3 Cub Scouts Pack 120: Popcorn Sales Popcorn sales have begun! Our goal is for each Scout to sell $100 of popcorn. Scouts can win prizes for selling a minimum of $25 door-to-door, and an Amazon gift card for selling a minimum of $100 online. 7 CONSENT AND LIABILITY WAIVER Please check the box that applies Adult (Over 18) Participant I wish to participate in the 5K or 1 Mile to be held on September 15, 2018 (the CK Run ). I represent and warrant to Christ the King Catholic Church and School that: To the best of my knowledge, I am in good health and physically able to participate in the CK Run and I assume all responsibility for my health and physical condition and my ability to participate. I understand and know that the course will be closed to traffic only between 8 a.m. and 10 a.m. and that there will be a possibility of traffic along the course; I understand and assume the risks of participation in the CK Run, including, but not limited to, falls or collisions with other participants and/or motor vehicles; I understand and agree that I am solely responsible for my personal property before, during and after the CK Run; I understand all registrations are final and no refunds will be issued in the event I am unable to participate or the CK Run is cancelled, and I agree to abide by all rules and regulations for participation in the CK Run, including without limitation, rules for the use of baby strollers, to wit, baby strollers are only allowed to line up at the rear of the 5K or 1 Mile race and cannot interfere with other participants. And, I understand that any failure on my part to follow the rules and regulations of the CK Run may result in my being asked to leave the CK Run. LIABILTY WAVER: Knowing the foregoing facts, and in consideration of accepting entry, set forth herein, I do on behalf of myself and Participant, our heirs, successors, assigns and next of kin, release, waive, hold harmless, defend and covenant NOT TO SUE and agree to defend, protect, indemnify, hold harmless, release and forever discharge, the Archbishop of the Archdiocese of Oklahoma City, the Archdiocese of Oklahoma City, Christ the King Church and School or any of their respective departments, directors, administrators, teachers, officers, agents, representatives, volunteers, helpers and employees, and other CK Run participants and attendees from any and all actions, claims, demands or liabilities whatsoever arising out of, or by statute or at common law, or under any other legal theory, including without limitation, those for personal injuries or property damage, that I may suffer due to illness or injury as a result of, or in connection with, participation in the CK Run, including without limitation, travel to and from the CK Run and medical treatment and any consequences that may arise as the result of said treatment, to the fullest extent permitted by law. CONSENT TO TREATMENT: In the event of circumstances that indicate that I or Participant are in need of immediate medical care, I authorize and give permission for myself and/or participant to be transported to a hospital/clinic/medical facility for evaluation and emergency medical or surgical treatment, including any necessary X-ray examination. I authorize any licensed physician or medical center to treat me and/or participant. I accept full responsibility for any associated medical or hospital bills. PHOTOGRAPHY CONSENT: I also grant permission to Christ the King Church and School to photograph or videotape me and/or participant in connection with the CK Run and to use any photographs or videos which include an image of me and/or participant for promotions and publicity of Christ the King Church and/or School as deemed appropriate by Christ the King Church and School, without acknowledgement and without being entitled to remuneration or compensation. 8 AUCTION JEANS DAY IS MONDAY! THIS IS THE ONLY DAY KIDS CAN WEAR SPORTS/NYLON SHORTS TO SCHOOL!!! Your child gets to wear any shirt with either jeans or sports/nylon shorts the 2nd Monday of every month beginning in September! *Please be sure your child's shorts go to the knee the uniform lengths still apply. Auction Jeans Day dates are as follows: September 10 October 8 November 12 December 10 January 14 February 11 March 11 April 8 May 13 Please make a check out to CK PTO and give it to Erin in the front office. The cost is $35 per student. In the memo line please list Auction Jeans Day. Auction tickets and sponsorships are now available for purchase on the CK website.
