Annual Faith Formation Report Each year faith formation leaders are asked to provide information through the Annual Faith Formation Report (AFFR). We appreciate the time it takes to complete this report which helps these offices to better understand the needs and services offered in parishes. We have made some adjustments to this year s report based on feedback from you. It is easier to update catechist information. Additionally, we have given you the ability to list all the ways that you provide systematic faith formation opportunities. Completing the survey should be as easy as ever. To access the survey go to the Archdiocesan website (http://www.dbqarch.org), click on Archdiocesan Data System at the bottom right and then AFFR. Or, you can go directly to the AFFR login page at (https://fmp.dbqarch.org/fmi/webd/#affr2017). Each faith formation leader will be sent a unique login and password when the AFFR becomes active this fall. Please do not share the login/password with anyone since that password gives access to your personal data. We know that this will take some time to complete, and that it can look a little overwhelming at first. Our goal is to have this report be as user-friendly as possible. We created a picture outline for you with screen shots of different parts of the report and the information that will need to be collected. If you have any questions or frustrations, please don t hesitate to call or email us. We are here to help in any way we can. Annual Faith Formation Report (AFFR) AFFR Home Page On the home page, click on one of the buttons to enter information: Catechetical Programs, Sacramental Prep, Other Programs & Ministries, Personal (FFL) Information, or Catechist and Volunteer Information. General Overview Verify that the parish(es) listed are correct and continue. Once you click yes, you will not see this page again. Click on the Continue to Program Information to advance to the next screen. If changes need to be made, please contact the Office of Catechetical Services. 3
RE Program Information Home Page An opportunity is a session of formal religious education. Please complete one opportunity for each time/day/place you gather a different group of children. Click the Edit RE Opportunities button to enter opportunity information, enrollment, and materials used. Click RE Program Home Page when done. Catechist/Small Group Leader/Program Volunteer Information Please provide the data requested on each of your catechists*. The catechists listed are those who were active in 16-17. Please check yes if they are active in 17-18. Please indicate Type of Volunteer (Drop down menu as follows: Catechist, Classroom Aide, Hall Monitor, Site Supervisor, Small Group Leader.) To add another catechist, click the Add Another Catechist button. To edit a catechist, click the yes box on the right for each catechist you would like to edit and then click the Edit Catechist button. Please indicate the total number of catechists for the grade level groups, the hourly pay and any other information that may be helpful. * Catechist = Catechist, Small Group Leader, Program Volunteer 11
New Catechist Information Please enter the catechist s name, volunteer type (Catechist, Classroom Aide, Hall Monitor, Site Supervisor or Small Group Leader). If you know the dates of the background check and Virtus training, please list those dates. The dates will be confirmed by Archdiocesan staff. Edit Catechist Information Edit the fields as necessary. To change a catechist s name/spelling, enter the changed name in the Name Change fields. To delete a catechist from your records, select the Delete Catechist button on the right. THIS CANNOT BE UNDONE. If you are editing more than one catechist, use these buttons to move from one to another. Click here to delete the current catechist. Careful!!! This cannot be undone. 12

Employment Information Please provide salary and benefit information for yourself. Each FFL will only have access to his/her own information. This year the annual salary and hourly rates have a drop down menu with pay ranges. This information is used to track professional compensation and assist other parishes with employment offers.
