MINISTRY INFORMATION FORM Special information regarding the visiting ministry of Hakeem Collins Ministries, Inc/Champions International Ministries, Inc. The following information below is designed to help you receive his ministry gift and make his time as smooth and productive as possible. Thank you so much for your cooperation. The airline of preference is as follows: Deltas US AIR, Southwest, United and Continental. We reserve the right to change the time, Departure and Arrival city at any time. In addition, please purchase tickets that are refundable and changeable without a penalty. Our office will be sending you a Travel Information and Flight/Hotel Accommodation forms regarding all details needed for the requested engagement. This will be sent as soon as the engagement is confirmed. All tickets and confirmation number should be issued electronically and emailed to championsinternational@gmail.com. Seating Preferences is the Window or Aisle seat not the middle. 2. AIRLINE RESERVATION AND REINBURSEMENT: initials here. The host church/ministry can book all flight and travel arrangements. If the hosting ministry decides that HCM office should book our own flight then please let us know in advance. Please note: If the engagement must be cancelled by your office and the ticket has already been purchased, and is a non-refundable ticket you will be liable for the exchange cost of the ticket or total cost, if it cannot be re-applied to another engagement and any other expenses, which are not fully refundable. 3. Hosting ministry/church that is requesting the ministry of Hakeem Collins is responsible to cover all airfare responsibilities. All air travel prior to purchase must be confirmed with the HCM executive administrator. Which can be reached at (267) 798-6389. Please provide a full-sized vehicle for pick-up from airport to accommodate space for light luggage, ministry products and his assistant. 4. HOTEL ACCOMMODATIONS: initials here. Hosting ministry/church that is requesting the ministry of Hakeem Collins is responsible to cover all hotel/suite accommodations. Please provide (1) Double or King/Queen bed. OR: (2) Double bed room if his assistant is traveling with him in a non-smoking room for Hakeem Collins. (1) Hotel room in the name of Hakeem Collins. Please be sure that a major credit card is placed on file at the front desk to cover all hotel expenses for Hakeem Collins. Hotel should have 24 hour room service or at least service until midnight. We prefer that the hotel has a continental breakfast available. We ask that he be allowed to go straight to the hotel upon his arrival. Once arrangements are projected and completed, please email the hotel, flight and general engagement form to our administrative offices. 5. SPECIAL NEEDS: initials here. ***Please Note: The following are suggestions, not demands. Many people have asked questions about the following, so we have found that it is helpful to share this information in order for your ministry to prepare. A. Assistant: Please assign someone to attend to any needs Hakeem Collins may have (Such as valet, contacting the pastor with questions, etc) during his time with you. OR: Inform the church if you are bringing your own assistant and if the church is expected to cover expenses or if you are covering the assistant s expenses. B. Hakeem Collins prefer a reserve a non-smoking room close to the elevator, near the ground floor with a king or queen size bed in quiet part comfortable of a good quality hotel, preferably with a restaurant. C. His preference is simply ice cold bottled spring water, apple juice in his hotel room if possible. D. Please have: Ice-cold bottled spring water, apple juice for him to drink before, during and after the service. E. Meals: Hakeem prefers to eat after service or meeting at a designated restaurant of choice and/or the choice to fellowship with the guest ministry or if the hotel has a 24 hotel service. If this cannot be met, then please provide meal allowance for guest ministry. F. Opportunities to rest if he is ministering at both the morning and evening meetings. G. Before meeting, Host ministry is responsible to have someone assigned to Hakeem who would provide above needs. After meeting Hakeem would like to return to his hotel immediately after meeting if he is not eating or fellowshipping with host church or ministry. 6. HONORARIUMS/LOVE GIFTS/SPECIAL OFFERINGS: initials here. Hakeem Collins does have a set minimum honorarium. Many people requesting engagements from Hakeem Collins ask what is his usual honorarium for a conference or ministry weekend however he would rather the hosting ministry set or have a specific budget for him to minister because we do understand that ministry cost can be expensive. Equally, should the host ministry/church should have an honorarium or have set a specific budget per
session/meeting, please note that Hakeem Collins will expect the specific budget to be in the form of a check or cash at the end of his ministry time with you. 7. PRAYER/ALTER CALL: initials here. Please discuss your preference with Hakeem Collins before the meetings. If there is music during the prayer, (state your preferences: instrumental, worship team, none, etc) 8. MEDIA MINISTRY: initials here. Hakeem Collins will either bring along with him or ship items from HCM/CIM media ministry. At the conclusion of Hakeem Collins visit, we request that a second master or a first generation copy of audio, CD, DVD or video tape be given either to Hakeem Collins or his personal assistant. Please provide our offices with any media publications you plan to use prior to publicizing the event Please make a Master quality Tape/CD/DVD of all audio and videotapes recorded during Hakeem Collins ministry. These can been given to him/her before he/she leaves or mailed to our office. 8. PRODUCT SALES: initials here. The product display requires one (1) 6-FOOT table. We ask that a volunteer be available to assist in this area if needed in sales. Information regarding the shipment of books and products will be provided after we receive the returned forms with your permission and shipping address. Please be assured, we will not distribute materials, nor solicit names for a mailing list. 9. PHOTOGRAPHS: initials here. If you are taking photographs of the meetings we would appreciate if someone could take a few pictures of the meetings with Hakeem Collins ministering and of the pastor and staff. This may be used in his publications or on our website. It is also very beneficial for our files to help us connect the faces with the names! Please indicate who is in the picture. We only need two or more copies of the best pictures. If you need to be reimbursed, please contact our office. CANCELLATION POLICY: initials here. In the case of an emergency the host ministry may cancel an engagement up to 48-72 hours in advance. Cancellation fee may incur due to conflict of scheduling with other ministries who wanted to schedule Ambassador Collins. The host ministry is responsible for any cancellation fees if applicable. Other arrangements to minister can be negotiable upon request if cancellation is due inclement weather, state of emergency, or any other circumstances that is out of the host or visiting speaker s control. 11.
