1 Christ Church Christmas Camp days of camp total 2 Days December 27 th and 28 th 2 Days January 3 rd and 4 th One Day Camp on December 21 st, 2018 *Age 3-6 th Grade (Age 3 by September 1, 2018 & 100% potty-trained) Registration deadline Friday, December 14, 2018 Payment in full due at time of registration. We will offer 4 days of Christmas Camp. Christmas Camp will run the same hours as summer camp from 9:00AM-3:00PM with extended care available. For your convenience we will run a manned drop off in the morning. We will offer more fun & games during extended care beginning at 8:00 AM if you need to arrive early or pick-up as late as 5:30 PM. We will spend the morning outside on the playground (weather-permitting) or in the gym playing kickball, dodge-ball, and other organized games. We will provide snacks only. Lunch will have to be brought from home. After lunch, we will head inside for an afternoon full of arts and crafts, games, Bible lesson and a movie. One Day Camp December 21, 2018 $50-1 st Child / $45 Sibling Lunch provided for this day only. 2 Here s everything you need to know about camp: HOW OLD DOES MY CHILD NEED TO BE? Age 3 6 th grade. *Must turn 3 by September 1, 2018 and be 100% potty trained. WHO S IN CHARGE HERE? Lisa Jones, Alexis Jones (After School Program Director & Children s Ministry Staff). We also use trained Middle School, High School, and College students who worked with us this summer. REGISTRATION QUESTIONS : Lisa Jones (954) ext. 231 or Ljones@christchurchfl.org. HOW DO I REGISTER? Fill out a Registration Form. Return your forms, with your payment to Christ Church at 4845 NE 25 th Avenue, Ft. Lauderdale, FL or turn them into the church office. The office is open Monday Friday from 8:30am 5:00 pm. We will guarantee your spot if you are registered and paid in full. We will not be able to take walk-ins. We will not give refunds. DEADLINE FOR REGISTRATION IS FRIDAY, DECEMBER 14, 2018 MORNING DROP-OFF Drop-off will be manned from 8:50 AM 9:10 AM for the regular camp day. If you arrive after 9:10 AM you will need to park and bring your child to the Crosswalk Room. If you need to arrive earlier then 8:50 AM you must register for early morning care and bring your child to the Crosswalk Room, located upstairs in the Life Enrichment Center. We are permanently changing the 9:00 drop off in the mornings and the 3:00 pick up in the afternoons to the West side of the church. It is the roundabout area where we have rainy day dismissal. Please do not drop off or pick up on the East side. AFTERNOON PICK-UP The regular camp day is over at 3:00 PM and parents should be in the pick-up line by that time. We will escort your child to your car. We are aware that an emergency will arise, or a traffic jam may cause a parent to be late in picking up a camper. If you are involved in this sort of situation, do not worry. Any child who has not been picked up by 3:10 will be taken to aftercare and may stay as late as 5:30. A Drop In fee of $10.00 will be collected for this service when your camper is picked up. IF YOU KNOW YOU RE GOING TO BE LATE, PLEASE CALL THE CHURCH OFFICE AT (954) EXTENDED DAY PICK-UP (3:00 5:30PM): IF YOU PICK UP BEFORE 5:00 you will need to come to the Church Office. We will bring your camper to the Office. After 5:00, the church office, elevators and gym lobby doors will be locked. The church office lobby will be open. Parents will need to go to the church office lobby to sign their campers out. Please call (754) and we will bring your child down to you. Any child not picked up by 5:30 will be charged a late fee of $10.00 per camper for every five minutes after the 5:30 pick up time. EARLY UNPLANNED PICK UP: If you pick up at any other time during the day you will need to check in at the church office. We will bring your camper to you. PLEASE DO NOT TRY TO LOCATE YOUR CHILD AND PICK HIM OR HER UP. If you know ahead of time that you will need an early pick up (Dr. appointments, etc.), please give us a note when you drop off in the morning and we will bring your camper to the CHURCH OFFICE for you to pick up at the requested time. DOES MY CHILD NEED TO BRING LUNCH? We will provide snacks in Aftercare at no additional cost. Campers must bring their own lunch to camp. Lunch is not provided. WHAT SHOULD MY CHILD WEAR TO CAMP? Campers should wear play clothes, sneakers and bring a sweatshirt. NO Sandals, flip-flops, or Crocs. Please put your child s name on everything. Money, stuffed animals, toys, hand held games, I pods, I phones, etc. are not allowed. We will provide all necessary equipment and supplies, we will not be responsible for lost items. WHAT WILL WE DO AT CAMP? We will split our day between indoor activities (PS2, Wii, four square, arts & crafts, board games, card games, etc.) and outdoor activities (weather permitting) along with a Bible lesion, gym time and an afternoon movie. 