1 Important notice: St. Clare of Assisi Faith Formation Program As I parent/guardian with a child(ren) in St. Clare of Assisi s Faith Formation Program and by my signature on the sign-in sheet at the parent meeting, I: o hereby acknowledge that I have received the St. Clare of Assisi Faith Formation Handbook and that it is fully my responsibility to review the information and explain the policies to my child(ren). o understand that we are required, by our registration, to adhere to the policies in the Handbook. o will keep this handbook in a convenient place for reference. At the baptism of their children, parents undertake the responsibility for their children s Christian formation, and by word and example, teach their children the Catholic faith from birth throughout life. We thank you for allowing us to assist you in the faith development of your children and strongly encourage you to take an active role in our program. Ministerial Statement St. Clare of Assisi s Religious Education Program challenges each family in our parish to live the Catholic faith in full communion with God, the Father, the Son and the Holy Spirit. Through personal prayer and family participation in the Mass and other liturgical services, we strive toward fulfilling the faith commitment made at Baptism. We proclaim God s word through education, preparation and celebration of the sacraments, sharing our faith and service to our community.

The availability of sessions depends on registrations and the availability of teachers and space and cannot be guaranteed. Session lengths are one hour and fifteen minutes each. This early-bird registration allows for a discounted rate but does not guarantee the 1st choice session, due to other variables. All students must re-register each year. Families must be registered members of St. Clare of Assisi parish in order to register in any of St. Clare s Faith Formation Programs. Except for children transferring from another parish religious education program, any child coming into the program after Thanksgiving may not have their partial attendance count towards the completion of the entire year s program, and may not be able to advance to the next level. The reception of a sacrament may also be deferred until the next year. If a child was baptized in a parish other than St. Michael s, Gibbstown, St. John s, Paulsboro or St. Joseph s, Swedesboro, a baptismal certificate must be submitted with the Religious Education registration form. Copies of certificates for any other sacraments received, and a letter from the previous parish pastor acknowledging the change of parish (if applicable), and a letter from the previous parish or school verifying prior religious education or Catholic school completion (report card or letter from prior parish). Students who register for the program but have not yet celebrated the sacraments appropriate to their level will be placed in the same level they attend at their daily school. In order to prepare students for the sacraments, they will be placed in additional classes as part of our Sacramental Preparation Program. Children not completing the full year of religious education or any of its requirements, including service and spiritual exercises, may not be eligible for promotion to the next level. Children with special learning needs are welcome and included, to the extent possible, in our programs. Parents must indicate their children s special needs and/or Individual Education Plan (IEP) when they complete the registration form, so that any additional help or materials needed in the classroom may be obtained. If the special need is diagnosed after registration is completed, the parents must notify the office in writing of the child s special needs and/or IEP. The religious education office will notify the catechist of the special needs, but the parents must stay in regular communication with the catechist to ensure the special needs are met. St. Clare of Assisi Parish does offer sacrament preparation and catechesis to children with needs beyond those that can be provided by mainstreaming. Please contact the Office of Faith Formation for information. Space may be limited. Children with special needs that cannot be met within the parish religious education program, or the parish special needs program, may be directed to the religious education program at St. John of God School in Westville, NJ. Arrival and Dismissal and Parking Please see the Arrival and Dismissal Instructions for your child s campus (a separate handout). Attendance Policies Faith Formation is an ongoing process. Continuous participation in the faith formation program from levels one through eighth is required for reception of the sacraments. A student removed from the program by the parents between levels two and six (other than those attending Catholic School in that interim) may not be accepted into the Confirmation Preparation program. The child may need to repeat the level last attended or be tested. If the child passes he/she will be placed in the proper level. Continued on next page -3

