2 I can t wait to be in Panama City Beach, Florida with you! If this is your first event with Rock Creek Students, I hope it is the best experience of your life and the beginning of your getting plugged in! I love this week because it is laid back but also full of fun! It is designed to give you an experience you will never forget. There is no better place for you to have an encounter with God! If you will take advantage of the flexible schedule, break from school, quietness away from cell phones, and the peace of the beach, God will teach you something. Enjoy the worship, time alone with God, and the teaching sessions. Take good notes and record what you learn. Take a nap, swim in the ocean, and fellowship with friends, but most of all sit at the feet of the Father to be restored, refreshed, and refocused. Jason Curry 2

8 Departure Instructions On June 11 STEP 1: Arrive at 9:30pm. Do not be late. You will have one suitcase, one pillow, one sleeping bag, & one carry on with you. STEP 2: Park in parking lot below The Cross Steeple at the East End of Rock Creek. Please do not pull up under covered area as busses will arrive there. Walk in. Do not stand outside. STEP 3: Check in & turn in medication STEP 4: Say bye to mom and dad & go sit with your life group, with your luggage! STEP 5: Jason Curry will dismiss by bus number. *Do not get on the bus when you arrive. *Do not get on the bus until Jason Curry says you can. 8

9 Medicine Information The Church at Rock Creek Prescription Form Student Name Date of Birth Section 1 Prescription Medicines Please fill in the chart for any prescription medicine to be taken at camp. All youth prescription medicine must be turned in during check in in Zip Lock Bag, labeled. Prescriptions will be secured with and dispensed by the Camp Medical Director. Prescription One Dosage Times to be taken Prescription Two Dosage Times to be taken Prescription Three Dosage Times to be taken Section 2 Over the Counter Medicines The Following is a list of over-the-counter medications that may be available & dispensed by the Event Leader and/or Camp Medical Director. Please check EACH ONE if this patient may NOT receive these medications. Caldecort/Cortisone cream to affected area PRN minor pain or discomfort. Acetaminophen, minor pain or discomfort. Ibuprofen, minor pains or discomfort. Benadryl, minor allergic reaction. Neosporin/Bacitracin Antibiotic Ointment apply to minor cuts/abrasions. Caladryl/Calahist lotion apply to affected area for minor itching. Section 3 Emergency Prescriptions (Epi-pen, Rescue Inhaler, etc.) Complete this section ONLY IF your child needs to carry emergency prescriptions with them at all times. (please initial) This is to certify that the above named student must carry the following emergency medication with him/her during the event: They have been trained by their physician on the proper use of the medicine. The student understands that they must report any usage to the camp medical officer immediately. I agree to update The Church at Rock Creek if any adjustments have been made to any and all prescriptions listed above. 9

I hereby agree to hold The Church at Rock Creek, their agents, and employees harmless of any and all liability, actions, causes of actions, claims, expenses and damages on account of injury to my child, property, even injury resulting in death, which I now have or which may arise in the future connected with this activity or participation in any other associated activities. I also give permission for my child to be transported by the agents and employees of The Church at Rock Creek during Beach Trip 2017, June Lack of cooperation with Beach Trip 2017 policies/ rules will result in a student returning home at the parent/legal guardians expense.
