FAMILY APPLICATION INSTRUCTIONS Updated 10/18/2013 INSTRUCTIONS FOR COMPLETING FAMILY APPLICATION Please complete this application using one of the options below. Option 1 Option 2 Complete Forms on Paper 1. Print out the entire application. 2. Have your pastor fill out the Form for Pastor. A. Be sure to include your name on the Pastor Reference Form. B. Your pastor may return the form to you or send it directly to ABT. C. Follow up later to make sure the form was submitted to ABT. 3. Both husband and wife have a close friend fill out a Form for Friend. A. Be sure to include your name on the Friend Reference Form. B. Your friends may return the forms to you or send them directly to ABT. C. Follow up later to make sure the forms were submitted to ABT. 4. Husband and Wife complete separate application forms. A. Type or write answers on additional pages as needed. B. Please type or write answers in the order they appear in the application, numbering and lettering each answer corresponding to the forms. 5.
