It is a joy to celebrate your following God s calling into ministry! The Board of Ordained Ministry of the Alabama West Florida Conference is excited to work with you to determine how you can best serve God through the United Methodist Church. Do your best work on this material and be sure to follow the instructions in this letter and throughout this packet. Failure to follow the directions given can be grounds for delaying your commissioning. This application packet is to be completed and returned to Rev. June Jernigan by 3pm Central Time, November 3, 2014 by those who seek to be commissioned as Provisional Elders at the 2015 Annual Conference. Remember, you are not required to complete this packet just because you are eligible to apply, if you do not feel you can do your best work. You simply may apply the following year to be interviewed by the Board of Ordained Ministry. You will receive information later with a specific day and time to arrive at Blue Lake for your interview with the Board of Ordained Ministry. Below are some guidelines for submission of your information, suggestions for making this process smooth for you, and answers to some frequently asked questions. Put your name and the date at the top of every page that you turn in. You are responsible for the cost of your medical exam. EKG s, etc. are not required unless your physician feels your condition warrants such tests for a clean bill of health. one copy of your sermon manuscript (not transcript) to Rev. June Jernigan at june@awfumc.org. Additionally, mail one copy of a DVD of your sermon being preached to Rev. Jernigan at the above address. Sermon guidelines are included in this packet. Follow them carefully. Print three copies of the sermon evaluation form (included in packet) and give to three lay persons to do the evaluation. Three evaluations must be received from laypersons. You are responsible for insuring our timely receipt of these evaluations. one copy of your Bible study to Rev. Jernigan. Guidelines are included in this packet. Follow them carefully. The doctrinal questions you will answer are from the 2012 Book of Discipline, Paragraph 324.9, which are included in this packet. your responses to Rev. If you are not presently serving or working at a church, have the PPR/SPR evaluation filled out by your home church PPR/SPR committee, or return it to Rev. Jernigan with a detailed explanation. The ministerial evaluations must be completed by members in full connection in the Alabama West Florida Conference. This includes retired clergy. (Three recommendations are required.)
3 It is your responsibility to see that we have a copy of your psychological report, medical report and background check. All of these must be within the past five years. Background check authorization can be found on our website at: (Background check authorization) Recommendations and forms which others complete on your behalf may take a while. Have them filled out ASAP, for your peace of mind. If they are not turned in by the deadline, your application packet will be incomplete, and you will not be able to appear before the BOM to be considered for commissioning this year. It is your responsibility to see that Rev. Jernigan receives all forms and evaluations by 3pm Central Time, November 3, This includes making sure that the papers being filled out for you by others have been mailed directly to her by this date. Please note the following: Completing these written assignments and appearing for an interview does not guarantee that you will be commissioned as a Provisional Elder. 4 PROVISIONAL MEMBERSHIP AND ORDINATION POLICIES 1. Authority: of the 2012 Book of Discipline 2. A person coming from another denomination or from another conference may be considered for membership provided they meet the requirements of All commissioned ministers shall serve within the scope of practice of the order for which they are preparing (i.e., deacon or elder) for the entire provisional period [ 326]. 5. CPE (Clinical Pastoral Education) is required of all persons seeking ordination as deacons and elders. CPE requirements must be completed prior to the beginning of the two -year residency. 7. Students who have completed ½ of the basic graduate theological studies as listed in par of the 2012 Book of Discipline may apply for commissioning. These basic graduate theological studies must includes courses in Old Testament; New Testament; theology; church history; mission of the church in the world; evangelism; worship/liturgy; and United Methodist doctrine, polity and history. However, all educational requirements (both Discipline and conference) must be completed prior to the beginning of the two -year residency. 8. 10 Guidelines for Bible Study for Provisional Elder Choose one: Judges, Job, Mark, James If you have submitted a study on one of these books previously, you must choose one of the others. You are to prepare a written six-to-eight week Bible study. The purpose of this requirement is to place emphasis upon your need to be effective in teaching the Bible to persons of all ages. The committee will be evaluating your level of understanding of the prescribed biblical text, your desire to open the Word of God to God s people through Bible study, and your comfort/anxiety level in leading Bible study classes. You do not have to teach this study prior to submission. Please follow these guidelines in fulfilling this requirement: 1. The Bible study should be designed to include six to eight sessions and COVER THE ENTIRE BOOK from the first chapter to the last. Each session should be no shorter than one hour and no longer than two hours. You may use any number of sources and must include a bibliography of those sources. 