St. Elizabeth Ann Seton Confirmation Requirements In order to be a candidate for High School Confirmation at St. Elizabeth Ann Seton, the prospective student must meet the following requirements:! High school student 15 years of age or older by January 1, 2015! Be currently enrolled and regularly attending the entire year (September May) of Catholic religious education! Attended a full year of Catholic religious education the previous year (ex. if confirmation is done in 10 th grade, candidates must have attended religious education in 9 th grade)! If religious education classes are done at SEAS, youth may not miss more than four (4) classes in one semester. Missing more than four will be a violation of regularly attending religious education for a full year.! Be baptized and able to renew baptismal promises! Be open to receiving the gifts of the Holy Spirit! Family must be parishioners at St. Elizabeth Ann Seton & attend Mass regularly.! Be willing to commit to active involvement in the parish Confirmation process! Be willing to continue to grow in faith following Confirmation and beyond

Confirmation Sponsor Information Your Confirmation sponsor should be someone whose Catholic faith inspires you. You need to feel comfortable talking about your faith to this person. The requirements are: Cannot be the parent of the Confirmation Candidate. Must be 16 years of age or older. Must be baptized and confirmed in the Catholic Church. Must be an active, practicing Catholic (i.e. if married, marriage must be recognized by the Catholic Church; attend Mass regularly & participate in the parish). NOTE: If your sponsor is not registered at St. Elizabeth Ann Seton, you MUST get a letter from their parish confirming their eligibility no later than Dec. 1, 2014. We cannot accept sponsors without this letter. Attach the letter to this form. If your sponsor is unable to attend the Confirmation rehearsal and Mass, you may have a parent, or another person stand as their proxy. Your sponsor s name will be the sponsor of record even though they were unable to attend. This Form Must Be Turned in No Later than Sunday Dec. 1, 2014. Community Service Hours Serving God's People Matthew s account of the Last Judgment tells us When the Son of Man comes in his glory, and all the angels with him, then he will sit on his glorious throne. Before him will be gathered all the nations, and he will separate them one from another as a shepherd separates the sheep from the goats, and he will place the sheep at his right hand, but the goats at the left. (Matthew 25:31-32 RSV). How will the people be judged? When we serve those who are hungry, thirsty, imprisoned, and lonely, we in turn serve Jesus. An important, but often overlooked part of our Confirmation preparation is taking some time this year to serve those in need. While serving others, we are asked to look for God in those we serve. Be aware of Jesus living in the person or persons that you are helping. From August 15, 2014 to May 4, 2015, you are asked to perform AT LEAST 20 hours of service to those in need. If you don t wait till March or April, you should have no problems fulfilling this commitment. Here are some suggestions for service ideas: Babysitting in the nursery during Mass (you must still attend a Mass) Liturgical Ministries (Lectors, Ushers, Altar Servers, Choir, etc.) Being an aide for Elementary Religious Education Nursing Home Visits Volunteering at the Keller Community Storehouse Volunteering with St. Elizabeth Ann Seton Social Services Service & Parish Life Hour Log Deadline is May 4, 2015

Parish Life Hours Being an active member of the Body of Christ From the Catechism of the Catholic Church: 1309 Preparation for Confirmation should aim at leading the Christian toward a more intimate union with Christ and a more lively familiarity with the Holy Spirit his actions, his gifts, and his biddings in order to be more capable of assuming the apostolic responsibilities of Christian life. To this end catechesis for Confirmation should strive to awaken a sense of belonging to the Church of Jesus Christ, the universal Church as well as the parish community. The latter bears special responsibility for the preparation of confirmands. It is in response to this call from the Catechism that we ask each candidate preparing to celebrate the sacrament of Confirmation to participate and to play an active role in the Faith life of our vibrant community here at St. Elizabeth Ann Seton. These Parish Life Hours are different from community service hours in that instead of spending time in service towards others (either here at the parish or in the larger community), we are asking candidates to spend time and energy in growing their own Faith here at St. Elizabeth Ann Seton. From August 15, 2014 to May 4, 2015, you are asked to perform AT LEAST 15 hours of parish life involvement. If you don t wait till March or April, you should have no problems fulfilling this commitment. Here are some suggestions: Attending our Youth Group (Wednesday nights 6:30pm - 8:30pm) Attending our Youth Bible Study (Monday nights 6:30pm - 8:30pm) Attending any of our Youth events (i.e. retreats, conferences, youth holy hours, March for Life pilgrimage to Washington DC, etc.) Attending any parish sponsored Faith centered events (Fridays for Life Speaker Series, Lenten Mission, etc.) Attending parish sponsored weekly classes/gatherings (Msgr. Parent Parish Life Hours Being an active member of the Body of Christ From the Guidelines for the Preparation and Celebration of the Sacrament of Confirmation for the Diocese of Fort Worth: III. Policies D. THE PARENT(S) OR GUARDIAN OF THE ADOLESCENT CANDIDATE Parent(s)/guardian are called to be actively involved in the Confirmation preparation process. The role of the parent(s)/guardian is an extension of the commitment they undertook at their child s Baptism. The family serves as the primary community of faith. As children grow in faith, the parental role lessens in regard to decision-making and becomes one of support and witness. For the Sacrament of Confirmation for adolescents, the parental role is very much one of spiritual companion and advisor, that is, both parents and adolescents learning and growing in understanding of Catholic traditions and beliefs. This points to the understanding that forming faith is a lifelong process and that parents, as well as adolescents and children, should continue their own faith growth. It is in this way that parents will be able to share their own faith journey, thereby helping the candidates to make decisions about their faith. Parents must: 1. Continue their own faith growth. 2. Indicate a willingness to support the candidate s faith journeys. 3. Support their adolescent candidates in choosing sponsors. 4. Support their adolescent candidates in prayer throughout the preparation process It is in response to this call from the Sacramental Guidelines that we ask the parents of the Confirmation candidates to participate and to play an active role in the Faith life of our vibrant community here at St. Elizabeth Ann Seton. The requirement of the Parent Parish Life Hours asks parents to be an active part of our community, and to grow in their own Faith. In doing this, parents are better able to fulfill their role in the spiritual formation of their children (CCC 2221). From August 15, 2014 to May 4, 2015, parents are required to perform AT LEAST 10 hours of parish life involvement. Here are some suggestions: Chaperoning any of our Youth events (i.e. retreats, conferences, youth holy hours, March for Life pilgrimage to Washington DC, etc.) Chaperoning the confirmation retreat (*note, to chaperone any youth event, parents must be current with the Keeping Children Safe certification) Attending any parish sponsored Faith centered events (Fridays for Life Speaker Series, Lenten Mission, etc.) Attending any of our adult religious education programs (visit www.seascc.org to see all of the different options that we offer regarding religious education for adults) Parent Parish Life Hour Log Deadline is May 4, 2015
