LOSS OF PROPERTY: Parent understands that neither St. Mary & Archangel Michael Coptic Orthodox Church nor any other Released Party is responsible for loss or damage to Participant s personal property or possessions in the absence of willful misconduct by such persons. Mobile communication devices (including cell phones), and any device with the capability of digital photography are strictly prohibited. Parent and Participant understand that St. Mary & Archangel Michael Coptic Orthodox Church STRONGLY discourages participants from bringing valuables such as radios, walkmans, CD players, I-pods, CDs, game-boys and expensive jewelry, etc.. If Participant chooses to bring such items, Participant does so at his or her sole risk. 7. MEDICAL RELEASE: I agree to allow the ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH volunteer staff to render care to, arrange transportation for and administer overthe-counter medications to, the named minor, within the volunteer staff scope of practice, and as deemed beneficial to the health and well-being of the named minor. I further agree that all over-the-counter and prescription medications brought to camp will be collected by and then only administered by the ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH volunteer staff, in accordance with all applicable prescriptive directions and/or on an as needed basis. No medications having reached an expiration date will be accepted or administered. In the event I cannot be reached by phone at the time of an injury or illness to the named minor, I hereby give, as parent/legal guardian, my permission to the doctor selected by ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH to hospitalize, access and procure necessary medical records, and secure appropriate treatment, including but not limited to injections, anesthesia, testing, radiology, or surgery for the named minor as deemed necessary. I understand and am aware that ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH does not provide any form of medical insurance coverage for the named minor. I release ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH from any and all medical costs and expenses rendered in the treatment on named minor. Furthermore, I accept complete financial responsibility for any medical expenses incurred for the named minor through the usage of my policy. In signing this Liability Release, I assume full responsibility for mitigation of such an incident, and I am granting permission for the participation of the named minor in all session related activities, unless specifically noted on this form. Should the named minor elect to participate in an activity off of the grounds of ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH, it is understood and agreed that transportation, to and from the activity shall be the responsibility of individuals as designated by the parent/legal guardian. This release is intended to stand on the behalf of the named minor, and in place of all claims by any family member or agent. These releases of ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH shall be in effect only for the duration of the camp session as indicated and only while the named minor is on the grounds of ST. MARY & ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH, and/or under the directed supervision of ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH volunteer staff. I, in my own behalf and on behalf of Minor, acknowledge and agree that such participation subjects Minor to possibility of physical illness or injury (minimal, serious, catastrophic and/ or death) and that I,
in my own behalf and on behalf of Minor, acknowledge that Minor is assuming the risk of such illness or injury by participating in the Activities. In the event of such illness or injury, I authorize ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH to obtain necessary medical treatment of Minor and hereby, in my own behalf and on behalf of Minor, release and hold harmless Releases in the exercises of this authority. I further acknowledge and understand that I will be responsible for any and all medical and related bills that may be incurred on behalf of Minor for any illness or injury that Minor may sustain during the Activities and while traveling to and from the site for the Camp whether or not the Camp actually occurs. 8. DAMAGE TO ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH AND OTHER S PROPERTY: Parent and Participant understand that Parent shall be liable for any damage to the property and/or facilities of ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH or of the property and/or facilities of others, including other participants, resulting in whole and/or in part from acts of Participant. 9. Appearance Release: I understand that ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH from time to time may produce promotional material relating to its programs. I understand as a participant and/or a spectator, that Minor may be included in videotapes and/or photographs taken during the Activities. Furthermore, Parent, for himself or herself and on behalf of the Participant, gives permission and consents to the taking of photographs, videotapes, and interviews of Participant, and agrees that such photographs, videotapes, and/or interviews may be published and used for advertising, promotion, publicity, or recreational viewing by Parent, other Participants and their families, as well as other persons, in publications and/or internet web sites related to ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH. Therefore, without reservation and/or limitations, I, in my own behalf and on behalf of Minor, hereby assign, transfer and grant to ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH, its successors, assignees, licensees, sponsors, any television networks, and all other commercial exhibitors the exclusive right to photograph and/or videotape Minor and to utilize such videotapes and photographs and Minor's name, face likeness, voice and appearance as part of the Activities, in advertising and promoting the Activities and/or advertising and promoting similar Activities. I further understand that neither ST. MARY AND ARCHANGEL MICHAEL COPTIC ORTHODOX CHURCH nor any third party is under any obligation to exercise any of the foregoing rights, licenses and privileges. AND I, the minor s parent and/or legal guardian, understand the nature of these activities and the minor s experience and capabilities and believe the minor to be qualified to participate in such activity. St. Mary and Archangel Michael Coptic Orthodox Church Camp Code of Conduct, Guidelines and Dress Code St. Mary and Archangel Michael Coptic Orthodox Church MCP is committed to providing a Christian environment in which all participants are treated with respect and dignity. These guidelines are in place so that the campers can have the most fun possible, without fear of injury or danger. Parents should share these rules and guidelines with their children before the first day so the campers know what they can expect at camp. Campers should keep in mind, that while attending camp events, campers are representing the Lord Christ and St. Mary and Archangel Michael Coptic Orthodox Church MCP. Camper Code of Conduct and Guidelines Everyone is expected to participate in all planned activities as defined by the camp schedule and to be in appropriate