This directory is for the Utah counties of: Box Elder, Cache, Davis, Garfield, Iron, Morgan, Rich, Salt Lake, Sanpete, Sevier, Summit, Utah, Wasatch, Washington, Weber and the Idaho county of Franklin. To access SelectHealth Advantage Dental Advantage online provider directory, you can visit selecthealth.org/medicare. For any questions about the information contained in this directory (hardcopy or online), please call our Member Services department toll-free at during the following dates and times: October 1 to March 31: Weekdays 7:00 a.m. to 8:00 p.m., Saturday and Sunday 8:00 a.m. to 8:00 p.m. April 1 to September 30: Weekdays 7:00 a.m. to 8:00 p.m., Saturday 9:00 a.m. to 2:00 p.m., closed Sunday. 5 Section 1 Introduction This directory provides a list of SelectHealth Advantage Dental Advantage network providers. To get detailed information about your healthcare and dental coverage, please see your Evidence of Coverage. The network providers listed in this directory have agreed to provide you with your dental services. You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. You are not required to obtain a referral to receive services that are covered by our plan. A network dental provider can submit a treatment plan to pre-determine your benefits prior to treatment. Billing Information Providers bill SelectHealth Advantage directly for your care. We then pay the provider for you, minus the copayment or coinsurance. If you receive a bill directly from a provider, please mail the bill to the following address: SelectHealth Advantage P.O. Box Salt Lake City, Emergency Coverage You must use network providers except in emergency or urgent care situations. If you obtain routine care from out-of-network providers, neither Medicare nor SelectHealth Advantage will be responsible for the costs. It s important to know you are covered and where to go when you need care. For emergency care, you may receive covered services from out-of-network providers. For all other dental services, you must receive care and treatment from an in-network dental provider. Our plan does NOT cover urgently needed care or any other non-emergency care if you receive the care outside of the United States. iv

How do you find Dental Advantage network providers in your area? This provider directory lists providers by provider type, county, and city. When you find the type of provider you are looking for, providers will be listed by county and city. Once you find the county and city you are looking for, Providers are listed alphabetically by last name. An alphabetical listing of providers is included in the index, which can be found at the back of this directory.
