Who is Hope House & What is Program Living? Hope House exists to alleviate physical and spiritual poverty through gospel restoration. Our Program Living Facility is an extension of that mission and is a Christ-centered, faith-based, gospel-driven reconciliation program for men. Program Living is not a come and go as you please Halfway House. We are in the business of going beyond treating the symptoms of men s life struggles. We believe the problems of all men such as addiction to drugs, alcohol, pornogaphy, gambling, sex, work, etc. are symptoms of a greater issue. We believe the greater issue is, in fact, that we all have a heart problem. Therefore, Hope House Ministries Program Living is a heart treatment facility. What is Program Living s format? We believe the best approach to long-term recovery and restoration is through a phased approach. This allows men to see and feel themselves achieving goals as they progress in overcoming their specific struggles. Our approach provides a safe and structured environment built around core principles of helping men grow Spiritually, Emotionally, Mentally, and Physically. Descriptions and Expectations of Phases Phase I. Residents in this Phase will do a minimum of 2 counseling services per week with Hope Center for Biblical Counseling to identify any at risk behaviors and develop a long term care plan. This will be in addition to a regular weekly meeting with Justin Crowe, Director of Programming and Counseling. If needed or directed by the Judicial System, counsel coordination can be made with Wilson Counseling Inc. to address clinical needs a resident may have. Phase II. Counseling will continue as needed, after reviewed by Director of Programming & Counseling and Hope Center Biblical Counseling. Residents will be rewarded passes based off previously established schedule of: 80(8hr), 100(overnight), 120(weekends, continuing every 3 weeks thereafter if earned ). Phase III. Residents will begin a probationary work period of 90 days with an approved and possibly appointed employer partner of Hope House Ministries. Residents will be required from this point on to pay fees of $500.00 per month (or more if debts are owed) to Hope House Ministries out of the income generated through employment. HHMPL will both establish and maintain checking accounts for all Residents throughout the remainder of their time in the program. Residents will be required to complete the Faith and Finances Course and a selection of other Courses in the evenings. Residents will be required to attend a selection of classes and programs either at HHM or at the PL House based off work schedule. Phase IV. 2
Residents will move into our Transitional Living Facility/House (once offered full-time employment) and will stay there throughout the remainder of their time with HHMPL. Residents will be required to attend a selection of classes and programs either at HHM or at the PL House based off work schedule. Residents will stay in our Transitional Living Facility/House at a determined rental rate. Residents that meet all requirements and are fully up to date on fees are allowed to spend weekends at home. Those home environments must be both drug free. (inasmuch as we can assure) and legally approved by Probation and Parole and/or the Judicial System. Residents will begin regular meetings and mentoring of Phase I Residents. Must be volunteering at a minimum 10 hours of community service per month with Hope House or an approved organization. Family and/or Marriage Counseling through Hope Center Biblical Counseling or Wilson Counseling Inc. will need to take place during this phase. Residents will continue to gather at Christ Fellowship Church every Sunday morning for worship at 10:30 am. Phase V. This is an optional Continuum of Care for those residents that have voiced, or we have identified as having, a need for longer safe housing while reestablishing themselves here in the Bowling Green area. Transitional Facility will also be open to former residents, of like-minded programs, in need of this form of safe transition back into culture. Residents will stay in our Transitional Living Facility/House at a determined rental rate for a period of 6 to 12 months based off need and availability A lease/rental agreement must be signed by all occupants of Transitional Living Facility! A 30 day notice will be required if a transitional resident chooses to move out before their lease agreement expires. Residents may be required to attend a selection of classes and programs either at HHM or at the PL House based off work schedule. Must be volunteering at a minimum 5 hours of community service per month with Hope House or an approved organization. Residents must attend a weekly church service of their choosing. 3

What if things are going well for a resident, can they leave when they want? Simply put, yes. No one will be physically held here or forced to stay against their will. However, at the core of our program is a covenant between Hope House Ministries and our Program Living Residents. We covenant with our residents to provide a safe, Christ-centered, environment to help them overcome the issues of life that have prevented them from being all that God would have them be and we ask for honest commitment from them. We ask that residents come into this relationship covenanting to us with all of their heart, mind, and soul to allow us the privilege over these next 12 months to help in overcoming these battles and trials. Understand, in our program, they will never be alone in the fight for their sobriety or mental, physical, and spiritual peace. We are dedicating ourselves throughout the duration of our time together to our residents success, for their good, and to the glory of God. We are walking together as once broken people, and now restored to God through the blood of our Savior. Ultimately, we hope to extend restoration! However, in order to protect the overall integrity of the ministry we will have a walk-off and AWOL policy in place. What do you believe at Hope House Ministries Program Living? Statement of Faith: The sole basis of our beliefs is the Bible, God's infallible written Word, the 66 books of the Old and New Testaments. We believe that it was uniquely, verbally, and fully inspired by the Holy Spirit and that it was written without error (inerrant) in the original manuscripts. It is the supreme and final authority in all matters on which it speaks. We accept those areas of doctrinal teaching on which, historically, there has been general agreement among all true Christians. Because of the specialized calling of our movement, we desire to allow for freedom of conviction on other doctrinal matters, provided that any interpretation is based upon the Bible alone, and that no such interpretation shall become an issue which hinders the ministry to which God has called us. 1. There is one true God, eternally existing in three persons Father, Son, and Holy Spirit each of whom possesses equally all the attributes of Deity and the characteristics of personality. 2. Jesus Christ is God, the living Word, who became flesh through His miraculous conception by the Holy Spirit and His virgin birth. Hence, He is perfect Deity and true humanity united in one person forever. 3. He lived a sinless life and voluntarily atoned for the sins of men by dying on the cross as their substitute, thus satisfying divine justice and accomplishing salvation for all who trust in Him alone. 4. He rose from the dead in the same body, though glorified, in which He lived and died. 5. He ascended bodily into heaven and sat down at the right hand of God the Father where He, the only mediator between God and man, continually makes intercession for His own. 6. Man was originally created in the image of God. He sinned by disobeying God; thus, he was alienated from his Creator. That historic fall brought all mankind under divine condemnation. 7. Man's nature is corrupted, and he is thus totally unable to please God. Every man is in need of regeneration and renewal by the Holy Spirit. 6
