Personal Spiritual Formation Assessment Spiritual formation is fundamental in the discipleship process. By filling out this graph, you will have a grasp of which areas you are strong, and those in which you are weak. Just because you score low in an area does not mean that you can t do it, it just means you need to develop that area. Each area should be an area that we are striving to be better in as we grow. points giving relationship outreach word upward prayer 41-50 31-40 21-30 11-20 1-10

Now that you have charted your scores, take a moment to pray for the areas where you have scored low. Use the questions below to spark your next steps toward spiritual growth. 1. What three things do you need to do to help you grow in each of the six areas of growth? Write the three things down under each of the headings. giving - relationship - outreach - word - upward - prayer - 2. What actions do you need to take to remove the barriers to growth in the six areas of growth? giving - relationship - outreach - word - upward - prayer - 3.
