Statement of Faith The following statements reflect the essentials that we believe at The Christian Church at Cogan Station. It is important that we know what our volunteer teams are teaching children/youth/parents with which they interact. Please read our statement of faith and initial below if you understand and agree with the following: We believe in one God: the Father, Son and Holy Spirit. (Matthew 5:16, 45; 6:1, 4, 8, 9; James 1:17 / John 1:1; 14:9 / Genesis 1:2; John 4:24; 14:16-20; 2 Peter 1:21) We believe in God the Father Almighty, Creator of all things visible and invisible. (Genesis 1:1; Acts 17:24-28; Hebrews 11:3) We believe in Jesus Christ, God's one and only Son, my one and only Savior, (John 1:14; 1 John 4:9 / John 3:16; 14:6; 2 Peter 3:18) o Who was born Jesus of Nazareth, both fully human and fully divine, conceived of the Holy Spirit and the virgin Mary,(Matthew 21:11; John 19:19 / John 1:14; Romans 5:15-17; Colossians 2:9; 1 John 1:1 / Matthew 1:18; Luke 1:26-38) o Who suffered and was crucified under Pontius Pilate, (Luke 23:1-46) o Who died and was buried, and Who rose again bodily from the dead on the third day, (Luke 23:44-24:8; John 20:24-29; 1 Corinthians 15:3-8) o Who ascended into Heaven and now sits at the right hand of the Father, (Luke 22:69; John 14:1-3; Acts 1:1-11; Colossians 3:1) o and Who will return to earth to judge both the living and the dead. (Matthew 24:36-42; John 14:1-3; Acts 1:10-11; 10:39-42; 1 Peter 4:5) We believe in the Holy Spirit, Who is an active and operative part of the triune God, (John 16:5-15; Acts 1:7-8; Romans 8:26-27) o and Who indwells every Christian. (1 Corinthians 3:16; 6:19; 2 Timothy 1:14) We believe in the Bible as God's Holy Word, Scripture. We believe God inspired the autographs (original writings) of the Scripture and those autographs were consequently without error. (Mark 12:36; John 14:26; 16:12-15; Acts 1:16; 1 Corinthians 2:12-13; 2 Timothy 3:14-16; 2 Peter 1:20-21) We accept the Bible as the final authority for all matters of faith and practice. (Isaiah 40:8; Matthew 5:18; 24:35; Romans 15:4; Hebrews 4:12) We believe the Bible teaches that man, created by God, willfully sinned against God and is consequently lost and without hope apart from Jesus Christ. (Acts 4:12; Romans 3:10-12, 23) We believe the Bible teaches that salvation is achieved only by grace through the blood of Jesus Christ. (Matthew 26:28; Romans 5:9; Ephesians 2:8-9; 1 Peter 1:18-19; 1 John 1:7) We believe the Bible teaches that one receives salvation by God's grace through faith, which necessarily involves trusting in Christ as Savior and Lord, repenting of sin, confessing that faith, and being immersed into Christ, but that none of these is sufficient in and of itself for the assurance of salvation. (Romans 5:1-2 / Luke 24:45-47; Acts 3:19; 17:30; 2 Peter 3:9 / Matthew 10:32; Romans 10:9; Philippians 2:11; 1 John 4:15 / Acts 2:38; Romans 6:1-7; Galatians 3:27; Colossians 2:12; 1 Peter 3:21) We believe in the Church of Jesus Christ, founded on the Day of Pentecost, as consisting of all Christians everywhere. (Matthew 16:13-18; Acts 2:14-47) We believe the Bible teaches that the Elders are to exercise authority over the local congregation. (Acts 14:23; 20:17, 28; Philippians 1:1; 1 Timothy 3:1-7; 5:17; Titus 1:5-9; James 5:14; 1 Peter 5:1-4) 2

Child/Student Safety/Required Clearances The safety of the children who participate in CCCS sanctioned programs is extremely important. The Common Wealth of Pennsylvania also has laws that we are required to follow. Please initial each section to affirm you have read it and will comply. 1. I understand that the role I am applying for at CCCS requires me by law to become a Mandated Reporter. I understand that a Mandated Reporter is required by law to report suspected child abuse if they have reasonable cause to suspect that a child is a victim of child abuse. I understand that if I suspect child abuse I am required to: a. Make an immediate and direct report of suspected child abuse to ChildLine either electronically at www.compass.state.pa.us/cwis or orally by calling 1-800-932-0313. b. After making the report to ChildLine, I am required to immediately notify the most accessible Ministry Staff person. c. If an oral report was made to ChildLine I must also make a written report, which may be submitted electronically, with in 48hrs. I understand I am required to familiarize myself with the new definition of Child Abuse which can be found at www.keepkidssafe.pa.gov 2. I understand that CCCS is required by law to have all adult Volunteers (unpaid positions) who are responsible for the welfare of a child/student (under the age of 18) or have routine interaction with children/students to complete specific clearances that must be renewed every 3 years. (Because