Faith Formation & Youth Ministry Registration Checklist Complete these simple steps to get your family registered for the upcoming year. Review the program information for your child(ren). Tuition For planning purposes the registration form and tuition must be received by Sept. 15 to avoid delay of your start date. Families who are registered parishioners and attending Mass at Holy Family Parish: Early Bird: Complete Registration & Payment Received before Aug. 15: $35 per child ($105 max per family) On Time: Complete Registration & Payment Received after Aug. 15: $45 per child ($135 max per family) Families who are not registered parishioners or families who are not attending Mass at Holy Family: $65 per child Book Fees: 2nd gr. / first Communion Book Fee: add $10/child ( Gold book is kept as a memento and is not re-usable; cost is $21.25 new) 6th gr. / Catholic Youth Bible fee: add $10/child (Youth Bible is kept by student and used in CFF MS classes; cost is $22.36 new) If you have any questions about completing the payment section of the registration form, please call the CFF office. In-Home Program: There is no fee for the parent guide which is now available online; however if you request the parent guide to be printed for you, there is a $5 fee. There is $10 book fee per student textbook, which the student uses therefore the books are not returned. (ex: A family with 3 students will pay $30 if using the online parent guide online or $45 if the parish is printing the guides per grade for you.) Tuition for children of Catechists in the CFF Elementary and MS/HS Core Team weekly program, as recognition for their certification and /talent/resources that they put into their ministry every week, will be waived. There is no tuition discount for Early Childhood Parent Aides due to the less frequent commitment and certification not required. Aides and other volunteers who commit to a weekly position will receive a 50% discount. (See volunteer descriptions for tuition discounts.) No one will be turned away if unable to pay the tuition. If you are not financially able to cover the tuition at this, please mark the statement on the registration form or contact the Faith Formation Office, to discuss. Either way, your situation will be held confidential and handled with respect. 4

Sacramental Preparation Eucharist and Reconciliation Preparation & Celebration Children who are baptized and have completed 1st grade Faith Formation and starting 2nd grade Faith Formation are eligible for preparation for the Sacraments of Eucharist and Reconciliation. 2nd graders must register and attend weekly CFF classes. (Complete the registration forms on p. 15-18.) There is a meeting scheduled in early October for parents who desire the preparation of their child for these Sacraments. Meeting date TBD and will be emailed to you with class assignments in late September. Brochures and registration forms will be provided at the parent meeting. Enrollment of Candidates for the Sacrament of Holy Communion will be at the 5:00pm Mass on Saturday, November 3. A book fee of $10 is due with tuition for Eucharist preparation to help cover the cost of the Gold Book. NOTE: In September a 1st grade Catholic Foundations Catch-up class is held for those missing their 1st grade Faith Formation. For more information, contact the CFF office Confirmation Preparation & Celebration: Incoming 8th grade students who are baptized and have completed 7th grade Faith Formation are eligible for the Sacrament of Confirmation. 8th graders will be Confirmed in the Spring of 2019 at the Cathedral of St. Andrew (date TBA). 8th graders must register and attend Confirmation classes. (Complete the registration form on p. 15-18) There is a one hour meeting for parents who desire the preparation of their child for this Sacrament on Tuesday, Oct. 9 at 7 PM at Holy Family. Brochures and registration forms will be provided at the parent meeting. Enrollment of Candidates for the Sacrament of Confirmation will be at the 5:00pm Mass on Saturday, November 3. NOTE: If your child was not here for 7th grade Faith Formation, you will be contacted by us to see if any missed Faith Formation preparation is needed. Other Ages or Circumstances Baptism For children ages 0-6 years, who have not reached the age of reason: please contact Christine Shafer for infant Baptism information. For unbaptized children/youth ages 7+ years, who are considered to have reached the age of reason: please contact Christine Shafer, Director of Faith Formation, for The Rite of Christian Initiation information, which includes preparation for the three Sacraments of Initiation (Baptism, Confirmation & Eucharist). The Church provides this process for those older than the age for infant baptism (age 7+). For those Baptized in another Christian Tradition (other than Roman Catholic): please contact Christine Shafer to discuss. Reconciliation For children/youth 3rd grade and up: contact Lesley Jeruzal for preparation & celebration information. Eucharist For Baptized children in 3rd grade: preparation/ celebration will be with the 2nd grade program. See description to the upper left. For Baptized children in 4th grade and older: