Health information exchange (HIE) is the ability to electronically exchange patient health information including: Lab results Pathology results Radiology reports Orders Patient care summaries (CCDs) Transcriptions Referrals Consult reports Meds, allergies, problem lists when it s needed for patient care. Individuals Organize Local champions organize and begin to work together to establish a vision for HIE HIE Awareness & Education Community forums, presentations and demos of HIE with CORHIO staff Community Assessment Collaborate w/ CORHIO for community requirements. Submit planning assessment and community goals. Clinical Advisory Committee Identify members and collaborate with CORHIO to establish committee to advise on use of HIE to achieve clinical goals. Implementation HIE participants and CORHIO implement HIE functionality and interfaces. CORHIO facilitates training and provides ongoing support. ID Community HIE Participants Hospitals, providers, LTC, mental health, public health & local labs/imaging Leaders Engage Community leaders & decision makers join conversations about HIE in the community. Collaborate with CORHIO for support. Letters of Intent & Assessments Submit individual LOIs. Submit HIT Assessments. CORHIO provides costs & a draft community plan with resource needs.
