Dear Parent/Guardian, Your child has expressed interest in participating in Food Fast, Catholic Relief Services' hunger awareness program. This is a 22 hour event, which includes fasting, prayer, activities, and discussion to teach youth about issues in the developing world. Food Fast will be held on March 15 th 16 th at the Sacred Heart Parish Hall. During Food Fast, your child will: fast for 22 hours and miss three main meals consume only juice, water, and a few small snacks (mini-meal) during the fast collect sponsorship pledges prior to retreat to raise money for Catholic Relief Services (CRS) In the Bible, the practice of fasting is connected with the ideas of personal and communal growth and conversion. By fasting, praying, and sharing conversation together we gain a connection with the people in the world who suffer from hunger involuntarily. If there are medical reasons that prevent your child from being physically capable to participate in a 22 hour fast, your child can still participate in the event in a modified way. Please contact me if you would like to discuss other options. The purpose of the fast is to create an awareness of the impact of poverty and inequity in a world of plenty and to give your child ideas about how to make positive changes in the world. The time before the retreat is just as important as the 22 hrs spent on retreat! Please continue to encourage your child to raise donations for CRS from now until March 15 th and help them in any way you can. Each participant has a goal of raising $50 and there will be awards for those who raise the most money! The first $15 dollars raised goes towards their retreat expenses (retreat tee shirt, juice, & other materials) so the minimum amount they need to raise is $15. After registration, I will contact the participant with an email template for a quick & easy way to invite others to sponsor your fast. Please complete the form below and all other release forms in the registration packet and then return the packet to the Sacred Heart Pastoral Center (1504 10 th Street) by the deadline, FRI, February 22. Erazo Sacred Heart Youth Coordinator Food Fast is a 22-hour educational event focusing on issues in the developing world. Students participate in a series of planned activities, and with the supervision of their youth director or teacher, fast for 22 hours. Anyone fasting must drink water and replace solid food with juices (fruit or vegetable) or other non-caffeinated drinks (such as herbal teas). Anyone with a cold or flu, liver or kidney problems, or a serious illness such as diabetes, heart disease, or ulcers, should not take part in the fast. If you are in doubt of your child's ability to participate, please consult your doctor. For most people, fasting is safe and can be beneficial; however, there are some people who should NEVER fast without professional supervision. (For example, persons who are too thin or emaciated; persons who have experienced anorexia, bulimia, or other eating or behavioral disorders; persons who suffer weakness or anemia; persons who take insulin for diabetes, or suffer from hypoglycemia or any other blood sugar problem.) Diocese of Fort Worth Code of Conduct, expectations of all participants and dress code The purpose of this Code of Conduct is to identify personal behavior that is consistent with the purpose and objectives of the Diocese of Fort Worth. The items within the Code are based upon performance and are designed to protect the welfare of all participants. CODE OF CONDUCT OF ALL YOUTH PARTICIPANTS Youth will respect themselves, other people, other people s property and church property. Youth will participate and contribute in a positive way in all activities and obey the instructions of the youth ministry, adult volunteers, and staff. During the event youth will show respect by not causing disruptions. Harassment, intimidation or bullying will not be tolerated under any circumstances. Youth will use language that reflects a Christian attitude. Youth will not use curse words or other foul language. Youth will maintain a Christian witness to the dignity of each person: no tickling, wrestling, piggyback rides or inappropriate touching (which means: do not touch anyone anywhere that a bathing suit would cover). In addition, physical displays of affection such as kissing, full body hugs, sitting on laps, and lying or sleeping next to each other are inappropriate. Being isolated or alone with another student is not allowed. Youth will wear modest clothing. No clothing that is sexually suggestive or spiritually degrading. T-shirts or other clothing with evil, vulgar, illegal, or inappropriate content will not be acceptable. In addition there should be: no midriffs, no low-cut tops, no spaghetti straps, no see through clothing, and no sagging pants to where underwear shows. Please do not wear short shorts. Shorts must follow the same guidelines as the school system. Youth will stay with the group at all times during the event and will not leave early unless approval from a parent