2018 CONFERENCE THEME IN THIS WAY THE LOVE OF GOD WAS REVEALED TO US: GOD SENT HIS ONLY SON INTO THE WORLD SO THAT WE MIGHT HAVE LIFE THROUGH HIM. (1 JOHN 4:9) Basic Information WHAT IS A STEUBENVILLE HIGH SCHOOL SUMMER CONFERENCE? Franciscan University of Steubenville, Ohio seeks to provide an opportunity for teens to come to know the saving love of Christ by providing a powerful experience of the Catholic Church and its sacramental life. This conference is specialized to address the resources and needs of today's Catholic youth, organized on a national and regional level. With an exciting line up of speakers, musicians, and presenters, the High School Youth Conference will challenge Catholic youth to stand firm in their faith, rely on the awesome power of Jesus Christ who is their guide, friend, and Savior, and show that faith to the world. The Sacraments are the highlight of the conference. Teens will join in a dynamic Eucharistic Liturgy during the weekend and will be given the opportunity to participate in the Sacrament of Reconciliation. There is also a powerful time of Eucharistic Exposition and Adoration. Many teens encounter the Holy Spirit for the first time. This leads to a tremendous conversion for many young people. WHY SHOULD A YOUNG PERSON ATTEND A STEUBENVILLE YOUTH CONFERENCE? The whole vision for the conference is to lead teens to Christ and to give them a deeper understanding and love for their Catholic Faith. If your teens are beginning their personal walk with Christ, needing strength to continue their journey, or just ready to rejoice in the many blessings He has given, then this is the conference you are looking for! WHO CAN ATTEND THIS CONFERENCE? The participants in the conference must be entering 9th, 10th, 11th, or 12th grades in the fall of 2018 or a 2018 high school graduate. We will also need chaperones, who must be at least 21 years old and have completed Safe Environment training and a background check with the Diocese of St. Petersburg. HOW WILL MY TEEN GET TO THE CONFERENCE? 2018 THEME SUMMARY Knowledge of the truth of who God is and who we are provides the foundation for living authentically. We will focus on identity at the root. "Christ came not only to reveal God to man but man to himself." (Gaudium et Spes) This truth will also help youth to understand true reality as opposed to the "virtual and idealistic reality" of the world. Officially, we will be responsible for your teen only while he or she is at the Steubenville High School Summer Conference. This allows you greater flexibility to work out carpooling arrangements with other parents. We do not anticipate having a large enough group to make renting a bus affordable for you, and any Church-sponsored transportation is subject to significant restrictions. I highly recommend taking the opportunity to connect with other parents to make transportation arrangements that will work for your family! Please note that self-parking at the Rosen Shingle Creek is $9 per day plus tax.

Cost and Payment Plan COST FOR YOUTH PARTICIPANT Conference Registration: $185.00 Hotel Room: $149.00 plus applicable fees and taxes per night/per room Miscellaneous Expenses (preparation and during the weekend) Total Cost of $300.00 per teen is calculated based on 4 teens per room and includes conference registration, hotel lodging at The Rosen Shingle Creek, main meals from Friday night dinner to Sunday morning breakfast, all conference programming, and some miscellaneous expenses. The actual total may vary slightly based on size of our group, and you may pay an additional fee to have a smaller number of teens in the hotel room. COST FOR ADULT PARTICIPANT (CHAPERONE) We must pay the same fees for adult chaperones as for teen participants. However, we are including the cost for chaperones in the participant fee and in our fundraising so that our adult chaperones do not have to pay out of pocket. A percentage of your teen s sales will be applied directly to your final balance. See Yankee Candle Fundraising packets for more details. If we need to continue with additional fundraising after the Yankee Candle Fundraiser, we are tentatively considering a car wash in May as an option. We will work out details as this date approaches.

Behavior Expectations for Youth As representatives of St. Mark the Evangelist Catholic Church and the Diocese of St. Petersburg, all teens attending the Steubenville Florida Youth Conference are expected to abide by the diocesan Code of Conduct for Children and Youth as well as all parish behavior policies. In addition, certain expectations have been provided by the conference organizers. These are printed here for your information. How to show love and respect for God: 1. Pray daily for self and others 2. Receive the Sacraments 3. Participate in the activities 4. Be open, flexible, and have a servant's attitude 5. Represent God in your words and actions How to show love and respect for yourself: 1. This is a "no smoking" weekend. All state laws governing alcohol, drugs and tobacco will be strictly enforced. Possession or consumption of alcohol and drugs is not permitted at any time during the conference. 2. Drink plenty of water, and make sure you eat all meals. This will allow you to fully participate and not be tired. 3. Remember that you are the Temple of the Holy Spirit, present yourself accordingly. 4. If you must leave an activity, your adult chaperone should accompany you. 5. Dress modestly Clothing must cover all undergarments and midriffs. Bikini tops, low cut tops, mini skirts, short shorts, and shirts with vulgar language are not allowed. How to show love and respect for others: 1. Be safe. No horseplay or other potentially harmful actions. Leave pocket knives, lighters, or other hazardous materials at home. No skateboards, ipods, laptops or rollerblades. 2. All words and actions you use should build up others and not injure. 3. No teenagers are allowed to drive to or from the conference. 4. The facility must remain clean and undamaged. 5. No outside visitors at the conference please. 6. All aisles and the stage area must remain clear for safety reasons. Remain in your youth group's area at all times. 7. Make sure that your actions during the activities do not distract others from hearing, seeing or praying Consequence Policy: 1. All youth are expected to follow the above outlined expectations, direction of security and conference staff. Any instances of transgression of these policies, lack of cooperation or insubordination will be subjected to appropriate discipline and/or fines. Failure to comply may result in immediate dismissal of the participant, at the expense of the individual.

