and physical stress in their life. Smt. Remya Mohan (District Collector, Navsari) was the Chief Guest. Women and adolescent girls from different villages participated enthusiastically and enjoyed the dramas, dances and songs prepared by our staff and students based on the theme. Our sincere thanks to Shri Devangbhai Desai, Smt.Snehalben Desai and Smt.Purviben Desai ( members of Alcoholic Anonymous and AlAnone, Navsari) to share their experiences with the participants and encourage them to help their family members to fight this illness. Introduction to English speaking for Youth. English has become a universal language and our youth are lacking in speaking skills. To encourage our youth and introduce them to the basic principles of speaking English we had organized a 3 day workshop for them in English Speaking. Shri.Rajendra Karnik from Surat conducted excellent sessions for the students in English grammar and speaking skills. This year the following staff members have joined us in our services for health and rural development (1) Dr. Mehul Patel (2) Dr. Ashish Patel. Acknowledgement We heartily acknowledge and thank all for their help and support. All the well wishers and donors in India and abroad who support us in achieving our objectives. Late Pushpaben Chaganbhai Naik (Gandeva) and family for support in construction of new outpatient department Timely Help saves Life Varsha was pregnant for the second time. She had a caesarean delivery the first time but had lost her child. Varsha was a resident of Nirpan village of Vansda block. Last village bordering Maharashtra. Hence she was admitted in a private hospital in Vansda about 25 to 30 km away from her village, when she had delivery pains early one morning. She also started bleeding and hence was transferred to our hospital at Kharel about 40 kms away from Vansda, in the evening in a very serious condition. On examination the gynaecologist suspected that Varsha may have had a tear in the uterus. Uterine tear is a complication which can cause death of both mother and child. Varsha was immediately taken to the operation theatre and operated. Though we could not save the baby, Varsha s life saved. We were also happy that we could save her uterus and hence a chance to become a mother in future. 6

9 Annual Report : Gram Seva Trust, Kharel Increase in Awareness and Timely Help saves Mother and child Hansaben was a resident of Dang district. She two previous abortions and was pregnant for the third time. During one of her checkups at a private clinic the doctor conducted a sonography. The sonography report showed a low lying placenta near the uterine opening. Such a condition can be dangerous as the patient can bleed to death. Hansaben and her family were informed of the risk. Hence when she started getting delivery pains she was immediately taken to the private hospital. The next day she had stated bleeding hence she was transferred to our Hospital in Kharel. Hansaben was in critical condition. She was immediately operated upon and both mother and baby were saved. The increase in awareness and antenatal care taken during pregnancy and the use of sonography even in interior villages saved the lives of a mother and her baby. H. No. 8, At & Post: Kharel, Dist: Navsari ] as on 31st March 2015 as well as Income and Expenditure Account for the year ended on 31st March We have conducted our audit in accordance with auditing standards generally accepted in India. These standards require that we plan and perform the audit to obtain reasonable assurance about whether the financial statements are free of material misstatement. 1. The accounts are maintained regularly and in accordance with the provisions of the Act and Rules 2. The receipts and disbursements are properly and correctly shown in the accounts 3. The cash balance and vouchers in the custody of the treasurer or trustee on the date of the audit were in agreement with the books of account 4. All books, deeds, accounts, vouchers or other documents & records required by us were produced before us. 5. A Register of movable and immovable properties has been maintained and the changes therein have been furnished to us. 6. All the necessary information required by us has been furnished to us by the Trustees personally whenever called upon 7. No property or funds of the Trust were applied for any object or purpose other than for the object or purpose of the Trust 8. There are no amounts which are outstanding for more then one year and written off 9. For repairs, tenders are not invited but for construction involving expenditure exceeding ` 5000/-, tenders are invited 10. The money of the trust have not been invested contrary to the provisions of section 35 of the Act. 11. There is no alienation of the immovable property contrary to provisions of Section There are no cases of irregular, illegal or imporper expenditure or failure or omission to recover money or other property belonging to the public trust or of loss or waste of money or other property thereof. 13. The provision of Budget to be filed under Rule in the form under Rule 16A is not applicable to the trust. However, the Trust has policy to perpare the Annual Budget. 14. The maximum and minimum number of trustees is maintained during the year. 15. The meetings are held as provided by such instrument. 16. The Minutes Books of proceedings of the meetings are maintained. 17. None of the Trustees has any interest in the investment of the trust. 18. None of the trustees is any debtor of creditor of the trust. 19. The irregularities pointed out by the Auditors in the accounts for the year under audit are have been duly complied with by the trustees. 20.
