FAMILY LIFE CENTER Reservation Policies, Procedures and Regulations Please note the following: 1. All guidelines for reserving and using the Family Life Center are contained in the Policies and Procedures manual. Please review it thoroughly prior to placing your reservation. 2. To reserve the Family Life Center you must submit a Family Life Center Building Use Application and your application must be approved by the Deacon body. 3. You are required to submit the appropriate rental fee with your application. See Policies and Procedures for a fee schedule. 4. Applications for general use of the Family Life Center must be submitted at least 35 days prior to your event. 5. Applications for use of the Family Life Center for basketball practices only do not require preapproval by the Deacons. You must submit one Use Application that can remain on file for all basketball events. To reserve, simply contact the church office to confirm availability and pay the required fee prior to the basketball practice 6. Your event is not permanently reserved until you are notified that the Deacon body has approved your request. 7. If you are a member of First Baptist Church, you are required to agree to be personally and financially responsible for any damage caused to the Family Life Center or to equipment owned by First Baptist Church. 8. If you are not a member of First Baptist Church, your application for use of the facility must be sponsored by a member of First Baptist Church. Both you and the sponsoring member must agree to be personally and financially responsible for damage caused to the Family Life Center or to equipment owned by First Baptist Church. 9. FIRST BAPTIST CHURCH FAMILY LIFE CENTER BUILDING USE APPLICATION Thank you for your interest in using the First Baptist Church Family Life Center! Please complete this application in its entirety to help us properly evaluate your request. You are also responsible for reviewing, understanding and agreeing to the document labeled Family Life Center Policies and Procedures, attached hereto. Upon completion of this application and tendering of the appropriate fees, we will tentatively reserve the Family Life Center for your event. Your event will not be permanently reserved until it is approved by the Board of Deacons or their designee. You will be contacted once your event becomes permanently reserved. First Baptist Church FAMILY LIFE CENTER Policies and Procedures GENERAL INFORMATION Address: 1090 Southwest Drive Kennett, MO 63857 Phone: (573) 888 9899 Web: http://www.fbckennett.com/reservations The mission of the Family Life Center is to enhance the lives of families by providing space and activities to grow physically, congregationally and spiritually through activity, fellowship and ministry. First Baptist Church constructed the Family Life Center to be used as both an outreach tool for the city of Kennett and as a house of worship for our members and guests to glorify God. We pray that the building serves both purposes well and trust that God will use the facility to strengthen the spiritual walk of our church family and the surrounding community. Use of the building is governed by the deacon body and, ultimately, by the members of First Baptist Church. The deacons of First Baptist Church are tasked with the responsibility of developing policies and procedures for use of the building and have the authority to enforce those regulations as developed. The deacon body may, from time to time, designate other members of the church to fulfill certain roles and responsibilities relating to the governance of the Family Life Center. All use of the Family Life Center by individuals or entities other than First Baptist Church or its affiliates must be approved in advance by the Board of Deacons or their designees. RENTAL PROCEDURE Any individual or group interested in using the Family Life Center shall complete a building use application and submit the required fees. The application is available in the church office from the church secretary and is also available online at www.fbckennett.com/reservations. Applications for use (excluding basketball practices see below) must be completed and submitted at least 35 days prior to the event to ensure action by the deacon body. All applications for use must be approved by the deacons of First Baptist Church or their designee and are not final or guaranteed until approved by the deacon body. The Board of Deacons reserves the right to reject any application for use of the Family Life Center. Deacon s meetings are typically held on the first Sunday of the month at 7:00 PM. For basketball practices only, pre approval of the reservation is not required by the Deacon body. The event sponsor must complete one use application that can remain on file at the church for all basketball practices hosted by that event sponsor. The required fee must be deposited prior to each basketball practice. A late fee of $15.00 shall be assessed for any application submitted after the deadlines set forth above (excluding basketball practices). First Baptist Church of Kennett, Missouri Page 1 of 4

