vw Overview vw This assessment was developed to be used together with the. After learning each new skill in the, the child s mastery of the skill can be evaluated by using the accompanying assessment. The assessment is designed to identify the specific weakness the child may have in any step of the reading process. The child that is being evaluated uses the assessment sheet that has the chart with the words only. The teacher or parent that is evaluating uses the teacher s version. The teacher s version is the sheet that has the chart with the or words, the boxes in the chart, and the tallying tables. The evaluator marks down any mistakes and the child makes as he or she is reading. Above and below each and is a box. Any incorrect pronunciation of an should be noted in the box above it. ASSESSMENT Student s Name Notes September 1, 2011- Ploni was evaluated on his mastery of letters with test one. He was struggling with the Dagesh letters and the,, and. Ploni did not know the letter at all. His teacher sent home flashcards with the letters that he was struggling with along with some extra hints and tricks. After one month Ploni mastered the letters. October 2, 2011 - Ploni was reassessed on his letters. He knew every single one. Any mistake that he made was self-corrected. November 13, 2011- Ploni was tested on the Nekudos with test two. He made 20 mistakes out of 56. He called a lot of different Nekudos a Shva. We see that he responds very well when he uses tactile manipulatives during Kriah such as play dough, sand paper.. We will work with him in that way. November 20,2011 - Ploni was reassessed on his Nekudos using test two again. He has made much progress, this time only making 4 mistakes out of 56. However he still needs a lot more review with the Shva. We will continue to use tactile manipulatives to help Ploni reach mastery of the nekudos. Novmber 25, 2011- Ploni was evaluated..
