Child: q Yes q No q Uncertain q Not Ready Yet Father: q Yes q No q Uncertain q Not Ready Yet Mother: q Yes q No q Uncertain q Not Ready Yet PARENT SCHOOL AGREEMENT 1. I realize that Westlake Christian Academy cannot take the place of regular spiritual instruction in the home and my child's regular attendance and participation at church, and I further understand that my regular attendance and involvement in a sound gospel- preaching church is necessary for my child's continued enrollment in this school. 2. I agree to uphold and support the academic standard of the school by encouraging my child to study and complete all homework and assignments. 3. I understand the tuition arrangements and agree to make payments according to that schedule. 4. I understand that the school may not transfer transcripts and other records, except as required by law, until all financial obligations have been met. 5. I agree to resolve any issues or concerns related to the school, its policies, procedures, or interactions with any member of the school community (teacher, administration, student or parent) by following the biblical principles found in Matthew 18:15-20. I will first discuss my concerns privately, prayerfully and in the love of Christ with the other person involved. I will in no case discuss or complain to another person regarding my concerns. 6. I understand that I am to learn, uphold, and support the standards, policies, procedures, rules and regulations pertaining to student life and parental responsibilities for my child at WCA. This includes, but is not limited to, my child not dishonoring the Trinity and the Word of God, and/or disrespect to classmates or personnel of the school. It also includes totally abstaining from the use of illegal drugs, alcohol, tobacco, and to refrain from immoral and illegal behavior (including profanity, obscene gestures, sexual activity, gambling, weapon possession, criminal activity, etc.) as long as my child is enrolled at WCA whether at school or off campus, whether during the school year or summer. (i.e. 24/7/365) I/we authorize the school to employ such discipline as it deems wise and expedient for the training of my child. (Proverbs 22:6, Ephesians 6: 1-2, Hebrews 12:6) 7. I understand that assessments will be made to my account to cover damage to school property caused by my child (Including breakage of property, abuse of books, etc.). 8. I understand that the school reserves the right to dismiss any child who fails to comply with the established regulations and discipline or if financial obligations remain unpaid. 9. I agree not to use the school telephone and address directory for the purpose of advertising, personal gain, or to spread influence. 10. I am in personal agreement with Westlake Christian Academy s Statement of Faith. 11. I am in support of the school s mission, as well as policies and procedures, including those in the Westlake Christian Academy s School Parent- Student Handbook. 12. I am willing for our child to receive Biblical training. 13. I relieve the school board and any WCA employees from liability in connection with the school activities and instructions.

14. I give permission for my child to take part in all school activities, including sports and school- sponsored trips away from the school premises (except as specifically communicated in written form and attached to this application). Further, in the event my child becomes ill or is injured while under school supervision, I approve the school authorities to will: A. Contact a parent of the student and follow his/her instructions. B. In the event neither parent can be reached, contact the student's physician and follow his/her instructions. Physician's name: Phone # C. If the student's physician cannot be reached, the school authorities will use their own discretion in contacting a properly licensed practicing physician and follow his/her instructions. STATEMENT OF FAITH 1. We believe the Bible to be the inspired and only infallible, authoritative written Word of God. (II Timothy 3:16-17) 2. We believe that there is one God, eternally existent in three persons: Father, Son, and Holy Spirit. (Matthew 28:18, II Corinthians 13:14) 3. We believe that the universe originated by the creative act of God as revealed in Holy Scripture, and that the form of every kind of life was fixed at the time of its creation. (Genesis 1:1, John 1:1) 4. We believe in the true deity and true humanity of our Lord Jesus Christ, His pre- existence, His virgin birth, His sinless life, His miracles, His vicarious and atoning death through His shed blood, His bodily resurrection, His ascension to the right hand of the Father, and His future visible return in power and glory. (Colossians 1:13-22, John 1:14) 5. We believe that the Lord Jesus Christ is the only mediator between God and man. (I Timothy 2:5) 6. We believe that all men are lost and sinful, and that salvation is received through personal faith in the Lord Jesus Christ and by the act of regeneration of life by the Holy Spirit of God. (Romans 3:23-26, Titus 3:5) 7. We believe that by the present indwelling ministry of the Holy Spirit, the Christian is enabled to live a Godly life, and that every believer should practice the holiness which God requires. (Ephesians 1:13-14, Romans 8:1-4) 8. We believe in the resurrection of both the saved and the lost; they that are saved unto the resurrection of life, and they that are lost unto the resurrection of damnation. (John 5:24-25, 28-29) 9. We believe in the spiritual unity of believers in our Lord Jesus Christ. (John 17:20-23, 1 Corinthians 12:12-13) 10. We believe that support of and participation in the task of fulfilling the great commission is an obligation of all believers in our Lord Jesus Christ. (Matthew 28:19-20) 11. We recognize that there are other doctrines held by various Christian believers, which they believe to be consistent with the above. However, such teachings shall not be incorporated as approved by this school.

PROFESSIONAL MEMBERSHIP Westlake Christian Academy is member school of the Association of Christian Schools International (ACSI) with headquarters in Colorado Springs, Colorado. It is the largest association of Christian schools in the world and also serves as an aid in legal and legislative matters and the educational ministry of Christian schools. Our school benefits from the special bulletins and periodicals which are issued by the association. Our faculty takes part in the professional growth conferences which are conducted annually. Our students benefit from the association by having the opportunity to participate on a broader basis with students from other Christian schools in speech, academic, math, music, and athletic meets. ACSI provides certification for teachers based on their qualifications in their academic area, Biblical studies, and the Christian philosophy of education. Full- time teachers are required to be certified by ACSI. We believe that requiring teacher certification and appropriate professional credentials is another area of excellence for which we can strive in doing our best for the Lord. Furthermore, Westlake Christian Academy is recognized by the Illinois State Board of Education. This recognition affords the school opportunities to excel in several areas without jeopardizing our autonomy as an independent Christian school. Such benefits include but are not limited to increased recognition within the local community and throughout the state, and enhanced scholarship opportunities. OUR SCHOOL IS A MEMBER OF THE ASSOCIATION OF CHRISTIAN SCHOOLS INTERNATIONAL AND RECOGNIZED THE BY THE ILLINOIS STATE BOARD OF EDUCATION.
