Transformational Church Assessment November through November 19 Mt. Olive Baptist Church members have the opportunity to participate in an assessment of our church. We will use the Transformational Church Assessment Tool. This survey has been used in thousands of churches to identify ministry strengths and focus efforts on ministries that serve to lead people and communities to transformation in Jesus. A church assessment is a positive step forward! This tool allows everyone to participate by providing insight that will be used to help your church create future ministry goals and action plans. Participation in this survey is voluntary and anonymous. You will not be identified or required to participate in any follow up exercises as a result of your survey responses. Thoughtful, honest responses will provide the best feedback. Instructions Option 1 From the Pastor 1. November - Nov 19, 21 visit the website http://tc.lifeway.com and in the area marked First Time User, enter the church s passcode stsd2abyj. The passcode is case sensitive. Users must enter it exactly as it appears. 2. Complete the registration by creating your own username (can be anything you want) and password. Registration information is used only for a system-generated email to you in the case of a lost password. You will not be personally identified and your answers will remain anonymous. 3. Begin taking the survey by following the instructions given on the screen. If you need to stop and come back, you can log back in later with your username and password to complete the survey.. Answer all questions with honesty and to the best of your knowledge. Click to submit the survey only when completed.
