IN THE CIRCUIT OR COUNTY COURT OF THE FIFTH JUDICIAL CIRCUIT OF FLORIDA IN AND FOR LAKE COUNTY State offlorida VS Perjury in Official Proceedings False Official Statements Shawn Raymond Bultema Florida Department ofcorrections / Office ofthe Inspector General 18-13347 AFFIDAVIT OF PROBABLE CAUSE Comes now, the affiant, Roy Long, Law Enforcement Inspector, Florida Department of Corrections / Office of the Inspector General, a sworn law enforcement officer, who personally appeared before a notary public or certified officer, makes this affidavit, which has been electronically submitted to the Court. The affiant swears under oath that he has probable cause to believe that certain laws have been violated in Lake County, Florida and that on or about January 31, 2018, Shawn R. Bultema did commit Perjury in Official Proceedings in violation offlorida State Statute 837.02(1) and False Official Statements in violation of Florida State Statute 837.06. 1. That the events hereinafter set forth occurred at 19225 US Highway 27, Clermont, Lake County, Florida. 2. On January 22, 2018, You Affiant was assigned an Official Investigation involving Improper Conduct, 010 Case Number 18-01399. 3. On January 24, 2018, Your Affiant interviewed Sergeant Mark Caruso during the course of this investigation. Sergeant Caruso was placed under oath and stated that on December 5, 2018, he reported to Lieutenant Shawn Bultema that he had received information concerning a possible escape plot by inmates at Lake Correctional Institution. Sergeant Caruso states that upon advising Lieutenant Bultema of the escape information, he (Bultema) sent an email to an outside investigator in an attempt to obtain further information related to the inmates plotting the escape. Sergeant Caruso also stated that he completed an incident report outling the details ofthe escape plot and turned it in to Lieutenant Bultema. 4. Your Affiant obtained an Official Department of Corrections Incident Report submitted by Correctional Lieutenant Shawn R. Bultema, dated December 7, 2017. The report stated in part: "At no time did I receive an incident report from Sergeant Caruso for any reason. Any information that is given to me is documented per policy and procedure and the ChiefofSecurity and the Duty Warden is notified."

5. On January 31, 2018, Your Affiant interviewed Lieutenant Shawn R. Bultema as a potential witness. Your Affiant asked Lieutenant Bultema to raise his right hand and administered the following oath: "Do you solemnly swear or affirm the testimony you 're about to give is the truth, the whole truth and nothing but the truth", Lieutenant Bultema responded: "Yes Sir". During the interview Lieutenant Bultema was specifically asked twice if Sergeant Mark Caruso ever provided him with information about an escape plot, Shawn R. Bultema responded: "No he did not" and ''No sir". Lieutenant Bultema stated that the first information that he received concerning a potential escape plot was on Staff received information December 6, 2017, when1 during an inmate interview. 6. On February 6, 2018, Sergeant Mark Caruso's employment with the Florida Department of Corrections was terminated as a result of Lieutenant Bultema' s testimony as well as other circumstances. 7. On July 20, 2018, Lake Correctional Institution Warden Michael Mashburn requested a extensive search of Lieutenant Bultema's State of Florida Email account for the date of December 5, 2018. 8. On July 27, 2018, Warden Mashburn received the requested Emails. The following email was recovered, dated December 5, 2017 at 1:41 PM: To: Mike Gampfer, Orange County, FL From: Shawn Bultema "Hey bud, I need a little help with something? Can you forward this to Chad Goodling as well? The above inmate is supposed to have a brother in New York that creates fake ID's and other items. His name is Johnny Rosado; he is supposed to be making some fake 1D's for multiple inmates at Lake CI to assist them in ~ ou see if ou have an information on this dude? Tosado 111111111111111 Any help would be great, this is supposed to happen by Christmas. Thanks bud!" 9. The recovered Email sent by Lieutenant Bultema on December 5, 2018, directly contradicts the written and sworn testimony that he provided in Offical Proceedings. 10. Sergeant Mark Caruso was adversely affected (loss of employment) due to the false sworn testimony and documentation provided by Lieutenant Shawn Bultema. 11. August 16, 2018, Shawn Bultema resigned his position as Captain with the Florida Department of Corrections without providing appropriate notice.

Your Affiant believes that probable cause exists for the arrest ofshawn Raymond Bultema for Perjury in Official Proceedings in violation of Florida State Statute 837.02(1) and False Official Statements in violation offlorida State Statute 837.06.
