Deadline for return of license reporting form (30 days after the end of the annual license period) HOW TO USE THIS REPORTING FORM Thank you for purchasing the Evangelical Lutheran Worship Liturgies License. This report form contains a detailed listing of the material covered by the license. It corresponds to the abbreviated listing in the Evangelical Lutheran Worship Liturgies License The terms of your license agreement require that you record the uses of this licensed material. You must log this usage during each year your license is active and submit the reporting form within thirty days after the end of the license period. For each occasion an item covered by the Evangelical Lutheran Worship is used, please record that use by making a hash mark in the Tally column for that item. At the end of the reporting period, record the total number of hash marks for each item in the Total column. Return this Reporting Form to Augsburg Fortress within 30 days after the end of the annual license period. QUESTIONS Why does my church need to keep this record of our uses? Augsburg Fortress has a contractual obligation to share a portion of the license fees with many of the authors and composers whose work is covered by this license. In order to allocate those fees fairly, we need accurate information about the instances that each item is used. This allows us to pay the author and composer share of the license payments in proportion to the actual usage of each item. Many titles are included more than once in the listing. How do I know where to mark my usage? Titles may be listed more than once if: There are multiple versions of the same item. For example, if there are two versions of Text Only material, the title will be listed two times with parenthetical comments identifying a particular version. When entering hash marks in the Tally column, be sure to mark only the version being used. The license covers several items of the same song. For example, if the license covers both the text and music for a song, the record book lists the title two times: one for you to record your usage of the Text Only material, and the second for you to record your usage of both the text and the music Our congregation does not include the entire liturgy in our worship folder every week. How do I record that we ve reproduced only a portion of the liturgy? The recording form does not require you to note which portions of the liturgy you reproduce. If you use any or all of a particular liturgy, then depending on whether you use the Text Only or the Text Only and music, you ll need to record that use in the appropriate line for that liturgy. The Text Only material for many of the liturgies is the same. When using the Text Only material, does it matter where we record that use? Please record your uses as accurately as possible. That will assist us in gathering valuable information about worship practices in the church. We use the same folder at all of our weekend services (Saturday night, Sunday morning, and Sunday night). How do we record those uses? If the folders are essentially identical, then record it as one use. If there are variations between the services and each has a separate worship service, then record them as separate uses. Our congregation projects the service on a screen. How do we count those uses? For purposes of the reporting form, consider each service to be a separate usage event. Why do we need to keep a record of our usage each year? Other license programs, including OneLicense for hymns, have gone to recording usage on an ongoing basis. We are conforming our practice to this standard. What if I lose this form? A downloadable copy of this form is posted on the Augsburg Fortress website at www.augsburgfortress.org. Although we presently do not have an electronic reporting mechanism for this license, we are developing a process and anticipate that you will be able to do so in the future. If you wish to total each line, fill out the form as a Word document, and submit it as an attachment to the e-mail address above, you may certainly do so. We prepare a single worship folder that we use at each Wednesday night service during the season of Lent. Do we count that as one use or do we record each time we use the folder? If you distribute and collect the same folder for reuse each week, then record it as one use. However, if you reprint the folder each week, record each printing as a new use even if you ve continued to use the same items each week. Questions 2