Any non member of First Baptist Church applying for use of the Family Life Center shall be required to have a member of First Baptist Church sponsor their application. Members sponsoring non members shall agree to be financially responsible to First Baptist Church for any damage caused to the Family Life Center. PRIORITY OF USE All activities of First Baptist Church shall have priority for use of the Family Life Center. Organizers of church activities should take care to notify the church secretary of their intended use of the facility to guarantee priority on the calendar. All other applications for use shall be determined on a first come, first served basis, subject to approval by the deacon body. First Baptist Church schedules certain open use hours for our members to enjoy the Family Life Center. These hours may be adjusted as necessary to accommodate requests for use of the facility. Open use hours will be made available to the church as they are scheduled. FEES FOR USE The following fees shall apply for use of the Family Life Center: Athletic events: $20.00 per hour Use of gym only (no kitchen, food, drink, tables, chairs or A/V equipment) Daily rates: Small Events in Gym Events with no more than 50 attendees Includes use of tables and chairs for 50 Use of stage not included $50 per day Large Events in Gym Kitchen Classroom Events with more than 50 attendees Includes use of all tables and chairs Use of stage included $200 per day $100 per day $50 per day Late fee: $15.00 Applies when application submitted after required deadlines In the event use of the stage is requested, the Large Event rate of $200 per day will apply regardless of the number of attendees. Fee waivers may only be granted by the First Baptist Church Board of Deacons and requests for same must be submitted in writing at the time the application for use is submitted. Requests for fee waivers will not be regularly granted. AUDIO AND VIDEO EQUIPMENT Audio and video equipment is not available for use or rental. Any group or individual renting the Family Life Center is welcome to provide their own audio and video equipment. A number of electrical outlets are available on each wall of the Family Life Center and will be made available for use. KITCHEN Use of major kitchen appliances (stoves, warmers, dishwashers, ovens, etc.) will be limited to trained individuals. Any individual wishing to use major kitchen appliances must indicate as such on the building use application and coordinate training with the Church Hostess or other authorized individual. Groups and individuals using the kitchen are responsible for cleaning all utensils and cookware used and must return them to their proper place of storage. All countertops and other surfaces used must be cleaned after use and returned to their original condition. Trash should be bagged and left in the kitchen following the conclusion of the event. TABLES, CHAIRS AND STAGE The Family Life Center has round tables and chairs available for use. The round tables seat from 6 8 individuals comfortably at each table. Tables and chairs are generally available to seat approximately 300 individuals. A stage area is available at the Family Life Center and can be provided for use if requested. Please note that unless you specifically request use of tables, chairs or the stage, none will be provided and they will not be available on the day of your event. Advance notice must be given if you plan to use tables, chairs or the stage. First Baptist Church is pleased to provide this equipment for your use, but cannot guarantee the exact number of tables and chairs nor the size of the stage. It is the responsibility of the individual renting the Family Life Center to ascertain the number of tables and chairs available and the size of the stage. The tables, chairs and stage cannot be loaned or rented for use outside the Family Life Center. KEYS AND BUILDING ACCESS Keys to the facility will be issued to responsible individuals reserving the facility. Keys will be distributed and signed for at the church office and must be returned immediately following the event. Portions of the facility not requested and reserved will remain locked. The individual receiving a key to the building is responsible for locking all doors and leaving the facility in a secure state. First Baptist Church of Kennett, Missouri Page 3 of 4

Individuals receiving keys are only authorized to access the facility at the times reserved. Unauthorized access of the facility will result in a cancellation of your reservation and a suspension of use privileges. GENERAL REGULATIONS All individuals under the age of 18 must be accompanied by a responsible adult. Appropriate athletic shoes, shirts and other attire must be worn at all times during athletic activities. Appropriate God honoring speech, behavior and attire should be used and worn at all times. Equipment and materials must be used for their specified purpose. No hanging on basketball rims or nets. Intentionally hitting or kicking balls into ceilings and walls shall be prohibited. Food and drink shall only be allowed during specified events. During athletic events, food and drink is prohibited in the gym area. Decorations must be freestanding. Nothing shall be attached to the walls of the Family Life Center at any time. The use of tobacco, alcohol, controlled substances or firearms shall be expressly prohibited. Following use, all lights must be turned off and all doors locked. Any videos or movies shown or music played shall be appropriate for a house of worship and shall not contain vulgar or explicit lyrics or images. POLICY ON VIOLATIONS AND DAMAGE TO FACILITY Individuals or groups failing to abide by the policies and procedures governing the Family Life Center will be asked to leave the building and may have further use privileges suspended. Individuals or groups causing any damage to the Family Life Center shall reimburse First Baptist Church for the damage incurred.
