CHECKLIST Fr a BYU Husing Cntract The living envirnment f the students is an integral cmpnent f the unique experience Brigham Yung University seeks t prvide. Therefre, the University in its sle discretin reserves the right t determine which ff-campus husing prperties will be cntracted fr its students. All wners wh are seeking a BYU cntract r cntracted wners seeking t btain a cntract fr additinal facilities must first visit with a husing fficial and prvide the fllwing required dcuments t the BYU Off-Campus Husing Office (C-141 ASB, Prv UT 84602). This visit is t cllect infrmatin and initiate cnsideratin f the prpsed dwelling(s) which may lead t cntracted husing, but des nt mean a cntract will be granted nw r in the future. Additins t the BYU Cntracted Off-Campus Husing prgram must nt impse an unreasnable administrative burden n the university. This Cntracted Husing Checklist utlines the requirements fr cnsideratin f btaining a BYU cntract. First, determine that yur prperty is within the BYU cntracted bundaries. See www.byu.edu/ffcampushusing/newbundaries.html. Secnd, be aware, a university cntract is nt assignable r transferable. Owners acquiring a prperty that has a BYU cntract must apply fr a cntract and prceed thrugh the cntracting prcess. N wner r purchaser can be assured f a cntract regardless f prir cntracts until the applicatin prcess has been cmpleted and the cntract granted. Third, Yu are strngly advised t btain a Zning Verificatin Frm frm the seller r frm Prv City befre purchasing a rental prperty. First Meeting Requirements Owner/Develper Interview: Every wner/develper is required t meet with an ff-campus husing fficial t review this prcess. (This can be accmplished in persn r ver the phne) Disclaimer: Discuss and sign the BYU Cntracted Husing Disclaimer. A cpy will be prvided fr bth OCH and the wner. Yu acknwledge that this meeting is n indicatin that a cntract will be granted nw r in the future, that the Agreement fr Cntracted Off-Campus Husing is an annual agreement and that BYU may r may nt enter int subsequent cntracts based upn the university s sle discretin. Yu acknwledge receipt f a Cntract Packet and Checklist and verify the prcess fr btaining a BYU cntract has been explained and an pprtunity t ask questins has been prvided. Cnsent and Disclsure Frm: Discuss and sign the Cnsent and Disclsure Frm. BYU Off-Campus Husing Handbk: The handbk is fund at www.byu.edu/ffcampushusing n the hme page. Yu acknwledge the handbk gverns the Off-Campus Husing Prgram. Applicatin Deadlines: The University will nt review applicatins fr new prperties and wners during the mnths f August and September due t the administrative burden at the beginning f fall semester. Owners acquiring a prperty that has a BYU cntract, are given a 30 day perid after applicatin has begun, t cmplete the prcess fr Cmmittee Review. Next Required Dcuments Owner Applicatin Infrmatin Frm: The Applicatin Infrmatin Frm including the Agency Authrizatin Frm requests infrmatin abut the facility, the wners and their agents. In additin yu are required t submit a descriptin f the rganizatinal structure f the prpsed prperty and the rle that each persn will have with the students. Zning Verificatin Frm: A Zning Verificatin Frm is needed t verify tenant ccupancy and n-site parking limits. Written Business Plan: Submit yur business plan in writing addressing the items listed belw: Descriptin f Need: Describe the prperty lcatin, address, and why it meets the university s needs and is a desirable prperty fr university husing. Integratin with the BYU Off-Campus Husing Objectives: Submit a descriptin f hw this facility helps the university accmplish its missin and a cmprehensive business plan f hw yu will achieve this bjective. Include a list f amenities and services (including green space ) prvided n-site fr the students that will be cnducive t their mral and spiritual grwth; as well as their academic and scial needs as determined by the university. Separatin Plan: Fr multi-unit facilities, wners desiring t huse bth men and wmen must submit a separatin plan in writing (including site plans, dimensins and flr plans f the units). The plan must be reviewed and accepted in writing in the sle discretin f the university befre a cntract can be granted. Multi-unit cmplexes must separate men and wmen by building r by wings f buildings with cnsideratin f line f sight. When

