Details needed for Church Records: Records and Details Baptized Full Name of Candidate: Candidate s Birthdate: Mother s Maiden Name: Father s Name: Date of Candidates Baptism: Name of Parish Candidate was Baptized: Diocese Candidate was Baptized: If Candidate was baptized outside the Diocese of Kalamazoo we need a copy of the baptismal certificate. Return copy with Confirmation Packet in August. If you have questions please do not hesitate to contact Susie or Dale Boone at: 269-226-0064 text or call, use the app or email - confirmation@stannaugusta.org.

What are the requirements for a Confirmation Name? In order to manifest the close relationship of Confirmation to Baptism, the candidates, according the longstanding custom of the Church, may retain their baptismal name. However, a special name may be chosen if desired, in which case it should be that of a recognized saint of the Church, or a person from Scripture. (CF Canon #855) How do you choose? Ask people who their favorite saint is. Start conversation and ask people what saints they know and their stories. Go to the Newman s Bookshoppe in Kalamazoo. Browse and read about the saints. Plan an hour or so and dive into some books. Check with Pat Vanderpool at St. Ann. She has many really good resources to research the lives of saints. For more information on Saints and biblical characters you can visit: catholic.org franciscanmedia.org Learn as much as you can about the saint and for life you have someone to pray with. Parents, once a name has been selected, download/buy/borrow a prayer card of that Saint (Google, Newman s, Amazon, Catholic Gifts) and start praying to him/her for your child. If you have questions please do not hesitate to contact Susie or Dale Boone at: 269-226-0064 text or call, use the app or email - confirmation@stannaugusta.org.

Saint Name Candidate s Name Saint Name Information I am choosing as my Confirmation Saint. Please share some facts about this saint s life: Why is he/she a saint? What were some significant aspects of their life? Where and when did they live? What miracles have been attributed to their intercession? Please explain why you choose this saint as your Confirmation name and patron/patroness: What excites/inspires you about this saint s life? What connections do you have with this saint? Do you have anything in common with this saint? How might you ask this saint intercede for you?

Choosing A Sponsor All Confirmation candidates must be presented to the Bishop by a qualified sponsor. This sponsor, while chosen by the candidate, must have certain qualities that are spelled out fully in church law. A sponsor: Must be a Catholic who actively practices the faith and attends Mass weekly. Must be at least 16 years old and already confirmed. May not be a parent of the candidate. If married, the marriage was in a Catholic church or validated by the Catholic Church. Need not be the same gender as the candidate. In addition to the requirements stated above, a sponsor should exemplify these qualities: The life of this person is a testimony of his/her commitment to the Lord and to the Church. This person is willing and eager to guide the candidate as they continue to learn: To pray in new ways To witness To recognize the workings of the Holy Spirit More about the Catholic faith More about their parish and how to participate in parish life How to overcome doubts, fear, and temptation through their relationship with God This person will pray for the candidate regularly. Is the candidate able to approach this person with questions they have about their faith and spirituality? After the candidate has prayerfully chosen a Sponsor, have him/her complete the Confirmation Sponsor Certificate. All sponsors must have their Sponsor Information Form signed by their parish pastors. Sponsors are asked to listen to two Confirmation presentations and encourage candidate and parent to listen to a presentation. Sponsors are asked to attend practice/rehearsal the night before Confirmation Mass and attend Confirmation Mass. Sponsors are respectfully asked to dress with a form of modesty for practice and Confirmation Mass.

Meetings and Adoration Preparation was designed to aim at leading the candidate toward a more intimate union with Christ and a more lively familiarity with the Holy Spirit - his actions, his gifts, and his biddings - in order to be more capable of assuming the apostolic responsibilities of Christian life. To this end catechesis for Confirmation should strive to awaken a sense of belonging to the Church of Jesus Christ, the universal Church as well as the parish community. (CCC 1309) While you might not have made it to every meeting, Adoration is the alternative to prepare for the Sacrament. Adoration is not a punishment. No No No! Adoration is preparing your heart, mind and soul to receive a most beautiful gift and grace. Our meetings are designed to draw you closer to Christ but there is nothing better than spending an hour with our Lord in the Blessed Sacrament! That is the best preparation. Adoration is an intimate union with Christ. If you missed meetings and have not gone to Adoration, do not stress, there is plenty of time!! The chapel at St. Ann is open 24/7, start now and go an hour a week. Take your Decision point book or sit silently and talk to Jesus. Prepare. Provide a brief reflection on the past year preparing for the Sacrament of Confirmation. Parent Letter to Pastor The Church fully understands and appreciates the role of parents as first and best teachers of the faith. Due to your most complete knowledge and insights we are asking you to share with us your son s or daughter s disposition toward the Sacrament of Confirmation. Below are listed some suggestions of how you might choose to characterize your son s or daughter s level of readiness to be confirmed in the faith of the Catholic Church. It is not necessary to discuss each item identified; the bullets are simply ways to think about your son or daughter. How might you describe you son s or daughter s prayer life? (frequency, regularity, longevity, depth, significance) What would you say is your son s or daughter s level of knowledge of the faith? In what ways do you witness you son or daughter daily integrating the moral teachings of the Church? Does your child participate fully in the sacramental life of the Church? (Mass on Sunday as well as regular Confession) Does your son or daughter have a personal faith in Jesus Christ and the teachings of the Church? Does your son or daughter have a sufficient understanding of the graces and responsibilities connected to the Sacrament of Confirmation? Return letter with Confirmation Packet in August. Pastor Meeting with Candidates In June a schedule will be available for Candidates to select a session with Father before the Mass of Confirmation. Dress n Up for Confirmation This is a BIG deal and BIG moment in your life. Let s dress like it is! Young Men: Jacket, shirt and tie, dress pants and polish up the shoes. You will feel like the man that God has created you to be. Trust us, you will be able to use the jacket during high school. Goodwill is a great place to scout jackets and shopping at Macy s with a good coupon is a deal :) Our boys might have a few jackets that can be loaners. Young Ladies: What a special time to present ourselves to our parish family, our priest and our Bishop. Instead of giving a list of don ts how about we go with some do s: Wear a top with sleeves Wear a top that covers your shoulders Wear a top that covers your back Wear a top that covers your cleavage Wear a dress or skirt that comes to your knees or below Of course we do not want this to be a financial burden, we can help and will enjoy making the day extra special. If you have questions please do not hesitate to contact Susie or Dale Boone at: 269-226-0064 text or call, use the app or email - confirmation@stannaugusta.org.
