Personal Spiritual Formation Assessment Spiritual formation is fundamental in the discipleship process. The only way we can be sure to grow up in our faith is to honestly reflect and evaluate ourselves. Through this assessment you will be able to track your growth in six key areas: giving - to build the kingdom and help others relationship - ministering to the church outreach - sharing the gospel with the world around us word - knowing, understanding, and living God s Word upward - worshiping the Creator corporately and personally prayer - spending time abiding in Christ As you complete the assessment, avoid rushing. Listen for God s voice to encourage and challenge you. Consider this experience as one on one time with Him. Be intentional in your growth toward being like Christ. Use the following scale to respond to each statement: never - 1 seldom - 2 occasionally - 3 frequently - 4 always - 5 giving - to build the kingdom and help others 1. I seek to give 10% of my income toward kingdom work 2. I try to give above my tithe toward special giving for projects or ministries 3. I try to give above my tithe for faith promise missions giving 4. I give my time not out of compulsion but out of careful & prayerful thought 5. I practice hospitality by sharing my home and possessions with others 6. I give time to help in the community 7. I give during the holidays to help the less fortunate 8. I give to the generous bucket at least twice a month to help the needy 9. I seek to live below my means to ensure I am able to give to meet needs 10. I view all of my earthly possessions as tools to advance the kingdom giving total
relationship - ministering to the church 1. I understand my spiritual gifts and often seek to use them to serve my church family 2. I serve others expecting nothing in return 3. I build relationships by regularly participating in G.R.O.U.P.S. of some kind 4. Meeting the needs of others gives me a sense of purpose in my life 5. I strive to share biblical truth with those I serve when God gives opportunity 6. I seek to find opportunities to combine my passions with my gifts to serve 7. I seek to admit my errors in relationships and humbly seek forgiveness when I have wronged someone, and confront in a loving way those who have hurt me. 8. I am slow to speak and become angry - and quick to listen 9. I help others identify their gifts and passions and help them become active 10. I remain confident of God s love and provision during difficult times 4. My prayers focus on discovering God s will more than expressing my needs 5. I pray daily, outside of meal times 6. I believe my prayers impact the lives of others 7. I pray expecting an answer, and listen even if the answer is not the one I want 8.
