Prospective Member Information Form Rev. 3/1/10 Thank you for your interest in joining Corydon Baptist Church (CBC)! God adds people to His church (Matthew 16:18; Acts 2:47), and He desires His church to care for each other and to be united in heart and purpose (John 17:6-23; Ephesians 1:22-23). For these reasons and more, we take church membership seriously. We interview each member to learn of their relationship to Jesus Christ, and we have a new members class to help them become active, thriving members of this community. This form makes this process much easier. If you have any questions as you fill out this form, please let Pastor Tommy (812-738-2034) or Pastor Jim (812-736-9203) know they will be glad to help. Also, feel free to use extra sheets of paper. And don t worry if you don t know how to answer a question, simply leave it blank. At this point in your life, do you believe that you will go to heaven when you die? Why or why not? If your answer is, Yes. Please share the details of how you were saved. 2
In order to help you, we are interested in finding out if you are regularly reading, studying, and meditating on the Word of God and praying, both individually and as a family. What is your current personal worship practice, and how regularly do you have family worship? What can CBC do to help you grow in these areas? 3 The Bible says that every believer has been given at least one spiritual gift for the benefit of others in His church (Romans 6:6-12; 1 Corinthians 7:7, 12:4-11, 28; Ephesians 4:11-13). What gifts has God given you that would be helpful to others in His and world? How have you exercised these gifts in the past? If an unbeliever asked you to explain how a person could have eternal life when they die, what would you say?
4 Our church has adopted the Baptist Faith and Message 2000 as our statement of beliefs. After reading it, do you have any questions about our beliefs? Is there any part of that statement with which you disagree? CBC has a Church Covenant between members that outlines our responsibilities to one other. Please read the covenant (see attached). Are you ready to make this commitment to the other members of this church? Currently, our church is in need of childcare workers to serve on a rotation. The more members we have involved in each area, the less each of us has to do. Is there any reason why you can not or should not serve in this ministry? It takes substantial financial resources to operate this church (pastoral staff, buildings, ministries, missions, etc.). Are you prepared to invest in the ministries of this church with a regular and substantial portion of your income? To get every member off to a good start, we regularly hold a class for new members called Membership Matters. It lasts three weeks during the Sunday School hour. Can we enroll you in the next class? Do you have any comments that you would like to share with the pastors? Any questions? Thank you very much for your time!
Corydon Baptist Church Covenant I have one desire for my life and this Church, to know and please the Lord Jesus Christ by whom and for whom we were created and given new birth. I long, by God s grace, for this to be a church that genuinely exists for His pleasure and glory alone. 1 I realize that for us to please the Lord, we desperately need Him and each other. 2 Therefore, relying upon the grace of God, I joyfully and thoughtfully enter into a bond of encouragement 3 and accountability 4 with the other members of this body. I have repented of my sin, 5 trusted in Jesus Christ as my Savior and Lord, 6 and have professed my faith publicly through baptism. 7 I will faithfully participate with this Church in worship, prayer, Bible study, fellowship, service, giving, and the ordinances of baptism and the Lord s Supper. 8 I will watch over the other members in love as they watch over me. 9 I will remember them in prayer, 10 help them in sickness and distress, 11 promote their spiritual growth, 12 restrain them from sin, 13 and stir them up to love and good deeds. 14 I will shun gossip and divisive words, knowing that they dishonor God and destroy Christian fellowship. 15 I will diligently train myself and my family in the discipline and instruction of the Lord. 16 I will strive to promote marriages that reflect the relationship of Jesus Christ and His Church. I will seek the preservation of marriages, knowing that God hates divorce, and I will submit to biblical regulations regarding marriage. 17 I will seek the salvation of my family, friends, neighbors, co-workers, acquaintances, and people of all nations through loving, bold evangelism and a life that reflects my King. 18 I promise to remain faithful to this Church in membership and participation. I will consult with the pastors in advance if I sense that God is directing me elsewhere. If I must leave, I will not delay in uniting with another church that preaches the biblical gospel. 19 1 Psalm 63:1-8; Ephesians 3:10, 20-21; Philippians 3:7-16; Colossians 1:16 2 John 15:5-6; Ephesians 4:14-16 3 Acts 2:42; Romans 14:19; Ephesians 4:1-6, 16, 29; Philippians 2:1-4; 1 Thessalonians 5:11; 1 John 1:6-7; Hebrews 3:5-15; 10:19-25 4 Proverbs 27:17; Matthew 18:15-17; Galatians 6:1; 2 Thessalonians 3:6, 14-15; James 5:16 5 Matthew 4:17; Mark 1:15; Luke 13:1-5; Acts 2:38; 17:30; 26:20; 2 Corinthians 7:10 6 John 3:16; Acts 4:12; 16:30-31; Romans 10:9-11 7 Matthew 28:19; Acts 2:38; 8:36-38; 10:47-48 8 Matthew 28:18-20; Luke 22:14-20; Acts 2:41-47; 2 Corinthians 8:1-9; Ephesians 4:7-16; Hebrews 10:24-25 9 1 Corinthians 10:24; Galatians 6:1-2, 10; Philippians 2:1-4 10 Ephesians 6:18; James 5:14-16 11 Matthew 25:34-40; Romans 12:13, 15-16; Galatians 2:10; 6:2, 10; James 2:15-16; 1 John 3:16-18 12 Ephesians 4:11-16; Titus 2:1-15 13 Proverbs 24:11; Galatians 6:1; Hebrews 3:12-13; James 5:19-20 14 Ephesians 2:10; Titus 2:1-10; 3:8, 14; Hebrews 10:24-25 15 Proverbs 6:16-19; 26:20; Romans 16:17; Ephesians 4:29, 31; Titus 3:10-11; James 3:6-10 16 Deuteronomy 6:4-7; Psalm 78:5-7; Ephesians 6:4; 1 Timothy 4:7-8; 1 Peter 2:1-3; 2 Peter 1:5-11; 3:17-18 17 Matthew 5:31-32; 19:3-9; Mark 10:2-12; 1 Corinthians 7; 2 Corinthians 6:14-18; Ephesians 5:21-33; Hebrews 13:4 18 Matthew 5:13-16; 28:18-20; Romans 10:14-15; 1 Corinthians 9:19-22; 2 Corinthians 5:18-20; 1 Peter 3:15 19 Proverbs 3:5-6; 11:14; 12:15;18:1; 1 Corinthians 12:18; Ephesians 4:11-16; Philippians 2:3-4; Hebrews 10:24-25