Modesty Policies for All Conference Attendees Catholic Servant Ministries, Inc. has very specific guidelines regarding modest dress. We ask that you strictly adhere to these policies. Also, please keep in mind that some rules apply specifically to males while others are directed to females. 1. Shorts, skirts or dresses that are shorter than five inches above the knee while standing are prohibited. 2. Skirts that reveal midriff are not permitted, nor are shirts that are too low cut from the neck area. 3. Shirts, shorts or pants are not allowed to sag to the point of revealing one's undergarments while standing. 4. During all liturgies and prayers, every male, youth or adult must remove and headwear. This includes all hats visors, headbands, bandanas, ect; this is more out of respect than modesty. 5. Clothing that may appear to have an anti-christian message will not be permitted. 6. Shirts with writings that include crude language, promotion of alcohol or drug use, or suggestive material of a crude, sexual nature are prohibited. 7. There will be no tolerance for cross dressing. 8. Any individual wishing to serve as an Extraordinary Minister of Holy Communion for the Liturgical celebration is asked to wear long pants or a skirt. 9. At such events where music is being played, many people often feel moved to dance. While we encourage, we feel it is necessary to remind you that no moves that appear tasteless and vulgar will be permitted. We invite you to have a great time, but please keep it clean! "Purity requires modesty, an integral part of temperance. Modesty protects the intimate center of the person. It means refusing to unveil what should remain hidden. It is ordered to chastity to whose sensitivity it bears witness. The event will take place between July 27, 2018 and July 29, 2018, and will involve the following activities: speakers, small group discussions, Mass, meals, praise & worship with live music, Exposition of the Blessed Sacrament, overnight hotel stay in a resort hotel with swimming pools, and opportunities for Reconciliation. I understand and assume the risks inherent with such an extended field trip, which may involve certain risks beyond the reasonable control of the Parish and the Diocese of St. Petersburg and their respective representatives, volunteers and agents. Such risks may include but not be limited to accidents, emergencies, exposure to nature and the elements, and/or negligence of the Adult Chaperones and staff of the entity at the location of the event, the Parish and the Diocese of St. Petersburg disclaim any and all responsibility for any such risks. I am also aware that all reasonable care and supervision will be exercised by the Adult Chaperones to provide for the general well-being of my youth. However, I understand that there will be times when my youth is not directly supervised and it is impossible for the Adult Chaperones to supervise my youth every minute of every day while attending this event. I acknowledge placing my trust in my youth to adhere to proper standards of conduct and to follow the rules of the Group Leaders and Adult Chaperones, especially when not being directly supervised. I understand and assume the risks described above and those inherent with such an extended field trip. In consideration for the benefits my youth will receive in attending this event, I, individually and on behalf of my youth, do hereby RELEASE, DISCHARGE AND COVENANT NOT TO SUE the Bishop of the Diocese of St. Petersburg, the Parish and all parishes within the Diocese, all employees, agents and volunteers for this event, and agree to hold them harmless from any and all claims whatsoever arising from the conduct of any person(s) which result in any injury or loss to my youth, or myself. MEDICAL PERMISSIONS FOR YOUTH: If on the day of travel to the location, my child should have any symptoms of headache, vomiting, sore throat, cold, fever, flu, diarrhea, onset of any contagious illness, or should otherwise not attend the conference for health reasons, I agree that I will not permit my child to travel to the location. Before leaving the Parish, or during travel to or attendance at the location, in THIS FORM CONTINUES ON THE REVERSE SIDE OVER

the event it comes to the attention of the Parish that my child shows up with any illness or there is an accident or emergency, I agree that in the sole discretion of the Parish, my child may be sent home immediately without any liability to the Parish or the Diocese of St. Petersburg. I have given consent for emergency medical treatment that may become necessary at the time of registration. I hereby ratify and incorporate that consent by signing below. Further, my youth is in good health and I am not aware of any medical conditions that would impair or prevent my youth from attending this extended field trip. PERMISSION FOR OTHER MEDICAL MATTERS: In the following, check ONLY those that apply to your child: YES, if upon leaving home I know my child is to be taking prescription or non-prescription at the time of this event, I give permission to the location s medical staff or Parish staff to administer the medication to my child, provided, however, that it is my responsibility to send with my child the appropriate quantity of clearly labeled medication showing dosage and frequency and to speak to a chaperone about this in advance. I understand that the Parish cannot be responsible for my failure to send the appropriate quantity of medication or for errors in the dosage and frequency due to any cause whatsoever. YES, in the event it comes to the attention of the Parish that my child complains of illness, I grant permission for nonprescription medication (such as aspirin, throat lozenges, cough syrup) to be given to my child by the location s personnel or Parish personnel. CODE OF BEHAVIOR: I agree to instruct my child to abide by all rules and regulations including the Parish Handbook, that are imposed for this extended field trip, that are sometimes referred to as a Code of Behavior ( the Code ). I understand that if I have not previously seen the Code, it is my duty to seek a copy of the Code and to review it and to explain it to my child prior to signing this waiver. I agree that if my child fails to abide in any way by the Code, that my child can be dismissed from the location and sent home immediately at my expense for the immediate transportation home with no right of reimbursement or refund for any amount in connection with such transportation from the Parish. I fully understand the consquences of the foregoing statements and sign this Consent Form and Liability Waiver knowingly, freely and willingly.
