Attached you will find the revised Status Animarum [literally, state of souls ] form, which is intended to provide a clear picture of your parish and its activities for the calendar year January 1 to December 31, 2017. Please note the instructions for completing and transmitting this report included below: Completing the report: Layout of Report: Following this cover page are four pages total, containing 21 items. This year s form is nearly identical to last year s. Please complete each item as carefully and accurately as possible. Please send the report to the Pastoral Center no later than Friday, March 2, 2018. Total registered families/individuals: Please include the total number of both active and inactive registered families and individuals. Faith Formation and Other Ministries: For the number of children, teens and adults in your Faith Formation programs and other ministries (#s 11-14), please include the number enrolled/attending at the end of 2017. If you are uncertain of the exact number, please use your current statistics. Average Weekend Mass Attendance: As in past years, please note that item #3 asks for an average weekend Mass attendance. Totals for the periods requested are to be divided by the number of Sundays during that period (Example: The total weekend attendance for all Masses during the period Jun.-Aug. is 15,600. There are 13 Sundays during those three months. Average weekend attendance would be 1,200. This is the number which should be listed on your report). Marriage information: Please include all convalidations in your totals for marriages. What is typically called a convalidation is actually the celebration of marriage. Please also include Convalidations along with marriages in items #9a, b, and c, depending upon the religious status of the parties.
