Dear Parents, The purpose of the Discipleship Award Program is to help the young people of St. Lawrence learn, explore, and experience our Catholic faith in a way that leads them to a greater understanding of and a deep, lifelong commitment to our faith. It is important for our children to learn that we are not passive spectators in our relationship with God. Each and every day, we must engage ourselves in activities that draw us closer to God and to our final goal of eternity with him in Heaven. It is the goal of this program to give our children opportunities to do just this, engage in activities that draw them closer to God, and to recognize them for their efforts. The Discipleship Award Program is an optional program for those students and families who wish to use it. There is an award booklet for each grade level, Kindergarten through 8th Grade. Each grade level booklet will contain age appropriate activities. It will be important however, especially with younger children, for parents to be actively involved in assisting children with completing the program requirements. In fact, some of the activities included are family activities. The Church teaches that Parents are the principal and first educators of their children (CCC 1653), and children will receive the most benefit from this program if families are actively involved in the learning process. How the program works: Each year, students will have the opportunity to achieve the discipleship award three times (see due dates to turn in paperwork on the following pages). If a student earns all three awards in a given year, he/she will receive special recognition in the form of an additional award, called the Trinity Award. Students who achieve the Trinity Award for 3 or more consecutive years are invited to an annual dinner in their honor. The activities within the program are separated into four categories based on the Four Pillars of the Catholic Church: 1) What We Believe; 2) How We Worship; 3) How We Live; and 4) How We Pray. Some activities within each category will be required, and some of the activities will be listed as options within each category. The due dates and directions for each of the award periods are given on the following pages. Homeschoolers: When instructions are given to share something with a class, the student can share the project at a homeschooling or similar type gathering. We look forward to partnering with you in encouraging our students to grow in knowledge and faith. 2018-2019 Fifth Grade Discipleship Award Part A Due Monday, November 5th The requirements for this award are listed below. This form and any required attachments must be turned in to the Campus Ministry office by Monday, November 5th. (Form can be turned in to the school or Faith Formation office, or mailed to the Campus Ministry Office 5223 N. Himes Ave. Tampa, FL 33614.) Section One: What We Believe REQUIRED ACTIVITY We believe in the Blessed Trinity, the Three Persons in One God: God the Father, God the Son, and God the Holy Spirit. The Nicene Creed is a profession, or statement, of what we believe as Catholic Christians. On a separate sheet of paper, make a chart with three columns, one column for each of the Persons of the Trinity. Then, list in the columns our beliefs about each of the Three Persons of the Trinity that are stated in the Nicene Creed. Attach your list to this form. CHOOSE ONE OF THE FOLLOWING ACTIVITIES Option #1: On a separate sheet of paper, write a reflection or poem about the Blessed Trinity. Ask your teacher for permission to share your reflection or poem with your class. After sharing your reflection or poem, have your teacher sign here. Option #2: We believe that the saints are examples of holiness for us. With your parent s help, go to the following site:https://religion.sadlierconnect.com. Click on Religious Resources, then select the icon for The Lives of the Saints. Read about one of the following saints: St. Elizabeth Ann Seaton, (Jan. 4) or Gregory XIII, pope (April 10), or St. Madeline Sophie Barat. (May 25) After about the saint, click on and print out the Intermediate Activity. Complete the activity and attach it to this form. Section Two: How We Worship REQUIRED ACTIVITY As Catholics, we believe that the Sunday celebration of the Lord s Day and his Eucharist is at the heart of the Church s life (CCC 2177). It is important that we gather as a Church family each Sunday and on Holy Days to worship. When you attend Mass on Sunday or a Holy Day, USE THE I attended Mass today STAMP AT ONE OF THE CHURCH EXITS TO STAMP THE APPROPRIATE DATE on at least 7 of the following weekends/holy Days. Section Two Continued CHOOSE