Please make sure to fill out both sides. Returning students need to fill out a form as well as new students who also must turn in copy of baptismal certificate with their registration form. All families must be registered in the church parish. Please visit the church office to do so. The new CCD school year will begin with an Open House on Wed., August 29th at 6PM in the La Salette Hall.