or guardian ahead of time or in person. Youth will not possess any weapon of any kind (knife, gun, etc.) at any youth ministry activity at the church or event. Youth will not use, possess or be under the influence of alcohol, tobacco, marijuana or any other illegal drugs at any youth ministry activity. Youth will not engage in sexual intimacy or sexual harassment, and will not possess pornography at any youth ministry activity. Any illegal activity will prompt reporting to proper authorities depending upon the situation. In the event the Code of Conduct is violated by a youth, proper diocesan protocols will be handled by the diocesan employee, pastor, youth minister or adult volunteer in charge. Failure to comply with the Code of Conduct will result in disciplinary action, up to and including removal from the event. Sacred Heart is required to have a completed Annual Parental Liability Waiver, Permission, & Medical Information (known as the FORM A) for each teen who participates in Youth Ministry events at Sacred Heart. This form is valid through the end of May 2019. If you completed the ONLINE FORM A at the beginning of this school year, you do not have to complete a FORM A again; to verify we have your child s Form A on file, you can email cerazo@sacredheartwf.org: * This electronic form is to be filled out by parents. Please use the following instructions to fill out the FORM A (separate form for each child) from the convenience of your home. Any questions/concerns, please email cerazo@sacredheartwf.org #1: Go to http://www.fwdioc.org/diocesan-forms online. (Or go to www.fwdioc.org, then click Ministries from the menu and select Youth Ministry from the drop down menu. Next click Youth Ministers button and then click Diocesan Forms button.) #2: Select Form A in English. Form A s need to be completed by the Food Fast registration deadline (Fri, February 22).

Keep this page & the Sponsorship page (last page of packet) and use them to continue collecting donations for Catholic Relief Services. Turn in the Sponsorship Pages & donations on FRI, March 15 @ retreat check-in desk. Date: Fri -Sat, March 15 & 16 (8pm-6pm) Purpose: to answer our Lenten call to prayer (during retreat), fasting (we will only drink juice, water, & a snack during the 22 hrs in order to walk in solidarity with our brothers & sisters in Christ we go hungry), and alms-giving/service (each participant collects sponsorship donations in the month before the retreat for emergency relief projects in Guatemala). Cost: Food Fast retreat is FREE but a part of the experience is raising money to help feed the global hungry. Each participant must raise at least $15 prior to the retreat (which will go towards their retreat t-shirt). Every dollar you raise over $15 will go to Catholic Relief Services to help relieve world hunger.

You will fast and learn more about what can be done to make a difference in the lives of our brothers and sisters around the world. Use the information below to let your sponsors know why you are soliciting pledges. List all sponsors, pledges, and payments on the reverse side of this sheet. How do the pledges benefit the sponsors? Let your sponsors know that through their donations, they will be making a positive and direct impact in someone s life. Contributions will support Catholic Relief Services emergency relief efforts in Guatemala (where 1.7 million people were effected by the volcanic eruption of Mount Fuego last summer). What is Food Fast? Food Fast is an annual Lenten 22-hour awareness program for Catholic youth focusing on global poverty and hunger which puts our Catholic faith into action to assist the poor. Grounded in Catholic Social Teaching, Food Fast teaches youth about issues faced by their peers in the developing world and encourages them to take action on behalf of the poor & hungry. Who is Catholic Relief Services? Catholic Relief Services is the official overseas relief and development agency of the United States Catholic Community. The fundamental motivating force of Catholic Relief Services is the Gospel of Jesus Christ. That motivation compels the agency to alleviate human suffering, to foster justice and charity, and to enhance the development of peoples around the world. Catholic Relief Services is one of the most efficient humanitarian agencies in the world, with over 92% of all resources going directly to programs. Set a goal for the amount of money you will raise! Set a personal goal for how much money in donations you will raise and explain to your sponsors why it is important to you. The goal of each participant is $50, but you can set a higher goal for yourself. The participant who collects the largest total amount of donations will be announced and receive an award during the retreat. The first $15 in pledges will go towards the participant s Food Fast T-shirt and materials for retreat. All other donations will go to Catholic Relief Services and give much needed aid to Guatemala. In partnership, we can reach out to our brothers and sisters who need our help.
