I hereby certify that the said pioneers mentioned in this application, and the facts herein set forth, are true to the best of my knowledge and belief. I understand that I will be expected to research the records of my pioneer ancestors, write their histories, and read them at camp meetings. After each history is read at the camp meeting, it is turned over to the Camp Historian for transmittal to the Company Historian who forwards it to the International Historian, International Society Daughters of Utah Pioneers, where it is permanently recorded and filed. 5 INTERNATIONAL SOCIETY Daughters of Utah Pioneers 300 North Main, Salt Lake City, Utah INSTRUCTIONS TO COMPLETE APPLICATION If you are applying as a Member-At-Large, read the paragraph at the bottom of these instructions entitled FOR MEMBER-AT-LARGE before proceeding. Page 1 1. DO NOT enter anything for NO.. ISDUP will enter the membership number here. 2. Print the name of the camp and company you are joining and the city and state where the camp is located. Canadian residents include city and province. 3. Print your full name including maiden name and all married names. 4. Print the full name of your husband, even if divorced or deceased. 5. Print your mailing address (include apartment number or P. O. Box number). Please include a 9-digit zip code. This is needed for bulk mailing rates. Page 2 This section requests information on direct-line (biological) pioneer ancestors who came to the geographic area covered by the State of Deseret or Utah Territory between July 1847 and 10 May 1869, through whom you are eligible for membership in International Society Daughters of Utah Pioneers. Only one direct-line pioneer ancestor is required for you to be eligible for membership, BUT please list four direct-line pioneer ancestors if you have them. Both Paternal and Maternal lines can be used. A direct-line pioneer ancestor represents a direct, parent-to-child relationship from one generation to the next back to the pioneer ancestor that qualifies you for membership. 1. Print your full name on the top of the page. 2. Print your full name again and your birthdate. Birth dates should always include day, month, and year. 3. Please complete the section asking for your direct line pioneer ancestors, their birth place, birthdate, and the pioneer's relationship to you. There are spaces for four direct-line ancestors. 4. Print the year they came to geographic area covered by State of Deseret or Utah Territory. 5. Print the wagon company in which your pioneer ancestor came. Many can be found in Mormon Overland Travel website (Google Pioneer Company Search). 6. Print where your direct-line ancestor first settled. The state should be the present-day state. 7. Print your pioneer's death date and place of death.

6 Page 3 1. Print your full name on the top of the page. 2. Begin the pedigree by listing your parents and grandparents. Choose one grandparent from lines 1-4 from whom one of your direct-line ancestors comes and write that grandparent's name on the pedigree line labeled Grandparent in the pedigree below. Continue to trace your pedigree to direct line pioneer ancestor through whom you are eligible for 1SDUP membership. The Pedigree Chart has space to accommodate through 5th great-grandparents, though you may not have to go back that far to reach a qualifying ancestor. 3. A proper pedigree must be submitted with the application. Filling out the pedigree on this page of the application form as described, fulfills this requirement. 4. In the space designated, write a short history of one of the direct-line pioneer ancestors listed on page 2 of the application. Indicate the names of their parents, birthplace, name of spouse and children and how they crossed the plains. Give any information to prove they were in the State of Deseret by May 10, Page 4 1. Print your full name on the top of the page. 2. List full names of your children and their birth dates. 3. PRINT OFF A COPY OF YOUR APPLICATION IF FILLING IT OUT ON THE COMPUTER (you may not be able to save your information). 4. Read the PLEDGE and sign where it says "Applicant's Legal Signature." NOTE: THIS IS A LEGAL DOCUMENT AND THEREFORE NEEDS TO BE SIGNED BEFORE WITNESSES (husband, friend, or relative is sufficient). These persons are to witness you signing the Membership Application form, not to verify any information in the application. 5. Give the application to your Camp Registrar for approval and one check made out to ISDUP for $20.00 to pay for $15.00 International dues and $5.00 registration fee. The Camp Registrar will fill in the amount being sent and the check number. She will also assist you with the name of the camp and company you are joining, if needed. 6. This is a legal document and must be signed by the Camp Captain and Registrar and the Company President and Registrar. The Company Registrar will fill out the last line requesting Company Registrar s Mailing Address (see page 1, no. 6 above for information on where to get the 9-digit zip code). 7. Company Registrars will mail the completed application with the check for $20.00 to: International Registrar, C/O of International Society Daughters of Utah Pioneers, 300 N. Main St, Salt Lake City, UT FOR MEMBER-AT-LARGE The same directions above apply with the following exceptions. Check the box on the first page where indicated for Member-At-Large. Skip the first three lines of the application and begin on fourth line Applicant s Name. You do not need the signatures of the camp or company officers, only the witnesses. Disregard the last line concerning the Company Registrar s address information.