TWO OF THE FOLLOWING ACTIVITIES: Option #1: With your family, serve as greeters at a Sunday Mass. You should arrive 15 minutes prior to Mass to greet people and open the church doors for them as they arrive for Mass. Write the date you served and have the priest or deacon sign here. Option #2: Carry a banner in the Saint Procession at the at the 9:30 Mass on Sunday, October 28th. Banner carriers should arrive no later than 9:20 and report to Higgins Hall. Use the I completed this activity stamp at the back of the church to stamp here: Option #3: With your family, write the names of your deceased family members in the Book of the Deceased which is displayed in the church during the month of November, OR place photos of deceased loved ones on the Wall of Memories which is displayed in the church during the month of November. Write the date you wrote the names in the book or hung the pictures here. Option #4: With your family, attend the All Souls Procession and Mass on November 2nd at 7:00 PM. See the bulletin or website for details. Have the priest or deacon sign here. Option #5 : Baptism is the foundation of Christian life. Through our Baptism, we become God s children and share in Jesus role of priest, prophet, and king. Attend a celebration of the sacrament of Baptism at St. Lawrence or another Catholic church (between September 5 and November 5). On a separate sheet of paper, write a paragraph or two explaining what you experienced. Make sure to name who was baptized and the name of the priest or deacon. Attach your description to this form. Optional: Take a picture with the newly baptized and attach it also. Option #6: With your family attend the Family Faith Fest celebration of the Diocese 50th Anniversary, October 27th at Al Lopez Park. Stop at the St. Lawrence table sign in. Take a picture of you and your family at your favorite event and attach it to this packet. Section Three: How We Live We believe that when we serve others we give witness to Christ. CHOOSE TWO OF THE FOLLOWING ACTIVITIES Option #1: We give witness to Jesus Christ when we perform the Works of Mercy. Learn both the Spiritual and Corporal Works of Mercy and recite them for your teacher, Mrs. Roman or Mrs. Williford. Have your teacher, Mrs. Roman or Mrs. Williford sign here. For a list of the works of Mercy, use your religion book, the Catechism of the Catholic Church (Par. 2447), or this site: http://www.loyolapress.com/corporal-and-spiritual-works-of-mercy.htm. Option #2: Choose two of the Corporal Works of Mercy and two of the Spiritual Works of Mercy to perform during one week s time. On a separate sheet of paper, keep a journal of how you performed these Works of Mercy during the week. Attach your journal to this form. Option #3: Lead your family in a family project to choose two of the Corporal Works of Mercy and two of the Spiritual Works of Mercy to perform during one week s time. Option #4: Donate 10% of your Halloween Candy to the Annual Candy Collection (Nov. 1-5). Write down the date you donated the candy here. 4
Section Four: How We Pray CHOOSE TWO OF THE FOLLOWING ACTIVITIES: Option #1: Take your pet to the Blessing of Pets and Animals on September 30th at 3:00 PM or October 4th at 7:00 PM. Have the priest or deacon sign here. Option #2: During the month of October, visit the Marian Shrine in the church. Pray one decade of the Rosary for your own personal intention. Write the date you visited the shrine here. Need help with how to pray the Rosary? Ask your parents go to this website: http://www.ewtn.com/ Devotionals/prayers/rosary/how_to.htm) Option #3: The basic forms of prayer are: blessing, petition, intercession, thanksgiving, and praise. Using your religion book, this website: http://www.loyolapress.com/our-catholic-faith/prayer/ personal-prayer-life/different-ways-to-pray/prayer-and-forms-of-prayer or the Catechism, research these different forms of prayer. Then, on a separate sheet of paper, create a short prayer of each type. Attach your prayers to this form. Option #4: With your family, make a special visit to the Christ Child Chapel for Adoration on any Thursday, Friday, or Saturday. If you do not know the code to enter the Christ Child Chapel, call the parish office at 875-4040. List the date of your Adoration visit here. Option #5: With your family, participate in the parish Traveling Chalice Program to pray for vocations. Contact Mrs. Boston (875-4040) to schedule your family s week with the chalice. Once you have the chalice, follow the instructions provided and pray each day of the week, as a family, for vocations. Take a picture of your family with the chalice and attach the picture to this form. 5