8. The salvation of man is wholly a work of God's free grace and is not the work, in whole or in part, of human works or goodness or religious ceremony. God imputes His righteousness to those who put their faith in Christ alone for their salvation, and thereby justified them in His sight. 9. It is the privilege of all who are born again of the Spirit to be assured of their salvation from the very moment in which they trust Christ as their Savior. This assurance is not based upon any kind of human merit, but is produced by the witness of the Holy Spirit, who confirms in the believer the testimony of God in His written word. 10. The Holy Spirit has come into the world to reveal and glorify Christ and to apply the saving work of Christ to men. He convicts and draws sinners to Christ, imparts new life to them, continually indwells them from the moment of spiritual birth and seals them until the day of redemption. His fullness, power, and control are appropriated in the believer's life by faith. 11. Every believer is called to live in the power of the indwelling Spirit that he will not fulfill the lust of the flesh but will bear fruit to the glory of God. 12. Jesus Christ is the Head of the Church, His Body, which is composed of all men, living and dead, who have been joined to Him through saving faith. 13. God admonishes His people to assemble together regularly for worship, for participation in ordinances, for edification through the Scriptures, and for mutual encouragement. 14. At physical death the believer enters immediately into eternal, conscious fellowship with the Lord and awaits the resurrection of his body to everlasting glory and blessing. 15. At physical death the unbeliever enters immediately into eternal, conscious separation from the Lord and awaits the resurrection of his body to everlasting judgment and condemnation. 16. Jesus Christ will come again to the earth personally, visibly, and bodily to consummate history and the eternal plan of God. 17. The Lord Jesus Christ commanded all believers to proclaim the Gospel throughout the world and to disciple men of every nation. The fulfillment of that Great Commission requires that all worldly and personal ambitions be subordinated to a total commitment to "Him who loved us and gave Himself for us." Does someone have to share same beliefs as we do? No, we will not prevent anyone from joining Hope House Ministries Program Living based on a difference in or even lack of belief. Christ teaches just the opposite should be true as He Himself said that He came not to be a physician for the well but rather the sick. Therefore we see our need not to be for people who agree with us and are doing well in life, but instead to those who do not know our Savior and have a need that is beyond their own abilities to conquer. We will be respectful of them in all areas of faith, we require the same in return. However, there are two stipulations to this: 1. One must agree that even if you do not agree with us you will not act irreverently or disrespectfully toward our beliefs, the beliefs of any other resident or anyone teaching, praying, or sharing testimonies related to our faith 2. Even if you do not agree with us you will follow all direction, guidelines, assignments, and work as it pertains to both our programming and the faith that leads us as a ministry. 7
How much will Hope House Ministries Program Living cost a Resident? We have determined the basic cost of our programing is $48.00 per day per resident. Therefore, our program will have some cost associated to the programming and general expense of operating. These resident fees are both initial and ongoing while in Hope House Ministries Program Living. We want to affirm the good practice of knowing when worth is applied to something then it s value is legitimized and respected. Essentially when you are invested in something you appreciate it more. Our enrollment fees are $250.00 due at intake and the associated monthly fees of $500.00. This is to help in covering a portion of our expenses for your care, treatment, and support. If a resident receives any additional income (such as SSI, EBT, Food Stamps, Workers Compensation, or Disability) Hope House Ministries Program Living will absorb that revenue to help cover the cost of the residents care while in our program. The only exception to this policy is if a resident would choose to be 100% self pay. In this scenario the resident would pay the initial entry fee of $250.00 and the 1st month's and last month's resident fees up front in the amount of $1440.00 per month(based on the daily rate of $48/day @ 30 days/month) with the remaining 10 installments due on the 1st Friday of months 2-11. Any addition cost charged to Program Living for drug screening and health assessments would also be the responsibility of the residents in the self-pay status and due along with the payment installments on the 1st Friday of each month. An invoiced bill will accompany these charges and total amounts owed for full disclosure for these residents. It is our goal that no man truly in need of our services and seeking the opportunity to change would ever be turned away because and inability to pay. Therefore we will work with your family, friends, loved ones, employers, or whomever else to make possible the chance for anyone to attend our program that is desiring this type of restoration for their lives. Before completion of Program Living a resident will meet with the staff and/or the Board of Hope House to determine a fair payment schedule for any owed amounts to Program Living. In keeping with the design of our program this timeframe will be determined on a case by case basis to ensure massive debt does not lead to a person's failure. Arrangements, requests, and any other questions concerning this issue will be addressed directly by the Program Living Coordinator. How does someone apply for Hope House Ministries Program Living? 1. Complete the Program Living Application 2. Complete a one-on-one interview with either the Program Living Coordinator or Director of Programming and Counseling for Program Living 3. Have the ability to obtain health insurance and food stamps 4. Be the legal guardian of your own welfare, with a GAF score greater than 40 5.
