Life Experience & Ministry Evaluation for Bachelor Degree Program General Information Completing this portfolio is an opportunity to receive college credit for significant learning experiences obtained outside the formal college classroom and curriculum. Since our degrees are in the area of Christian service, students with ministry experience are eligible for additional credit based on the quality and suitability of their past activities. There is no charge to receive and review this questionnaire; however if you wish to obtain any college credits, this document must be completed and submitted to Patriot Bible University with the $40 evaluation fee. No guarantees are made that your experience qualifies for credit. If credits are deserved, then students pay $5 for each credit hour earned in this manner. If you decide you are not eligible or do not wish to complete this evaluation, simply discard this form. No other response is necessary. Life Experience Many workshops, seminars, volunteer activities, and church related programs are eligible for credit. Certain learning experiences while in the military service or your employment may also be eligible. All volunteer Christian experience should be listed, including Sunday School teaching, Awana and other youth activities, Bible Institute classes, and positions of responsibility in your local church or other Christian organizations (Child Evangelism, Youth for Christ, Navigators, Bible Study Fellowship, etc.). Please give the name of a person who can verify your experience or submit documentation that validates your experience. List all activities where you gained significant educational information.