2. The Bible should serve as the primary source, and the study should be your work. 3. You are not to copy the study from another text, though you may follow the outline of another text or curriculum. It is expected that the study will reflect theological understandings that are consistent with our Wesleyan heritage. The committee is looking for evidence of the ways you worked with the biblical text. Work with questions that deal with the biblical text as opposed to those that might deal with a commentator s ideas. 4. Describe the setting of the study and the response of participants to the study (if it has been taught). Describe the teaching method that you will use (lecture, discussion, small groups, etc). The teaching method should be selected based on the objectives for the study, your gifts as the leader, and the direction of the text. 5. You are to use an outline form of no more than one to two pages per session. These outlines should cover the entire book, from the first chapter to the last. This outline should include the objectives for each session (no more than three for each session), primary and secondary topics, resources that are to be used, questions that are to be asked, etc. Objectives for each session should be written clearly in correct form, such as: At the end of this session, the class will know.... Or At the end of this session, the class will be able to. 6. For one of the sessions, be as detailed as possible. In manuscript form, give time frames for each activity, major and minor points to be discussed and very detailed information concerning the class, the environment, and materials used. This one session should be as informative for the committee as possible. Use as many pages for this session as needed to give the details required. 7. As with any professional work, careful attention should be given to proper grammar and spelling. Bring a copy of the Bible study with you to the interview. 13 Doctrine and Theology Committee THE SUCCESSFUL CANDIDATE 1. The successful candidate generally submits a well constructed, carefully proofread paper of 30 to 40 pages double spaced. The Disciplinary questions should be included in the paper with each response given. These questions should be single spaced using 12 point times new roman. 2. Computer spell checking is generally not sufficient for the type of work expected of those with college and graduate degrees. While spelling and grammar are not a part of the Doctrinal Examination, they are important tools of the ministry of Associate Members. 3. There are not any yes or no questions among those asked. The purpose of the questions is to give the candidate an opportunity to demonstrate theological reflection and understanding of the doctrinal issues under consideration and to communicate these reflections to a larger group. 4. Choose a writing style guide and use it when writing this scholarly paper. Some of the common style guides are The Chicago Manual of Style, the MLA Style Manual, and Turabian s Manual for Writers. 5. The interview will give the candidate an opportunity to respond to points in the paper raised by members of the Doctrine Committee as well as to address items not specifically covered by the paper. STATEMENT OF PURPOSE: The purpose of the Doctrine and Theology Committee of the Board of Ordained Ministry of the Alabama-West Florida Conference is to examine future ministers of the United Methodist Church as to each candidate's understanding of the doctrines that distinguish us as Christians and United Methodists. We hold to the distinctions made by John Wesley in his sermon Catholic Spirit, that there are essential doctrines and non-essential doctrines among all believers. At the same time, we realize that there are certain doctrines that distinguish Christians from all other religions and United Methodists from all other denominations. In addition, it is essential that the candidate for ordination understand the issues of polity and discipline that are an integral part of the United Methodist Church. It is to address these issues that the Doctrine and Theology Committee functions as a part of the Board of Ordained Ministry. The guidelines for questions to be asked of candidates seeking admission as provisional members are found in 324 of the 2012 Discipline. For those seeking full connection and ordination as a Deacon the questions used as guidelines are provided in 330 of the 2012 Discipline. For those seeking full connection and ordination as Elder the questions used as guidelines are provided in 335 of the 2012 Discipline. The Committee does not seek to create points of contention with a candidate, but rather to be sure that the candidate fully understands the essential doctrines of Christianity and the United Methodist Church. Should the candidate show a lack of knowledge of key doctrinal issues, or have issues with United Methodist doctrine or polity, it is the responsibility of the committee to draw attention to these issues and bring clarity to the candidate. If necessary, the candidate may be asked to do additional study in the area of disagreement or confusion. It is the responsibility of the committee to report their findings to the Board, and make recommendations as to their approval or disapproval of a candidate.