6 No matter the level, students are expected to attend every class in order to gain the full knowledge and experience that the catechists have to offer them. If absence from class is unavoidable, it is the responsibility of the student with the parent s supervision to complete the scheduled class work (reading of the scheduled chapter) for the missed class before the next class. Lesson schedules can be found stapled in the front of every student textbook and on the religious education website by level/session. This also applies to chapters scheduled on dates that the sessions may be closed due to weather related causes or Church liturgies/events. Absentee notes, explaining the reason for the absence must be submitted online in order to be admitted to the next class.. After a second absence, the family will receive a telephone call from the faith formation office. Three absences per school year may be cause for a meeting with the pastor and Coordinator of Faith Formation. Poor attendance may cause sacrament deferral. Chronic absences without an approved reason, and without the completion of all assignments, will render a student ineligible to receive the next sacrament and may prevent promotion to the next level until all the work is completed and reviewed. Consistent tardiness or absenteeism will be addressed by a phone call or meeting with the Coordinator of Faith Formation. Books, Supplies, and Assignments Students in levels 1 through 6 receive textbooks. The textbooks are available in bi-lingual (Spanish) editions upon request). Students in levels 7 and 8 receive a Gospel Weeklies Handbook and Gospel Weeklies weekly lesson pamphlets which are based on each week s Mass readings. Parents must provide a pocket folder and a pencil and/or pen for each child. It is recommended that students keep all religious education materials in a backpack or carry bag dedicated for this purpose. It is the responsibility of the parents to ensure that their children complete review each week s chapter in the home that was covered in the classroom. It is also the total responsibility of the parent to teach their child(ren) the grade level prayer assigned for the year. Prayers are said in the classroom but the teachers do not teach the prayer. Parents are expected to assist their children in remembering to take their books to class each week. If a textbook is lost, the student will be required to purchase a new one. Contact the office for the replacement fee. Child Abuse Prevention Policy The religious education coordinator, catechists, and aides are required by the New Jersey State Law and Diocesan Policy on Child Abuse to report suspected or actual child abuse or neglect to the proper authorities. Parents who observe actual or suspected child abuse are encouraged to report the matter in confidence to the Coordinator of Faith Formation immediately. Child abuse includes, but is not limited to, the following: engaging in a sexual activity with a child denial of proper or necessary substance, education, medical care, or other care necessary for the child s health use of restraint procedures on a child that cause injury or pain administration of prescription drugs or medication without the ongoing supervision of a licensed physician providing alcoholic beverages or controlled substances commission of any act, other than by accidental means, that threatens or results in any injury or death to the child. 7 The Child Lures Prevention program required by the Camden Diocese Office of Safe Environment is used to educate children in all levels concerning personal safety skills. Parent participation in this program through reinforcement of the lessons is essential. You can read more about Child Lures Prevention: Think First & Stay Safe by visiting the Diocese of Camden website. Just click on the Diocese tab and then click on the tab for Safe Environment. Catechists or parents who witness an act of child abuse perpetrated by a cleric, employee or volunteer of the parish, suspect that such an act has occurred, or receive a report of such an act, must report the incident immediately to the diocesan office. The telephone number is Class Cancellations Faith Formation classes will NOT be automatically be cancelled whenever inclement weather forces closings for local schools, Announcements of cancellation of religious education classes will be listed on the KYW website aired over KYW Newsradio 1060 (St. Clare s registration number is 1245), and sent by Gloucester County Alert text notifications. Registering with Gloucester County Alert is mandatory. If there is sufficient time, s may be sent but, should not be relied upon. Telephone calls will not be made. Cancellations may also be posted on the religious education website. Class Placement Any request to transfer a student to another class/session must be made in writing, explaining the reason for the request, to the Office of Faith Formation. Such changes will be made only for serious reasons. Teachers are not permitted to admit a student to join their class without the permission of the Coordinator of Faith Formation. Communication The Office of Faith Formation issues a Newsletter twice each month. This Newsletter and the regular s are the Program s most essential vehicles of communication. Do not unsubscribe from the Newsletter! In doing so, you will severe this essential link and will not receive the information you need! If you are not receiving the Newsletter, contact the Office of Faith Formation immediately. Additionally, all important information is posted and updated continuously on the religious education website. It is the responsibility of the parent to visit the website frequently. The website is listed on the front page of this document. All parents will be provided with a calendar of activities and important dates in the beginning of the school year. As the need arises, additional information may be distributed to all students via a flyer distributed in class although is always the main form of communication between the Office of Faith Formation and families in addition to notices in the OFF Newsletter. In rare cases, notifications may be sent via U.S. mail. On the religious education registration form, all parents are required to give emergency and health information. If this information or addresses change during the course of the