preparation for Holy Communion will be with several other children alike in age who are also preparing for their first Communion. Contact Christine Shafer for information. For children 7+ years old who are not baptized: please contact Christine Shafer for The Rite of Christian Initiation (Baptism, Confirmation & Eucharist). The Church provides this process for children to celebrate all the Initiation Sacraments at one since they have reached the age of reason in the eyes of the Church. Confirmation For Baptized or unbaptized youth in gr. 9-12: please call Christine Shafer to discuss preparation and celebration. For Adults thinking about becoming Catholic, whether baptized or unbaptized, Catholics wanting to come back to the Church, or Catholic adults wanting to be Confirmed, please contact Christine Shafer. Faith Formation for Children with Special Needs We would like to help provide Faith Formation for all of our parish children in the way that best suits the child s formational needs. We can offer help to children with special learning, behavioral, emotional or physical needs in a number of ways, depending on the level or type of need. One way is by pairing the child up with a Special Friend. A Special Friend is a volunteer in the classroom matched to the child to offer aid, encourage peer support, and provide individualized instruction when needed, helping create an atmosphere of acceptance and friendship where the special needs child can feel welcome within the regular classroom. If a child would benefit more from an alternative Faith Formation program (perhaps small group or individual) rather than our mainstreamed classroom format, we can work with the parent/s in providing the appropriate formation. If your child has a special learning, behavioral, emotional or physical need, and you would like to arrange some type or level of help for your child s formation, please mark a star (*) on the registration form next to the child s name (p. 16), or contact Christine Shafer. You will be contacted via email for more detailed information regarding your child and his/her needs in late September to discuss your child s need, and the type or level of assistance. Also, if you would like to help with this ministry, please contact Christine Shafer or mark that on the Time & Talent section of the registration form (p. 17). Program Some families elect to provide religious formation for their children through home-study rather than our formal parish Faith Formation classes. Our parish provides resources, textbooks and support to those parents choosing this method. The parent is the catechist for his/her family only, and is provided with a manual and student textbooks. The program allows the family to study one theme at the same, but at age appropriate levels. This program will take approximately 30-60 minutes per week and includes a variety of topics. Parents are informed at the beginning of the year of various projects and activities happening in the CFF grade levels, and are invited to contact the CFF office to get their children involved in those events. Parents selecting In-Home Faith Formation must be certified as a Catechist with the Diocese of Grand Rapids. Certification material will be provided with the In-Home materials. Parent Catechists in the In- Home Program option are only certified for the catechesis of their own children. There is no fee for the parent guide which is now available online; however if you request the parent guide to be printed for you, there is a $5 fee. There is $10 book fee per student textbook, which the student uses therefore the books are not returned. We ask that those parents who provide In-Home Faith Formation please mark that box on p. 16 of the registration form so that our parish has accurate Formation records and can contact families to provide them with Diocesan approved materials. A parent orientation can be scheduled in the fall by contacting Christine Shafer. Please Note: Because of the communal nature of the Sacraments of Eucharist, Reconciliation and Confirmation, students must be prepared through weekly parish Faith Formation classes, and not through the In-Home program. Social Media Holy Family s Internet Use Policy for its employees establishes strict guidelines as to proper conduct on the Internet and the appropriate content of communications, blogs, and websites. Behavior must adhere to the Diocesan Standards of Ministerial Behavior in Dealing with Children and Young People at all s. As church workers who care deeply about our young people, we urge parents to regularly monitor their children s internet, electronic devices and social networking activity. The Virtus, Michigan CSI, and MI Attorney General s websites have great information on the parental role in internet safety. We intend to communicate regularly with students via Social Media. The Diocese of Grand Rapids does not allow ministry participant minors to interact with employees of the parish without written consent from the parent or guardian. Your permission is requested on the registration form. We encourage you to Like our pages as well. Adult Faith Formation Growing in our faith is a lifelong process that doesn t stop