separatin f genders is prpsed t be accmplished in wings f a building instead f individual buildings, the design elements f the prject must articulate a clear separatin. Buildings f ppsite genders may nt have access t parts f the building except at the grund level, i.e. stairs and elevatrs may nt be shared between buildings f ppsite genders. Huses are nt allwed t have bth men and wmen. Management Plan: Submit a management mdel shwing plans fr training and the hurs planned fr n-site management. A management training plan is very helpful t yur applicatin. Please be aware cndminium cmplexes are nt a desirable management frmat fr BYU cntracted husing. Maintenance Plans: Submit a prperty maintenance plan including emergency respnse, regular maintenance, and preventative maintenance. Prvide a cpy f the service cntract if this service is utsurced. Landscaping Plans: Prvide plans fr maintaining a clean and rderly landscape that will reflect an extensin f the university campus. Parking Plans: Prvide a parking plan that shws hw yu will manage the parking resurces that meet Prv City regulatins and meet the needs f the tenants. Flr Plans: Prvide an 8 ½ X 11 inch architectural drawing with details including a flr plan shwing rm dimensins, dr and windw lcatins, bedrm windw pening dimensins, hall dimensins, ceiling heights, enclsed clset bar dimensins, kitchen cunter space dimensins, and square ftage f each unit. Yu must resubmit the flr plans if there is a material change. Initial Inspectin Initial Inspectins: An initial inspectin shall be perfrmed by an Off-Campus Husing Official t determine whether the prperty will be a welcme additin t BYU Off-Campus Husing and assess ptential cncerns. Cmmittee Review Cntract Cmmittee: Yur applicatin shall be reviewed by a university cmmittee, which will decide whether r nt the university will grant yu a cntract t huse BYU students. If yur applicatin is denied, the cntracting prcess will be terminated. If Apprved by the Cntract Cmmittee: Cmplete the final requirements listed belw: (Final apprval will nt be given until a final inspectin is cmpleted n existing facilities r new cnstructin, and all the required Final Requirements are cmpleted.) Final Requirements fr BYU Cntracted Husing Create Net ID: Create a Net ID n BYU s Rute Y and submit the Net ID t the Off-Campus Husing Office t allw yu access t the listing service and student address verificatin system. Facility Certificatin Dcument: Owners shall certify in writing that their facility des cmply with The BYU Minimum Specificatins as utlined in sectin 20 f the BYU Off-Campus Husing Handbk by cmpleting the frnt and back f the Facility Certificatin Dcument and signing n the frnt page. See www.byu.edu/ffcampushusing. Handbk Training Exam: The BYU Off-Campus Husing Handbk Training Examinatin is a take-hme, pen-bk exam with 40 multiple-chice questins. The wner and their authrized agent(s) must pass the exam. The Handbk can be fund at www.byu.edu/ffcampushusing. Rental Dwelling License Applicatin Respnse Letter: Unless yur prperty is wner ccupied, yu must submit t the Off- Campus Husing Office a cpy f their Rental Dwelling License Applicatin Respnse Letter. Yu can btain this dcument frm Prv City fr a fee. Please allw tw t three weeks fr a respnse letter frm Prv City. Final Inspectin: A final inspectin shall be perfrmed by an Off-Campus Husing Official t cnfirm that crrectable prblems nted in the initial inspectin(s) are reslved. Agreement fr Cntracted Off-Campus Husing and Addendum One Frms: Yu shall agree in writing t cmply with and pursue student cmpliance t the university s husing plicies as stated in the Agreement fr Cntracted Off-Campus Husing and The BYU Off-Campus Husing Handbk. Yu must prvide the names, addresses and Net ID s f at least tw cntacts. One must reside within the immediate area f Prv, Utah.
