Honoring our Teammates A community produces most what it honors most. ~KT, our Mayor Purpose: Honoring teammates for living our s This document will help guide you when you want to honor a teammate for living and modeling our s, or The DaVita Way. As a leader in the Village, we should always be watching for examples of our teammates exhibiting The DaVita Way and our s in action. Honoring their actions is a wonderful way for you to create Village spirit! Who is eligible: ALL teammates are eligible to win a. How to do it: Make it special! It's very important to honor teammates who are consistent role models for one of our s with one of these pins, so ensure the ceremony feels special. Awards: Ensure nomination forms are available to all teammates in a central location. Purchase sets of s through the DaVita Store. Order Certificate(s), if desired, through the DaVita Store. From the home page, click "On Demand." Fill out a nomination form (attached in this document) and privately share it with your manager to ensure you are aligned and agree with the nomination. Based on the nomination form notes, prepare and practice what you will say. Short and specific is usually best: Prepare by writing down the key behaviors that you observed or want to honor. Include the impact that their behaviors had on you, the team, the patients, etc. An example script is provided in this document for you to follow. Make sure whatever you write is thoughtful and authentic. Choose a day/time you feel appropriate to honor the teammate. Keep the name of the recipient secret, but when ready, announce ahead of time that an award will be given to a teammate and gather the team. The recipient MUST speak! Even if they don t want to, this is a critical part of the Award process that they express their thoughts and feelings, however brief, in the moment. Optional: Get a picture (or video) of the award recipient with you and any other managers that are present, and display or provide to the recipient so they remember the event. Guidelines to help you: Each manager, director and VP should honor at least one teammate each time we have a special Village occasion, or within your center or department, 2-3 times per year. Make it special. Ensure it s a public ceremony, even if the teammate is shy! If possible, ask for your manager to be there (director, VP). If they have some words to add, even better. Be authentic, and have fun!

Presenting the Award Presentation preparation Describe, in detail, the reason that this teammate has been selected to receive this Award. Give specific examples that clearly explain how this teammate is deserving of the award, and the impact that they have had because of their behaviors. Example script: Today is a special day. We re here to honor someone on our team because they re a role model for one of our s. As you are all very much aware, we ve been quite busy lately with our heavy treatment schedule, and yet this person has stepped up to ensure we are having FUN around here! The idea of the Jeopardy game while we ve been working so that we and our patients can have some laughs and lighten the mood; getting Mr. Johnson to smile over and over again and making his day for the first time in months since his wife passed away; and taking the lead on our Halloween party are just a few examples. These things have really made a difference in my looking forward to coming to work, and several of you have come up to me and commented that it s having a huge impact on your attitude, even on our really tough days.