attire. Chaperones are responsible for ensuring that campers participate in all sessions of the planned program activities, unless excused by the camp coordinator. All campers are to be in their assigned areas at curfew and to comply with quiet time and lights out. Campers must sleep in assigned dorm areas; no boys in girls dorms or bathrooms and no girls in boys dorms or bathrooms. The possession and use of alcoholic beverages, tobacco, and/or drugs, other than prescribed medication is prohibited. Medications must be kept in the assigned designated areas as directed by the camp coordinator. No matches, lighters, chewing tobacco or smoking at camp. No camper may leave the grounds without permission from the camp coordinator. All campers 17 years of age or younger may only be checked out by parent/legal guardian from camp; however, once the camper is checked out the camper may not return to the camp for the remainder of the weekend. Setting off fire alarms, tampering with fire extinguishing equipment, and/or security/safety equipment is prohibited. Gambling and betting is prohibited. Obscene and discriminatory language, roughhousing, running through camp, throwing things, and insubordination will not be tolerated at any time. Campers must demonstrate respect to each other and to elders at all times. Abiding by the dress code is required. Sickness or injury must be reported to the camp chaperone and coordinator immediately, no matter how minor. Any medication or prescription drugs are to be turned in to the camp appointed representative and shall only be administered by the camp representative. In matters of dress, MODESTY must prevail. Halter tops, short shorts, etc. will not be allowed. Campers will wear shoes at all times. Non-marking / non-skid sneakers are required, due to the fact the camper will be participating in games that require proper support and protection for feet. Heelys, cleats, or any shoes
which may cause damage to floor surface etc. are prohibited. All camp volunteers and representatives must and will enforce the dress code. (Please see the Camp Dress Code Below) No pajamas or sleeping garments are to be worn outside of dorm rooms. Campers are discouraged from leaving camp for outside activities, such as ball games, etc. Please do not deface camp property. It is your facility, too--help keep it looking good. Intentional damage or damage due to unnecessary carelessness may result in the camper being charged for damages, and/or further disciplinary actions. Radios, CD/MP3 players, magazines, comic books, cell phones, laptops, and any other electronic games or devices are a distraction to camp activities and are prohibited. The Camp chaperones and coordinator have the right to confiscate such items. No camper is to be out of his/her bunk and/or dorm rooms after lights are out. Please observe all boundaries, guidelines and rules arranged by the Camp. Food is not permitted outside of the banquet hall; it attracts bugs and uncleanliness. Gum and Candy etc. are not permitted outside of banquet hall. Telephone calls may be made during game/gym and break times only with the permission of camp chaperone. No camper will be permitted in the camp kitchen area. DO NOT move or put the beds together. Campers will stay within the boundaries of the CYC and Church Buildings and will remain with their chaperons at all times. If a camper leaves the premises, parents will be informed immediately. Campers will remain at their program areas until dismissed. Campers will respect others, both physically and emotionally. Campers will respect CYC, church property/building and other campers property. Abuse of any equipment or property, theft and vandalism will not be tolerated and parents will be notified immediately. Campers will behave appropriately at all times. Campers will obey their chaperones and instructors at all times. Campers are responsible for policing their dorm areas and eating areas for trash, as well as picking up after themselves during activities. It is the policy of the CYC Camp that we will leave the camp cleaner than we found it. All Camp rules are to be followed at all times. Campers are encouraged to ask questions if they do not understand a rule or a consequence. Camp utilization rule: You get it out, you put it back; you mess it up, you clean it up; you break it, you buy it; you abuse it, you lose it. The Camp Director reserves the right to dismiss any camper or refuse admission to anyone who is not observing Camp Code of Conduct, Rules, Guidelines and Dress Code, etc. No refunds will be made. This policy is not all inclusive; the MCP Leaders have the right to make modifications as deemed necessary. Penalties/Consequences for Infractions: (See Discipline Point Policy for details) Any or all of the following may be administered for infractions: sending a member home; barring that member from future MCP and/or CYC events; assessing the member the cost of damages and repairs in the event of damage/destruction of property. Parents will be notified immediately of action taken. If a member is sent home, fees will not be refunded. Parents should share these rules and guidelines with their children before the first day so that the child knows what they should expect at the camp. Personal Conduct: 1. Safety is first. 2. Cooperation and participation are required both in group activities and with other participants. ATTENDANCE - Required at all classes, services, activities and punctuality is expected. 3. Respect those around you, both other participants and servants/chaperones. No physical conflicts or fights. 4. Respect for property. 5. Appropriate language and attire at all times. 6. Do not borrow from others without permission. 7. Be a good sport. 8. No one of the opposite gender will be allowed in the other gender s designated dorm rooms for any reason whatsoever. 9. Dorm rooms should be clean and tidy at all times. 10. No unnecessary noise, running or horse-playing. 11. Participants may not leave church grounds for any reason during camp. Please be advised that the following behaviors will detract from a positive camp experience: physical aggression, stealing, defiance in following instructions, and/or repeated incidents of inappropriate language and/or actions. NOTE: Please leave valuables at home (rings, jewelry, electronics, etc.). The Church, servants and volunteers will not be responsible for lost or stolen items. THINGS NOT TO BRING: Magazines, tape/cd s / DVD players, video games, electronic games, cell phones (if brought, must be left in suit cases), laptops, TV s, firearms, fireworks, knives, cigarettes, t-shirts with worldly advertising or associations, or any other apparel that does not comply with modest Christian attire standards. Dress Code: All clothing should be clean, neat and tidy, free of tatters and tears with no worldly logos or advertisements. No spaghetti straps, strapless clothing, short shorts or clothing that is low cut or exposes the midriff please. Modest long shorts and other appropriate attire may be worn by all during activity times only. Attire for Prayer Services and Liturgies: Proper church service attire is expected during these services. Please do not wear shorts or sleeveless attire during these services. All attendees will be considered to have agreed to uphold the letter and spirit of all camp code of conduct, rules and guidelines.
