The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic professional services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reasons in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery services, and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. The State of California requires all death certificates to be electronically filed. The physician of the decedent must supply the mortuary with the cause or causes of death and be available to sign the completed certificate as quickly as possible in order to file the death certificate prior to a funeral. If the physician fails to sign in a timely manner, or if the case becomes a coroner's case, the funeral service may be delayed. Chevra Kadisha Mortuary requires that all financial arrangements be paid-in-full prior to services. 2 A. BASIC PROFESSIONAL SERVICES OF FUNERAL DIRECTOR, STAFF & OVERHEAD...$ This is a basic fee that we charge every family for our professional expertise. The fee includes a proportionate share of the taxes, licenses, utilities, and business expenses required to serve the public. Our charge includes, but is not limited to, consultation with the family and the cemetery, and administrative and clerical services. There will be a charge for each additional service required. This fee for our professional services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already in our charges for immediate burials, and forwarding or receiving remains.) We collect a $50 deposit per chair and a $20 deposit per siddur, fully refundable upon return to Chevra Kadisha. B. PERSONALIZED SERVICE PROGRAM PACKAGES COMPLETE MORTUARY SERVICE WITH TRADITIONAL PINE CASKET... $3, This service includes basic professional services of funeral director and staff, local removal and transfer to mortuary during business hours, refrigeration, tahara, tachrichim and shomer; services of funeral director or staff for supervision of funeral service, use of funeral coach, documentation, and securing & filing the death certificate. This also includes a traditional plain pine casket, register book, yarmulkas, prayer booklets, a shiva candle and one certified copy of the death certificate. (This charge does not include cemetery charges, rabbi to officiate, or a tallis.) 2

FORWARDING OF DECEASED TO ANOTHER FUNERAL HOME $2, This service includes the basic service of the funeral director and staff, local transfer of the deceased to our mortuary, refrigeration, local documentation, one certified death certificate, transit permit and transportation to the airport.