year, please notify the religious education office. The burden of keeping this information updated rests on the family in order that communication flows freely between the family and the religious education office. While parents and teachers are encouraged to communicate with one another regarding the children s progress, the teachers are required to follow up with an or private discussion with the Coordinator of Faith Formation on any such communication. Behavioral issues are to be referred directly to the Coordinator of Faith Formation who will contact the parent directly. Curriculum The Catechetical Curriculum of the Diocese of Camden is used as a guideline to provide consistency throughout the diocese. St. Clare of Assisi Faith Formation Program s current textbook series is the We Believe, Project Disciple Series by Sadlier Press levels 1 through 6 and Pflaum Gospel Weeklies for levels 7 & 8. In addition, a Parent Prayers Booklet, listing prayers by level, is distributed to each family to assist in the teaching of traditional Catholic prayers and is considered part of the curriculum. Again, while prayers will be reviewed in class, parents are responsible for the teaching of each level s prayer. -5--

8 Discipline Policies Our Faith is grounded in the belief that every person is created in the image and likeness of God and therefore worthy of respect. Student behavior must be characterized by genuine Christian care, mutual respect, and honesty. Disruptive behavior is disrespectful to rest of the students and the catechist, a volunteer serving the Church for the love of God and the good of the students. Under no circumstances will a student or an adult be permitted to strike another or use abusive language for any reason. Lack of self-discipline during class will be addressed promptly. Removal of a student from the classroom may lead to suspension. Repetitive discipline issues may result in expulsion. School decorum is as listed but is not limited to: No gum at any time No shouting or other loud behavior No writing on desks No touching books or any other items inside a desk No writing on chalkboards without the permission of a teacher or aide No student is to be in the hallway without permission of an aide or teacher and must return to the classroom promptly The normal steps for disciplinary action will be: 1. Verbal correction by the teacher 2. Student sent to the office to meet with the coordinator 3. Parent notified by the coordinator and a conference held 4. Parent and child meet with the coordinator and pastor 5. Child removed from the program and student record documented 6. Sacraments may be deferred For any of the following violations, a student will be removed from the classroom immediately and temporarily suspended until disciplinary actions are taken: drugs/alcohol/substance abuse jeopardizing a person s safety verbal or physical harassment or fighting carrying any object of a harmful nature stealing or damaging parish property (e.g.knives, razors, etc.) Dress Guidelines Our dress code regulations are as follows: No shorts, skirts, or dresses that are higher than mid-thigh, with slits higher than mid-thigh, or that do not fit properly around the waist No halter tops, off the shoulder tops, sleeveless shirts, or tops that reveal bare midriff, abdomen, or back No sundresses with straps less than three inches wide unless worn with a tee shirt underneath No sheer garments that reveal undergarments No garments that display printed material that could be offensive, including but not limited to pictures promoting alcohol, tobacco, violence or inappropriate sexual nature, and no pants with writing across the buttocks No pajamas or bedroom slippers No shirts with low necklines No tight-fitting clothes No flip flops or heelies In order to enforce our guidelines, if a child comes to class inappropriately dressed the following steps will be taken: 1 st offense: Parent will receive a phone call notifying them of the offense 2 nd offense: Parent will be called to deliver a change of clothes All other offenses: Parent will receive a phone call to deliver a change of clothes and the child will remain in the office until the clothes are delivered -6--

9 Emergency Procedures Care of Students The parents/guardians of children attending St. Clare s parish faith formation programs, provide Emergency Contact information, for their children, using the link on the website. In the event of an emergency situation, the staff representative will first make every effort to contact the parent/guardian. Should they be unable to do so, the staff representative will make every effort to contact the person listed as the emergency contact. Parish personnel should not rush an injured child for emergency care without trying to notify parent(s) unless the condition of the child warrants immediate attention. In such cases, the emergency information from the registration form should be sent along with the child. Emergency medical technicians, hospital personnel and physicians will render First Aid only, or measures immediately necessary to prevent loss of life or serious disability to an injured minor. WRITTEN parental consent is required for any further treatment. Emergency Procedures During program session or at parish-sponsored activities: The responsibilities of parish personnel in case of student illness or accidents occurring during the above referenced times include: Giving immediate care Notifying the student s parent/guardian or designated person as soon as possible Treating minor cuts and bruises by using a band-aid or ice. If more than minor, the parents or emergency number will be called by the Coordinator or other designated responsible person Gloves