Page 4 of 6 Instructions for Completing Pages 1-3 SECRETARY S ANNUAL STATISTICAL REPORT FORM General Important. Accuracy is essential. Other reports and comparisons, as well as analyses and research, will be based on these figures. Neatness and clarity are essential. Submit an original typed copy or a computer-generated reproduction of this form. (See deadline data at end of instructions.) Presentation of Figures. See that all figures are properly punctuated. Separate the millions, thousands, and hundreds with commas. Do not use decimals. If you are using an electronic form, do not have the computer do the rounding, enter only whole numbers. Validity of Data. Be sure that the totals for each of the columns 4a through 5e agree with the sum of the matching columns of the four quarterly reports. Also, be sure that each total of columns 2a, 2b and 6 agrees with the totals given on the fourth quarter s report form. Scope. The purpose of this and the quarterly reports is to record a brief but comprehensive picture of the church=s status in each field. Membership data is to be an accurate summary of local church membership rolls. Sabbath School data should give as accurate a picture as possible. Finally, denominational employee data must account for every full time regular employee. INSTRUCTIONS BY COLUMN (The number of the instruction corresponds with the number of the column on the report form.) Page 1 1. Organization. Use the latest SDA Yearbook as a guide for correct and full English names and their correct alphabetical order. Use a separate form for each union conference/mission. List the union=s name first, followed by all fields (except attached fields) in alphabetical order, whether they are conferences, missions, or fields. Report attached fields last. Finally, prepare a recapitulation listing the name of the division on the first line, followed by the unions in alphabetical order, and attached fields after the last union. Place the division totals on the bottom line. (This arrangement is identical to that shown in the Annual Statistical Report.) 2a. Churches. All regularly organized churches, including the conference/mission church, are to be reported. This figure must be identical with that reported for the fourth quarter. 2b. Companies. All companies recognized by the conference/mission are to be reported. This figure must be identical with that reported for the fourth quarter. 3. Church Membership at Beginning of Year. This figure must be identical with the closing membership for the previous year. 4a-5e. Members Added... Members Subtracted. The additions and subtractions reported here, when combined with the figures in column 3 must equal that reported in column 6. They represent all four quarters of the year being reported. 6. Church Membership at End of Year. This figure must be identical with that reported for the fourth quarter. When adding the sum of column 3 plus columns 4a, 4b, 4c, and 4d minus columns 5a, 5b, 5c, 5d, and 5e, you should get the same total as the fourth quarter report. Page 2 15. Sabbath Schools. Count each church and/or company that has a Sabbath School program as one. Do not count Branch Sabbath Schools as they are part of an organized or mother church. Do not count the individual Sabbath School classes at a local church as separate Sabbath Schools. 16. Sabbath School Membership. The Sabbath School Membership count should include all those attending the various Sabbath School classes/divisions (including Cradle Roll, Kindergarten, Primary, etc.) at each Sabbath School location. s and s These should agree with your s and s Committee. Remember: no person shall be included in more than one category at the same time. Missionary s and Missionary s are to be issued to nonministerial employees. See GC Policy E 05-15. 22a-24. ed. Ministerial credential, commissioned minister credential, commissioned ministry of teaching credential, administrative ministries credential (NAD only), and missionary credential to whom the local conference or mission has granted credentials, whether in conference or institutional work, are to be reported in their respective columns. Bible instructor credentials are to be reported as missionary credentials, unless they have been commissioned. GC Policy E 05-10. 25a-27. d. to whom a conference or mission has granted licenses should be reported in their respective columns, whether employed by conferences or institutions. Bible instructor licenses are to be reported as missionary licenses unless they have been commissioned. 28. Literature Evangelist and. Only regular book and periodical literature evangelists who hold literature evangelist credentials or licenses are to be reported in this column. 29. All Other Regular. Include here all conference and mission employees to whom credentials or licenses have not been issued, such as office assistants, helpers, academy or church school teachers, etc., who are working full time and receiving denominational pay. Do not include lay preachers or others not on denominational payrolls. 30. Total Active. This figure is the total of columns 22a-29. The vertical total for this column is to be checked against the horizontal grand total of all totals in columns 22a-29. These two figures are to be the same. 31. Honorary/Emeritus ed. Retirement beneficiaries and others receiving honorary/emeritus credentials will be reported by the unions granting such papers. On the union line, report all such employees in one figure for each union. Page 3 Table: Denominationally Employed Workers Classified by Type of Employment. Please refer to the GC Working Policy E 05 for guidelines: This table summarizes all employees in the division. Each employee is to be accounted for by row and by column. The rows provide space for all credentials and licenses. (Bible instructors= credentials and licenses have been discontinued; use the missionary credential or license row as needed unless they have been commissioned.) The columns provide space for denominationally employed workers by type of employment. Example: A licensed missionary who works in an educational institution but is not a teacher would be tabulated in the box where the AMissionary @ row and the AOthers in Educational Institutions@ column intersect. List honorary/emeritus (retired) credentialed employees in the last column only, grouped according to type of credentials held. Do not list any who are licensed. (See GC Working Policy, E 05 40.) The row totals for each credential and license category must equal the column totals of columns 22a to 29 on page 2. The total of the column ATotal Active @ must equal the total of column 30 on page 2. The total of the column AHonorary/Emeritus ed @ must equal the total of column 31 on page 2. Submission: Divisions are to submit a single summary sheet of the entire division plus a sheet for each of the unions (and attached fields) of page 3. Column 1 is filled in with the names of the countries in your division or attached field listed by union for ease in reporting. Columns 2, 3, 4, and 5 contain shading to block out those countries for which information is already available from pages 1 and 2 of Annual Report. This column will be filled in when you receive the form. 2. Churches. Enter the number of churches for each country in the spaces that are unshaded. 3. Companies. Enter the number of companies for each country in the spaces that are unshaded. 4. Church Membership. Enter the church membership for each country in the spaces that are unshaded. 5. Ordained Ministers. Enter the number of ordained ministers working in each country in the spaces that are unshaded. Do not include those who appear on the union and division lines on page 2 of (those who serve in administrative positions at the NOTE: Information for columns 2, 3, 4, and 5 is needed only for countries that lie within conference or mission boundaries.