with our traditional schooling. The Catholic faith is deep in richness and truths which we come to know as we grow in age, wisdom and grace. At Holy Family, there are any number of ways to participate as an adult in your ongoing faith formation. There are small groups in the Fall and Winter/Lenten months which are described in the parish bulletin and newsletter for sign-up purposes. We regularly offer the DVD course CATHOLICISM: Journey of a Life by Fr. Robert Barron as a foundational course for all adult Catholics in the parish, knowing that we all need updating from our youth faith formation. We also offer a variety of speakers, religious exhibits, and our parish mission. Further, we provide Lighthouse Media CD s for sale ($3) in our Narthex on a variety of engaging topics for formation in your car or at home; these are also available as podcasts on their website. We are also blessed with an extensive Parish Library where parishioners can form themselves by checking out books and media. We also encourage adults to listen to Catholic radio at AM1140 as this is a great resource for learning more in the faith. Finally, there are a number of quality Catholic online mini-courses available for adults advertised in our parish newsletter. Early Childhood Faith Formation (older 3, 4 & 5-year-olds) Classes for young children are offered on Sunday mornings during the 9AM Mass. This program will explore God s love, God s creation, and some basics of our Catholic faith. This is done with children through ageappropriate and fun activities, crafts, games, songs, prayer, and Bible stories. Parents will be scheduled to volunteer in their child s class as an aide approximately every 4 weeks, and will need to complete the requirements for our safe environment, including a background check & Protecting God s Children training. There is no tuition discount for Parent Aides due to the less frequent commitment. Parent Aide schedules will be e-mailed in late September. To be eligible for these classes, children: must be potty-trained. must be 3 years old by June 1, 2018. must be in Kindergarten or younger. need to be developmentally ready to be separated from you, their parent, while you go to Mass. need to be able to adjust to a class in the morning, as somes children are too tired for this program, and end up unhappy and unwilling to stay for the class. Kindergartener s Parents: You can choose Sunday morning Early Childhood or Kindergarten CFF in the Elementary CFF program on Mondays and Wednesdays; see p. 9. Early Childhood 2018-19: Early Childhood classes are held Sundays during the 9am Mass from Oct. 7 - Apr. 28 No classes on: Nov. 25, Dec. 23 & 30, Mar. 31 and Apr 7 & 21 8

Elementary In our Elementary classes, students explore a variety of Catholic Christian teachings and practices, including sacraments, morality, prayer, social justice. Using our spiral curriculum, our children are formed in the richness of our faith year after year, going deeper in their journey to know, love and serve the Lord, Christian service, the Trinity, the Church, and Scripture. Catechists present lessons involving a diverse array of activities and learning/teaching methods to best help children grow in their faith. Students also participate in large group that incorporates our community life and develops deeper relationships among the children and God. It should be noted that students preparing for the Sacraments of first Eucharist and first Reconciliation (2 nd gr.), must attend Catholic Faith Formation classes, at a minimum, in both the year prior to preparing and the year of preparation (excerpt from the Parent Handbook). Components to The Source Middle School program: Within the 3 years of Middle School Faith Formation, students will have completed learning about the entire Catechism of the Catholic Church in a fun and engaging way. Students will be in a group setting with their peers and will build community while learning, praying and growing closer to God. Outside of the regular program, we also offer students several spiritual and social events to attend throughout the year such as retreats, rallies and many more. Many of these events are coordinated with other churches throughout our diocese and will include over 400 middle school students. Registration and further information on these events is communicated throughout the year. It should be noted that students preparing for the Sacrament of Confirmation must attend Catholic Faith Formation classes, at a minimum, in both the year prior to preparing and the year of preparation (excerpt from the Parent Handbook). 6th gr. The Summit program allows young people to be open to the Holy Spirit through social, educational, and spiritual experiences to build upon their foundation of the Catholic faith and become spiritual participants within the Church now and into the future. The Summit program emphasizes how the Catholic faith is relevant for teens today. It is a four year curriculum based on the USCCB s high school framework of catechesis. Outside of the weekly program, we offer students several spiritual and social events throughout the year, such as retreats, conferences, service trips, and so much more!
