If the church does not have a pastor, it should be completed by the senior member of the church leadership. (In this case, some questions will not be relevant and should be left blank.) If the church has several pastors, only the Senior Pastor completes this form. If there is no Senior Pastor, the current senior leader should complete it. Most of the questions are mandatory because they provide basic contact and demographic information. It will not be possible to process the survey data without this information.