will be used No child will be left alone or unsupervised by an adult during this or any other time. Medications A medical information form must be on file for necessary medicine/treatment of any student in the program. No aspirin or other medication may be given without parent s written permission. The medication must be properly labeled and in its original container. Telephone/oral permission is not valid. If there is a possibility of the child needing an inhaler, the inhaler must be in the prescription box when it is dropped off at the office during class time. Usage of the inhaler should be monitored and noted on the Medication Log. Parents are requested to administer medications before or after any catechetical program or activity whenever possible. Field Trips Field trips, whether religious in nature or for the purposes of offering the youth opportunities to recreate as a Catholic community, are off of the any of the campuses premises. Parents are required to provide transportation for all events not on the school grounds. A permission slip is to be signed by a parent at the beginning of each school year in order for a student to participate. Fire Drills/Lockdown Drills Fire drills/lock Down Drills are conducted at least once during the year. Students are expected to leave the building quickly for a fire drill and quietly and as directed assemble in the designated area where role will be taken. All persons in the building at the time of the drill will participate by evacuating the building. In the instance of a Lockdown drill, they are to quietly and calmly comply with the directions of their teachers and/or program administrator. Financial Policies Each family s tuition is based upon the number of children enrolled. Families are expected to pay their tuition in full at the time of registration. Families experiencing financial difficulties are asked to contact the pastor or Office of Faith Formation to explain their situation and request a payment plan or a partial or full waiver. Such requests will be kept in strict confidence. No child will be denied religious instruction due to a financial hardship. Continued next page -7-

10 In addition to the tuition fee, there may be an activity fee for all children who are celebrating sacraments. (See Sacraments). Families withdrawing children from the PREP program before the scheduled start date will be entitled to a refund. Those who withdraw from the program after the starting date will receive a pro-rated percentage of the fee paid, without exception. New parishioners who register after November 1 st will be charged the cost of the book and pro-rated fee. No exceptions. Mass Attendance Celebration of Mass is essential to developing our relationship with God, and is therefore integral to the religious education program, and is stressed throughout the curriculum. The importance of attendance at Sunday Mass and Mass on Holy Days of Obligations is at the core of the Catholic Faith. The love and gratitude for God which parents and catechists share with children in the home and in the classroom make sense only if the children faithfully attend Mass with their families. Progress Reports To encourage student accomplishment and accountability for learning, students will be monitored with Chapter reviews each week, throughout the year. If a student appears to be having difficulties comprehending the material, the Coordinator of Faith Formation will contact the parent and a conference may be held. A student who fails to meet the learning objectives may be required to work with a parent and the religious education office during the summer to complete all work. Progress reports are no longer distributed unless the student s performance is less than satisfactory, there is a behavioral issue or mandatory requirements are not being met. If necessary, such reports are distribution by . Parents are to review, confirm receipt of the report and respond. Areas assessed include grasp of content, class participation, prayers committed to memory, conduct, attendance, participation in an extraordinary spiritual exercise and completion of service. Rights of Non-Custodial Parents Both parents have the right of access to a child s faith formation progress reports and achievement information unless one parent produces a legal (court certified) document to the contrary. Upon reception of such a document, it will be attached to the child s record and all catechists to whom the child is assigned will be informed of all special directions. Progress reports to the non-custodial parent will be provided upon request, which must be in writing. Changes in the child s normal routine must be approved by the custodial parent and communicated to the religious education office. Catechists will be notified of these changes. No child will be dismissed from the program to the non-custodial parent without the written consent of the custodial parent. Sacrament Preparation Information regarding preparation for receiving the sacraments will be provided at the time of application. The reception of a sacrament is never automatic. At St. Clare of Assisi Parish, as recommended in the Camden Diocese sacrament model, First Eucharist may be received after First Penance and at the time that the child has reached the age of reason (usually around the age of 7), worships regularly as a family; can distinguish between table bread and the bread of the Body of Christ; has a knowledge of prayer and expresses a desire to grow in relationship with the Lord. Again, according to the Camden Diocese Sacrament model, the age to be Confirmed is around 11 or 12 and eligibility for the sacrament presumes an on-going faith-formation from the time of Baptism; must be in continuous religious education and completed four consecutive years of religious instruction; willingly participate in immediate preparation. At this time, preparation for the Sacrament of Confirmation in St. Clare of Assisi parish may be applied for and begin in level 7. Sacraments must be requested through the completion and submission of an application available from the Office of Spiritual Formation. The request is then evaluated based upon the proper disposition, formation, and readiness of the candidate. Approval is dependent upon submission of required assignments and documents, as well as parent and/or student attendance at required meetings and retreats. Continued next page -8-