4 CHRIST CHURCH CROSSWALK CHRISTMAS CAMP 2018 AND ONE DAY CAMPS REGISTRATION Page 2 of 2 Christ Church United Methodist, Inc Child/Youth Release, Permission Indemnification, & Medical Authorization Agreement I understand Christ Church United Methodist, Inc. is a not-for-profit church that sponsors various programs (hereinafter Ministry Programs ) and that precautions are taken to ensure that the programs and activities with the Ministry Programs in which I am participating are conducted in a safe and responsible manner. However, I further understand that, because of the nature of activities within the Ministry Programs, in which I am enrolling my child/youth, Name of child (hereinafter referred to as Participant ) regardless of the supervision, there is a potential for injury or death during any activity. I do recognize these risks and agree to allow my child/youth to participate in all activities as indicated below. I represent that my child/youth is physically fit and has the necessary skill and physical ability to safely participate in all activities. As consideration for the PARTICIPANT being accepted by Christ Church United Methodist, Inc. for participation in the Christ Church Ministry Programs, and specifically participation in CHRIST CHURCH CHRISTMAS CAMP & ONE DAY CAMPS (Specify travel, event, mission, or activity) Taking place on: December 21, January 4, 2019 (Specify date and/or time period) I, do hereby release, indemnify, forever discharge and agree to hold harmless Christ Church United Methodist, Inc. and the Florida United Methodist Annual Conference, their officers, directors, employees, pastors, volunteers, successors and assigns (collectively called RELEASEES ), From any and all liability, claims, or demands from personal injury, sickness, or death, as well as property damage and expenses of any nature whatsoever, whether legal or administrative, arising out of or in connection with any personal injury, including death, or property damage, resulting in any way from or in any manner arising from PARTICIPANT S participation in the Christ Church Ministry Programs whether caused in whole or in part by negligent acts or failure to act of Christ Church, its agents, servants, employees, pastors or volunteers. This release, indemnification agreement, and wavier specifically includes, but is not limited to, any claim which might otherwise be brought on behalf of myself or the PARTICIANT for (a) negligence or negligent supervision by RELEASEES; (b) any injury or harm, including death, that may occur while PARTICIPANT is participating in Christ Church Ministry Program or Special Event set forth above, including travel to and from, if required, before, during, or after the Ministry Program or Event and (c) any negligent act or failure to act on the part of those chosen to administer emergency medical care to the PARTICIPANT. I acknowledge by my signature below that I have carefully read this release and that I fully understand that this is a waiver of claim and release of liability of the RELEASEES from any and all claims made by me, or on my behalf, or on behalf of PARTICIPANT minor child, regardless of whether those claims are caused by the negligent acts or failure to act by Christ Church, its agents, servants, employees, pastors, or volunteers. The undersigned further consents to the administration of first aid and/or doctor s care or any form of medical treatment necessitated by illness or injury that may require the same. I further agree to indemnify RELEASEES for any costs or attorney s fees incurred by the RELEASEES resulting from any claim that I, my spouse, my child(ren), or heirs may make against any third party as a result of participating in the program activity or event set forth above, even if that claim is caused in whole or in part by the negligence of the RELEASEES. By my signature below, it is my understanding that Christ Church will attempt to notify me in case of a medical emergency involving my child/youth, but if the Church cannot reach me, I consent to the administration of first-aid and/or doctor s care or any form of medical treatment necessitated by illness or injury for the minor PARTICIPANT and I agree that I will pay for any medical expenses incurred. Christ Church is granted permission to use any individual or group photograph taken of me at the event for publicity or brochure purposes. 5 Christ church Behavior rules & expectations As a ministry of Christ Church, Christ Church Afternoon Adventures strives to be a place of grace and love for campers, counselors, and families. Please review the following rules with your child and help us maintain a safe and loving environment for your children. Christ Church Afternoon Adventures Behavior Rules & Expectations: Treat all others with courtesy and respect. ( The Golden Rule ) Use appropriate language. Keep hands and feet to yourself. Everyone participates in scheduled activities and our daily Bible lesson. Everyone helps to keep our rooms clean. Our Respect Pledge As campers and staff at Christ Church Afternoon Adventures, we agree to follow the principles of this pledge to honor a Christian community of caring, and to take responsibility for our own actions. R- RESPECT: I will respect others and myself. E- ENCOURAGE: I will encourage others and myself. S- SAFE: I will work toward making Afternoon Adventures a Christian place that is free of bullying, teasing and name- calling. P- PEACEFUL: I will help settle disagreements in a peaceful manner. E- EXAMPLE: I will be a positive, Christ-like example for my fellow Students. C- CONSIDERATE: I will be considerate of the feelings of others. T- TOLERANT: I will be tolerant of those who are different than I am. Please understand that parents of students who have difficulty with these rules will be notified. Students who purposely hurt others will be asked to go home for the remainder of the day.
Your child may attend Afternoon Adventures on an as-needed basis, but no child will be admitted without a completed registration packet.
