In each of these occasions parishioners were invited to give their input to the pastors or to members of the Living Stones Committee. In October there were three Town Hall Meetings, one in each parish, to present the work done by the Living Stones Committee and to ask for comment and input. The audio recordings of the town hall meetings at St. Patrick and Holy Family are attached. Also attached is the.pdf of the PowerPoint Presentation made at all three town hall meetings. The audio of the St. Anthony Town Hall meeting is not available because the person operating the recorder made a mistake. Also attached is a presentation made by one of the Living Stones Committee members (John Eisenhower) that explains well the work done by the Living Stones Committee. All input from these meetings was considered by the Committee, and some adjustments to the plan were made. Please attach any minutes for meetings in which the pastoral plan was discussed, including any consensus or decisions reached. Check when completed Do you believe that the possibility of a civil and canonical merger of these entities should be considered at this time? If yes, please indicate your reasoning below. For requirements of canonical modifications of parishes, please review Pastoral Handbook, Policy 406.10, and for the requirements of Closure of Churches and Relegation to Profane Use, please review Policy 406.11. Printed 8/20/2016 10 of 12

Yes No We believe that the three parishes are ready to merge into one parish with three worship sites. Already, for Sunday worship people tend to go to the church that has the time most convenient for them. The annual celebration of Confirmation is a joint effort of the three parishes. In the fall of 2018, we plan to merge our three faith formation programs into one program housed at Holy Family. We already have one bookkeeper who works at one location (St. Patrick s) taking care of the needs of all three parishes. The three church buildings are in close proximity of one to another. St. Patrick s and St. Anthony s are already linked with one pastor. At all of our meetings, townhalls, and written communications, we have not heard any objections to merging into one parish. The merger would greatly simplify the administrative tasks involved in operating a parish. How will honorable closure of any church building be commemorated, or how will the formation of a new parish family be celebrated liturgically? All current buildings will remain open. When the merger takes place, we would want to have a litugy commemorating the beginning of a new parish. 8. Additional comments The Living Stones Committee does not yet have a recommendation for the name of the new parish. Further consultation is needed. We will inform the diocesan Living Stones Committee when we have a final recommendation. Currently, the suggested names are: Immaculate Heart Holy Trinity The Divine Mercy Our Lady Queen of Peace Transfiguration Corpus Christi St. Monica St. Teresa of Calcutta All Saints St. Kateri The Holy Family of St. Anthony and St. Patrick Blessed Sacrament St. John Paul II Living Stones Unity of the Holy Spirit Proposed building usage Printed 8/20/2016 11 of 12

St. Anthony Rectory would be the parish and cemetery offices as well as office space for pastoral personnel St. Patrick s Pastoral Center and chapel would be used for group meetings. 9.