of the overwhelming cost of obtaining clearances for all the volunteers required we are asking you to cover as much of the cost as you are willing. Because we appreciate each of you immensely and we realize this could be expensive for your family we are more than willing to help cover the cost if you would like.) a. Report of criminal history from the Pennsylvania State Police (Act 151) i. Please obtain this, it can be completed and printed immediately for $10 online at epatch.state.pa.us b. Child Abuse History Clearance from the Department of Human Services (Act 34) i. Please obtain this, it can be completed, printed immediately and/or mailed to your home within 2 weeks for $10 online at www.compass.state.pa.us/cwis c. Volunteers who have lived outside the Common wealth of Pennsylvania in the last 10 years must obtain a fingerprint based federal criminal history (FBI). i. Please obtain this by going through www.pa.cogentid.com/index_dpw.htm following the directions and going to a finger printing site such as the UPS store on the Golden Strip for $27.50. The clearance will be mailed to your home within 6 weeks following the physical finger printing. 3

3. I am also affirming that if I am arrested for or convicted of an offense that would constitute grounds for denying participation in a CCCS program, activity or service, or are named as a perpetrator in a founded or indicated report, I must provide most accessible Ministry Staff person with written notice not later than 72 hours after the arrest, conviction or notification that I have been listed as a perpetrator in the state wide database. 4. I understand that if an administrator of any CCCS program has reasonable belief that I was arrested or convicted of an offense that would deny participation or named as a perpetrator in an indicated or founded report CCCS must immediately require me to and pay for me to obtain new clearances. 5. I affirm that: a. I have never been named in the Statewide database as the perpetrator of a founded report of child abuse committed within the last five (5) years. b. I have never been arrested or convicted of the following offenses under the 23 pa. C.S. 6344 (c) or convicted of an offense similar in nature to those crimes listed in 23 pa. C.S. 6344 (c) under the laws of the United States or one of its territories, possessions, another State, the District of Columbia, the Commonwealth of Puerto Rico or a foreign nation, or under a former law of this Commonwealth. 23 Pa. C.S 6344 (c) (c) Grounds for denying employment or participation in program, activity or service.-- (1) In no case shall an administrator hire or approve an applicant where the department has verified that the applicant is named in the Statewide database as the perpetrator of a founded report committed within the five-year period immediately preceding verification pursuant to this section. (2) In no case shall an administrator hire an applicant if the applicant's criminal history record information indicates the applicant has been convicted of one or more of the following offenses under Title 18 (relating to crimes and offenses) or an equivalent crime under Federal law or the law of another state: Chapter 25 (relating to criminal homicide). Section 2702 (relating to aggravated assault). Section 2709.1 (relating to stalking). Section 2901 (relating to kidnapping). Section 2902 (relating to unlawful restraint). Section 3121 (relating to rape). Section 3122.1 (relating to statutory sexual assault). Section 3123 (relating to involuntary deviate sexual intercourse). Section 3124.1 (relating to sexual assault). Section 3125 (relating to aggravated indecent assault). Section 3126 (relating to indecent assault). Section 3127 (relating to indecent exposure). Section 4302 (relating to incest). Section 4303 (relating to concealing death of child). Section 4304 (relating to endangering welfare of children). Section 4305 (relating to dealing in infant children). A felony offense under section 5902(b) (relating to prostitution and related offenses). Section 5903(c) or (d) (relating to obscene and other sexual materials and performances). Section 6301 (relating to corruption of minors). Section 6312 (relating to sexual abuse of children). The attempt, solicitation or conspiracy to commit any of the offenses set forth in this paragraph. (3) In no case shall an employer, administrator, supervisor or other person responsible for employment decisions hire or approve an applicant if the applicant's criminal history record information indicates the applicant has been convicted of a felony offense under the act of April 14, 1972 (P.L.233, No.64), known as The Controlled Substance, Drug, Device and Cosmetic Act, committed within the five-year period immediately preceding verification under this section. 4
