This REGISTRATION packet includes: Welcome/Information Letter Sacrament & Faith Formation Registration Form Emergency Medical- Photo/Video Use Authorization & Student Code of Conduct Volunteer/Ministry Sign-Up Form Full payment should accompany your Registration forms. If for any reason you are unable to make full payment at the time of registration, please contact Katie McCarty. The tuition rate is $125.00 per student. There is also an additional $30.00 book fee for those participating in first Reconciliation and first Eucharist to defray the cost of published materials ($15.00 per sacrament). For all registrations fully completed (Registration Form, Emergency Medical/Photo-Video Form/Code of Conduct, Volunteer/ Ministry Sign-Up Form with full payment made) by Friday, June 1, 2018, the tuition rate of $100.00 per student will be offered as Early Registration tuition. The Early Registration deadline June 1-assists our staff as they focus on curriculum development, ordering of texts and materials, volunteer recruitment and training of needed catechists and volunteers. REGISTER EARLY--$100.00 PER STUDENT WHEN ALL FORMS ARE COMPLETE AND RECEIVED NO LATER THAN JUNE 1. Families may register for ON-SITE PROGRAMMING or FAITH (Faith Alive In The Home-the home study option). Our programs serve ages 3 through Grade 11. Early registration ensures YOU the day/time preferences you may have. All students will be assigned to classes on a first come, first serve basis. Space is sometimes limited and classes may fill before the June 1 deadline. [Important note: for any student in a sacrament year, parish policy requires that the student be registered in ON-SITE programming.] What if I don t know my child s extracurricular schedule for the school year? Our response: PLEASE REGISTER ANYWAY! We are aware that not all families know their child s fall/winter/spring extracurricular schedule due to scheduling, tryouts, etc. But to ensure your child s best possible placement, register now anyway. If there is a conflict in the school year with the day of the week you choose for Faith Formation, we will do what we can to accommodate your change requests as they come in. If on-site placement is not a possibility, your child will be enrolled in the FAITH option. Re-cap of Registration: 1. All families are asked to register their children by June 1 for the 2018-2019 Faith Formation year. Please register your child(ren) for the day/time/session that you foresee will fit your family s schedule best; 2. Should a conflict develop prior to the start of the FF year, please notify the grade level Faith Formation Coordinator as soon as possible; 3. The Faith Formation team will do their best to accommodate requests for changes as they are communicated to us. IF there are no openings in on-site classes, your child will be enrolled in the FAITH (Faith Alive in the Home) program for the 2018-2019 Faith Formation year. Description of our ON-SITE PROGRAMS: SUNDAY SCHOOL (ages 3, 4, 5/Kindergarten) Sunday mornings during the 8:30 a.m. & 10:30 a.m. Masses. The Sunday School program uses Our Sunday Visitor s ALLELU! Program, a curriculum developed with the young child in mind. Each Sunday morning session will include a themed lesson, prayer, activities and Bible stories. Students are grouped by age for classes. The children participate in a Nativity Program each December. We invite you to consider our Sunday School program on Sunday mornings beginning late September 2018 through April 2019! GRADES 1-6 -Tuesday or Wednesday afternoon/evening sessions **Note changed times In hopes of better serving our families, we have slightly altered our grades 1-6 time/day offerings for 2018-2019. Tuesdays will now have ONE session 5:15-6:30 p.m. Wednesdays will have two sessions 5:15-6:30 p.m. and NEW this year 6:45-8:00 p.m. Wednesdays 6:45-8:00 p.m. session will be in conjunction with our Jr. High (Gr. 7 & 8) program. This change was made in hopes to serve families that have students in BOTH elementary and junior high classes, AS WELL as our

families that need a later time slot to accommodate work/activity schedules. PLEASE NOTE due to space availability in the building, there will only be ONE class offering for each grade (grades 1-6) for the Wednesday 6:45-8:00 p.m. session. This session will be filled on a FIRST COME/FIRST SERVE basis, so please be aware of this and register EARLY if this session is a priority for your family. Grade 1-6 program uses Loyola Press Finding God curriculum. In addition to lessons from the text, students will learn about the liturgical seasons of the Church, saints, participate in Stations of the Cross during Lent and much more. These sessions offer a rich variety of activities and experiences. Our students really do have fun and learn at the same time! Sessions start late September 2018 and continue through late April 2019. Classes meet once per week. GRADES 7 & 8 -Wednesday, 6:45-8:00 p.m. *Note new time for 2018-2019 The Junior High program uses Loyola Press Finding God curriculum; these grades meet in small groups of 10-12 students. Students participate in many interactive activities throughout the year including the 7 th grade participation in Thanksgiving outreach, the 8 th grade Hunger Meal, Social Justice Projects and Learning Centers with a variety of themes. Sessions start late September 2018 and continue thru late April 2019. GRADES 9-11: (5:45-7:30 p.m. approximately 2 Sunday evenings a month October 2018- April 2019) OneWith is SJTW's new liturgy and Scripture-centered curriculum. Using the Mass as our model, youth will participate in and learn about Catholic worship from the inside out! Discipleship Groups (small groups) will engage in focused discussion and reflection on Scripture and Catholic Tradition as they experience it in Mass. SACRAMENTS for children and teens First Reconciliation and First Eucharist for children Students in 2 nd grade or older become eligible for the sacraments of Reconciliation and Eucharist. First Reconciliation preparation and celebration opportunities are offered in the fall of 2018. Preparation for the sacrament of first Eucharist will begin in January 2019. [Parish policy note: Students who prepare to receive a sacrament are required to be a part of a Faith Formation program at least one year prior to sacrament preparation or be enrolled in an accredited Catholic school.] Confirmation Preparation for Teens Youth entering Grade 10 or 11 are eligible to begin our two-year Confirmation preparation process. Candidates seeking this sacrament will participate in a variety of retreat and volunteer ministry experiences to help them better understand the mission of the Church and to mature in faith as a disciple of Jesus Christ. Confirmation is typically celebrated in the spring of the student s 11 th (or occasionally 12 th ) grade year. BAPTISM CERTIFICATE COPIES NEEDED (unless your child was baptized here at SJTW) Baptism certificate copies are required for all new students to our program as well as all students preparing for and celebrating first Eucharist and Confirmation in spring 2019. We do not keep baptismal records from year to year. PARENT-GRANDPARENT-GUARDIAN VOLUNTEER/MINISTRY SIGN-UP The successes of the Faith Formation programs exist because of YOUR volunteering spirit in many areas. We do need your help. You may wish to be a catechist or co-catechist. You may prefer to sign up to help with special events or tasks. We do request that each family offer their HELP in some way. The Volunteer/Ministry Sign-Up Form has a volunteer listing of possibilities for you to consider and check. Thank you for your help in making SJTW Faith Formation programs thrive! We appreciate your support and the great work you do as PARENT/GRANDPARENT/GUARDIAN of these young people of St. Joseph the Worker.
