3 STUDENT/FAMILY PROFILE Through academic assessment and the family interview, we seek to learn as much as possible about each student applying to The City School. Your honest response will allow us to serve each family to the best of our ability. How would you describe your child s personality? What are your child s extracurricular and/or musical interests or achievements? How would you describe your child s behavior at home? How does your child respond to authority and correction? What are the three most important things you are looking for in a school for your child? What are your reasons for selecting The City School? Does the student s family regularly attend or belong to a church? (**SEE NOTE BELOW) YES NO How would you describe your church involvement? Very Active Active Moderate Nominal Name of Church: Pastor: Address: Number and Street City State Zip Code Denominational preference (if any): Do you profess to be a Christian? YES NO If yes, help us to understand what that means to you. What makes you a Christian? How did you become a Christian? In what ways do you provide instruction for your child at home? How can our school help your child grow spiritually? **The City School is committed to Christ-centered education, Christian leaders, faculty, and staff, a Christian philosophy of education, and a Christ-centered curriculum. Although most of our families profess to be Christians, this is not a requirement for enrollment. However, the school does require that you understand and support the Christian nature of your child s education (see School Covenant). These questions will help us learn why you have chosen to apply to a Christian school.

4 SCHOOL COVENANT The City School is committed to spiritual and social formation and to academic growth in its student. Success is possible with the cooperation and dedication of three principle partners in your child's education: the parents/guardians, the student and the school. Through this covenant made between The City School and you, we pledge to strive, with the help of the Lord, to meet the expectations listed below. School Commitment As your child s teachers and administration, we commit to: 1. Provide a school community that is Christ-centered, safe, and nurturing. 2. Provide social, spiritual, and academic challenges which promote continual learning, growth, and improvement and which will promote acceptance into college. 3. Keep you informed of your child's progress through four sets of interim reports, four report cards, a parent/teacher conference session, and phone calls. 4. Work with you to assist with the spiritual, social, and academic development of your child. Family Commitment As a family we commit to: 1. Supporting the school's Mission Statement and Statement of Faith as the foundation for my child's education and supporting the spiritual, moral, and disciplinary standards of the school. 2. Ensuring that my child is at school every day on time, dressed according to dress code, with necessary books, materials, and supplies, except in cases of serious illness or family emergency. 3. Monitoring my child's academic progress and helping to hold him/her accountable for assignments. 4. Providing my child with a quiet, distraction-free environment for homework. 5. Agreeing to remain informed by reading all correspondence and by responding in a timely manner when requested. I also agree to communicate with the school office if information changes (e.g. phone numbers, emergency contact information, and medical information). 6. Attending all parent-teacher conferences (highlighted in the monthly family news) plus any additional requests for conferences as recommended by my child's teacher. 7. I understand that all school events and field trips are considered part of the curriculum, and the attendance of all students in these activities is vitally important. I understand that if my child does not participate that his/her grade will be adversely affected. 8. Not sending my child to school when he/she is ill, to prevent illness from spreading to other students. 9. Supporting the school's guidelines and policies/procedures as they are described in the school handbook. The school reserves the right to dismiss students whose behavior, or whose parent/guardian's behavior, does not respect it's standards or cooperate in the educational process. 10. Meeting all my financial obligations in a timely manner according to the policies listed under the financial policies section of the family handbook. 11. If I become dissatisfied with the school in any respect, I will seek to resolve the matter with the person or persons involved rather than spread criticism or hold a negative attitude in my heart (Matthew 18:15).

5 MISSION STATEMENT Our mission is to train student s minds, disciple their hearts, and bring light to the city one child at a time. STATEMENT OF FAITH At The City School, education is grounded in our Christian faith which flows from the conviction that the Bible is the inspired and authoritative word of God, the only infallible rule of faith and practice. We therefore affirm the following: That there is only one sovereign God, eternally existent in three persons: Father, Son, and Holy Spirit. The full deity-and-humanity of our Lord Jesus Christ, his virgin birth, sinless life, and proclamation of the kingdom through word and deed, leading to his atoning death in which he was made to be sin and through which believers are reconciled to God; his bodily resurrection, ascension to the right hand of God the Father, and personal return in power and glory. That all humanity has sinned rebelled against God and fallen short of the glory of God, resulting in spiritual and physical death. That a person is justified and reconciled with God by grace alone through faith alone in the Lord Jesus Christ alone. That unless a person is spiritually reborn by the Holy Spirit he or she will not enter the kingdom of heaven. The present ministry of the Holy Spirit, whose indwelling enables the Christian to walk by the Spirit, demonstrate the fruit of the Spirit, and follow Jesus through self-denial and cross-bearing. The continuing ministry of the Spirit to create the church, God s holy nation, and empower her to carry on the work that Jesus began while on earth, making disciples and bearing witness to the hope of the gospel. The spiritual unity of believers in our Lord Jesus Christ and his church. The will of God, revealed in the Bible, is for shalom peace, reconciliation, and harmony between God, humanity, and creation realized through faith in and obedience to Jesus Christ, who is the true source of peace and reconciliation. The fulfillment of God s kingdom when Jesus returns in glory to judge the living and the dead, ending the struggles of this age and bringing God s final victory in a new heaven and a new earth. NONDISCRIMINATION POLICY Enrollment shall be open to children of all faiths regardless of race, color, and national or ethnic origin.
