September 21 st, 2018 Dear Parents and Students: The Holy Trinity Youth Group will host a 4-week Skiing/Snowboard Program this coming January and February, 2019 at Perfect North for their 4-week Club Program!! All the necessary paperwork is enclosed. You must submit ALL completed forms before the due dates (October 20 th and November 13 th ) so that your child may participate in the ski event. We will accept NO late submissions. Please read through this information carefully with your child. Holy Trinity youth group is only offering transportation and chaperones. 55 passenger coach buses will transport us to Perfect North. Filling out the permission form DOES NOT guarantee you a spot on the trip. Paperwork submission to me AND online registration and payment with the Perfect North Club Program are still required to guarantee a spot! DATES & TIMES Ski Fridays: January 4 th, January 25 th, February 1 st, February 8 th We will meet at the Holy Trinity Youth Center at 4:00pm each time. Do not arrive before 4pm and hang out. The bus departs at 4:30 pm sharp. We will not hold the bus for latecomers! We will load the bus from Perfect North Slopes at 10pm & then return to the Holy Trinity Youth Center between midnight & 12:30am. COSTS Cost for the Perfect North Ski Club is as follows (paid to PERFECT NORTH, NOT Holy Trinity): Club Pass Program (4 weeks Skiing/Snowboarding): $130 for 4 Fridays $80 bus fee - Perfect North will be collecting money for the bus, so this price includes transportation costs for all 4 trips Without Rentals (skis/snowboard/helmet) - total of $210 With Rentals - total of $270 Additional Options: Ski/Snowboard Rentals: $60 per week Helmet Rental included with rentals (youth are required to wear helmets. He/she can bring personal helmet) Additional money is needed for food, if you d like. However, parent volunteers often bring food to share. DIRECTIONS TO PERFECT NORTH From Louisville, KY - approx 113 miles: Take I-71 N, take exit 185 to merge onto I-275 W toward the Airport. Take exit 16 toward Greendale/Lawrenceburg, IN. Merge onto Belleview Dr. Turn right into IN-1 N. Turn left to stay on IN-1 N. Turn left onto Pella Crossing. Turn left onto Perfect Place Lane. Perfect North will be on your left. REGISTERING TWO STEP PROCESS 1. Register with Holy Trinity Youth Ministry via Melissa Fisackerly The following from this packet must be returned to Melissa before you can order your passes: Fill and Sign out Ski Program Community Rules Agreement & Permission Form. Fill and Sign out HTYG s Ski Program- Lesson and Helmet Waiver Form(s) Fill and Sign out Health Release Form/General Permission Slip. Attach a copy of your child s insurance card (front & back) Upon submission, you will receive a club password to register online with Perfect North for your passes. 2. Register with Perfect North online Visit www.perfectnorth.com Select Club Log-in located on the bottom-left corner of the Perfect North home page Click H and select Holy Trinity Youth Group from the list of clubs Type in your club password Click on Purchase Items Select Club Program Pass then Friday Then Choose either with or without rentals Then click Add to Cart Once you ve completed that selection, you can either check out or click Continue Shopping and add additional items. When checking out, you will need to Add/Change Guest to each purchased item. This will give you the opportunity to input your information. Follow the on-screen instructions to do so. ADULT CHAPERONES We need parents to help chaperone this event. Chaperones!! You do not have to attend all 4 weeks, however that is allowed. You can choose which weeks you d like to chaperone. CHAPERONES: We know that with the size of our club need extra help. The Chaperone Pass is created for our club to be able to bring different helpers with you each week to assist you with your school club when arriving by bus. Ideally, I would like chaperones to help with the kids from the time they get off the bus, go to the outside ticket window to get their lift ticket and equipment rental form (if needed). Next, chaperones should help the kids at rental shop and assist them with their equipment. They should also assist the kids with getting out to their lesson if they are taking one. When your bus arrives at Perfect North Slopes for your club s first outing, we are asking you to remind everyone that the equipment is not permitted in the Ski Lodge this year with the exception of the West Lodge. Entry can be made by the side doors of the West Lodge in order to pick up passes or lift tickets. Equipment must remain in their possession at all times while inside. They should also leave by the same doors. The Chaperone Pass will include lift tickets and equipment rentals, if needed, for the people I have helping my club. This pass is only valid on your lesson night and the card is kept on file in the PNS office. Melissa, Jason Garvey, or Chip Hayward, will go inside information/ticket window to pick up these tickets and/or equipment rental forms. Holy Trinity Parish will be given one lift ticket and equipment rental form (if needed) for every 15 kids that you have with you. If additional tickets/equipment rental is needed, you can purchase these at a discounted rate. Only you can make this purchase for your extra people in one transaction. The lift ticket discount price will be $34 and the equipment rental discount price will be $20. Only the Introductory Lesson is available to chaperones. They do not qualify for the 4 week lesson program. If you have the same chaperone each week, they should either purchase a pass or you can use your points to help them get a pass. Again, this pass is to get different people each week to help you with your club. FOOD DONATIONS Something we want to continue ONLY if we have the help of chaperones was providing food for the participants. Perfect North lets groups bring their own food, such as: chili, soups, sandwich trays, veggie trays, etc. If you d like to donate food for the trip, please Kim Wade and sign up with on signupgenius and indicate what trip you d like to supply food and what snack you will be offering.

I will abide by the rules set by Perfect North. I will remain within Perfect North boundaries at all times. I will listen and abide by the directions of the leaders and Perfect North staff. I will pay for any damage to the facility or other s property for which I am responsible. I will respect the facility and environment and will do my part to keep all areas clean. I will treat others with respect. I understand that trips may be cancelled at any time. Important: I understand that the Community Rules are designed for the safety and well being of all participants. Violation of these rules may result in a phone call to your parents, sanctions and/or immediate dismissal from Perfect North at the discretion of the Director. If I am dismissed, my parents will be called to pick me up from Perfect North. This action may take place any time. I understand that this program is designed to build community, create an atmosphere of trust and foster spiritual growth among the Youth Group members. I understand that skiing is an inherently dangerous activity. I understand that the Archdiocese of Louisville and Holy Trinity Church do not provide insurance coverage for injuries incurred while on this trip. In consideration of permitting my child to participate, I hereby release and save harmless the parish of Holy Trinity Church, the Youth Director and youth leaders any and all liability for any injuries, loss, or other claims arising or resulting from this activity. Since there have been requests in the past, below is the OPTIONAL opportunity for you to release your child from one or both of these requirements.