The U.S. Preventive Services Task Force recommends routine periodic screening for colorectal cancer (persons years of age), breast cancer (women 40 years), and cervical cancer. The U.S. Preventive Services Task Force recommends that all pregnant women be screened for asymptomatic bacteriuria, iron-deficiency anemia, hepatitis B virus, and syphilis. The U.S. Preventive Services Task Force recommends against screening for hemochromatosis; carotid artery stenosis; coronary artery disease; herpes simplex virus; or testicular, ovarian, pancreatic, or bladder cancer. Outside of prenatal, preconception, and newborn care, genetic testing should not be performed in unselected populations because of lower clinical validity; potential for false positives; and potential for harm, including genetic labeling. For patients for whom genetic testing may be appropriate, referral for genetic counseling should be provided before and after testing. A human papillomavirus vaccine series is indicated in females ages 9 through 26 years, regardless of sexual activity, for prevention of cervical cancer. A single dose of tetanus-diphtheria acellular pertussis (Tdap) vaccine should be given to adults ages 19 through 64 years to replace the next tetanus-diphtheria toxoid (Td) booster. A zoster (shingles) vaccine is given to all patients 60 years and older regardless of history of prior shingles or varicella infection. Asymptomatic adults who plan to be physically active at the recommended levels do not need to consult with a physician prior to beginning exercise unless they have a specific medical question. Smoking status should be determined for all patients. Patients who want to quit smoking should be offered pharmacologic therapy in addition to counseling, including telephone quit lines. Routine screening is recommended to identify persons whose alcohol use puts them at risk. For management of alcohol abuse and dependence, referral for specialty treatment is recommended; for management of alcohol misuse, brief behavioral counseling may be useful.
25 Clues for chemical dependency include unexpected behavioral changes, acute intoxication, frequent job changes, unexplained financial problems, family history of substance abuse, frequent problems with law enforcement agencies, having a partner with substance abuse, and medical sequelae of drug abuse. Condom use reduces transmission of HIV, Chlamydia, gonorrhea, Trichomonas, herpes virus, and human papillomavirus. It is important to ask about domestic violence when patients present with symptoms or behaviors that may be associated with abuse. When an abusive situation is identified, address immediate safety needs.

32 Choose one passage: Guidelines for Sermon for Provisional Elders Genesis 1:1 2:4a; Psalm 8; 2 Corinthians 13:11 13; Matthew 28:16-20 You are to prepare and deliver a sermon based on one of the above prescribed texts and submit the full manuscript (not transcript), worship bulletin or worship plan, and a DVD video of the worship service. You may also Rev. Jernigan a digital copy of your presentation, but the DVD must still be submitted. The purpose of this requirement is to place emphasis upon the pastor s need to be effective in preparing and delivering sermons. The committee will be evaluating your level of understanding of the Biblical passage, your commitment to effectiveness in preaching the Word of God, and your comfort/anxiety level in preaching and leading worship in a United Methodist Church. Please follow these guidelines in fulfilling this requirement: 1. You should include a sermon title, which reflects the main theme of the sermon, as well as a statement of purpose that discloses what you anticipate the congregation will take with them or do after hearing the sermon. 2. The sermon should be neither shorter than fifteen minutes nor longer than twenty-five minutes, and the manuscript (not transcript) should not exceed eight typed, double-spaced pages. all work to june@awfumc.org. 3. You should submit a clear DVD video of the entire worship service in which you preached the sermon. The DVD may be of you preaching the sermon for a class or in another setting if it is not possible for you to preach the sermon in a worship setting. View your recording!
