Page 3 SOCIAL MEDIA, TEXT, AND PHOTO RELEASE Social media (Facebook, snapchat, Instagram, Twitter, etc.) and text messaging are forms of communication and are utilized within the St. John Youth Program to connect with students about upcoming events, build relationships and form the spirit with periodic reflections. We also utilize photographs of the events to update the parish of our happenings and advertise future youth group events. Use of these tools is not required, but recommended. When we are able to meet the students on their turf we are able to better communicate with them and engage in their lives through texting and social media. The volunteers who maintain a texting and social networking presence for the purpose of communicating with students and parents at St. John the Evangelist will: Limit communication to matters, volume, and times within the scope of ministerial responsibilities. Consistently communicate with Youth Director and Associate Youth Director social media interactions as well as any social media concerns that arise. Maintain a positive, Catholic, and accessible social media presence, open to all teens within the program. TEXT MESSAGING CONSENT: I give my student permission to access and use texting with St. John the Evangelist employees and volunteers for ministerial purposes. SOCIAL MEDIA CONSENT: I give my student permission to access and use social media with St. John the Evangelist employees and volunteers for ministerial purposes. I understand that I have a right to participate in the social media site and receive the same information my child does. I understand I can withdraw permission for my child to participate at any time by giving written notice to the youth minister. I give permission for my student to communicate via Facebook, Snapchat, Twitter, Instagram, and other forms of social media. PHOTO RELEASE CONSENT: I hereby authorize St. John the Evangelist Parish and its employees/volunteers to utilize my child s photographic images for the specific purpose of publication of the St. John the Evangelist events, including promotional materials.

Page 4 HIGH SCHOOL YOUTH CODE OF CONDUCT ** This form only needs to be completed once per year, but will be referenced in permission forms. Please maintain a copy for your records. ** STUDENT S NAME: 1. I agree to respect the rights and property of others. I understand that vandalism/stealing will not be tolerated. Financial obligations that result from such behavior will be the sole responsibility of myself and my family. 2. I agree to respect adult leaders and other participants. 3. I agree to demonstrate Christian values by my language and behavior. 4. I understand that the following behavior is appropriate conduct. I agree to dress appropriately. 8. I will act as a lady or gentleman and refrain from any misconduct, sexual or otherwise. 9. I will not leave an event, unless my adult leader grants permission. 10. I will not utilize radios, boom boxes, CD players, I Pods, and cell phones at an event, unless otherwise noted. 11. I will be open to building new relationships with my peers and adult leaders. I understand the need to agree to the above items. I realize and agree that if I do not abide by these rules, I may lose the privilege of attending a scheduled activity, or may be sent home at the discretion of the adult leader. My dismissal from youth group events will happen in a three-strike manner. Please see the back of this form for a more detailed explanation of the three-strike policy. Overall, I will be responsible for all consequences of my behavior. I, as a participant, agree to abide by these guidelines. Page 5 Three Strike Policy: At all St. John Youth events, youth are expected to abide by the Youth Code of Conduct (see back of this form). Overall, youth are asked to be respectful and courteous to those around them, and, if found to be out of line with the goals of the programming or the Youth Code of Conduct, then the following actions will be taken: Strike 1: When inappropriate behavior occurs at any youth event, the student will be alerted of the infraction that has occurred, and the parent will be notified by the Youth Minister via phone call that week to discuss the particular infraction. Students will be asked to write a letter of apology to any persons involved in the infraction, including a letter to the Youth Minister apologizing for the behavior. Strike 2: If the behavior does not improve and is found to be disruptive again, then the parents of the student will be required to schedule a meeting with the Youth Minister and Pastor to discuss the appropriate behavior modifications that need to take place before the student is allowed to participate again in youth events at St. John the Evangelist. Strike 3: Student will be asked to leave the event immediately (parents will be called) and will not be allowed to attend youth events at St. John the Evangelist until the behavior has been corrected and no longer exists. Duration of suspension is subject to the infraction committed and the judgment of the Youth Minister and the Pastor. In some cases, if student is allowed to continue participation, parent(s) are required to attend all youth functions with their child. **PLEASE NOTE: On overnight youth group trips and retreats, this policy is modified to a two-strike policy including strikes one and three. One addition will then be made: The student who committed the infraction will be taken to the nearest Greyhound bus station/airport and will catch the next bus/plane to Lawrence. The parent/family of the student will be responsible for the cost of the bus/plane ticket home.