11 Parents of students preparing for First Penance, Eucharist, and Confirmation are required to attend Parent Sacramental Preparation meetings, formation and interviews. These dates will be provided by the Office of Spiritual Formation on a Sacrament Calendar and parents/guardians are notified of specific dates and times generally through , unless an address is unavailable, and by telephone when necessary. Sacrament preparation for the children takes place outside of the regular religious education or catholic school classes and is mandatory. Please Note: Sacrament Activity fees are not included in the religious education registration fee. This fee helps to defray the cost of the retreats, certificates, textbooks and other costs incurred with the celebration of the sacraments. Fees are due at the time of submission of the sacrament application form. The amount of the fee will be listed on the application. The Sacrament Activity fees are due at the time of submission of any applications to prepare or receive a sacrament. As with the program registration fees, payment arrangements can be made, by contacting the Office of Faith Formation. School Property Students must not use or borrow any items from the desk in which they sit during classes. Any damage that occurs while the students are in class MUST be reported to the teacher. Students who deface or damage school property will be required to remain after class to clean or repair the damage and may face expulsion from the program. Parents will be notified of problems and are expected to reimburse the costs of any repairs. Search and Seizure If there is a suspicion of wrongdoing (weapons, drugs, alcohol), the Coordinator/or responsible adult will be contacted and a search may be undertaken and the parent contacted. Search will be confined to the outer clothing, purses, book bags, equipment bags, etc. The search process shall be done by having the student empty pockets, purses etc. any other body search is inappropriate and will not be done. Sexual Harassment Guarding the dignity of all members in the catechetical program should be a priority. Any violation of this policy will be addressed by the Coordinator. Service As Catholics, we strive to become more like Jesus, the definitive servant leader. Children should be trained to know service as a way of life. Parents are expected to take the lead in this training. Service is an integral part of religious education. Service Completion forms available in the appendix of this handbook and on St. Clare of Assisi's website, must be completed by the student and recorded by the catechist in order to have service acknowledged on the progress report. Non-completion of Service or spiritual requirements may result in non-progression to the next level. Based on the level, the minimum service requirements are as follows: Levels 1 & 2 1 service activity per year Levels 3 through 8 1 service activity per semester (2 semester per faith formation year) Two additional service activities are required for Confirmation preparation candidates. Note: A specific number of hours is not required. Service should be unselfish and fulfill the need of another human being. There should never be a monetary or material gain to the one providing the service. The benefit of service should be the satisfaction of having performed an act of kindness and charity for another as Jesus would, and the grace received from God. Of course, any service above the minimum required which the student provides to the Parish and/or the community is strongly encouraged. Continued next page -9-

Do not wait to upload completed form. Please make a copy should you wish to retain proof. Note: The Service Opportunities made available through the Office of Faith Formation (faith formation program) are opportunities, they are not mandatory. If your child wishes to perform other services that are at least equal to in quality which require effort on his/her part, this is acceptable. Submission of Service Forms remains a requirement for outside service. Spiritual Formation Exercises/Intergenerational Events Spiritual Exercises - Parents train their children in the Catholic faith and help them develop their relationship with Christ by taking them to Mass, teaching them the Bible and Catholic traditions at home, sending them to religious education classes, and more. Along with daily and weekly Mass, as well as faith formation classes, St. Clare Parish offers a wealth of opportunities for spiritual formation. Eucharistic Adoration, procession ceremonies honoring Saint feast days, Advent and Lent Liturgical Celebrations, (see helpful information in Prayers to Know booklet) are held annually, and weekly prayer of The Rosary at church, among other events, lead children to a deeper relationship with Jesus. As part of the faith formation program, each child is expected to attend at least one of these spiritual exercises per year and submit a Spiritual Exercise Journal form to their teacher. (Forms are available on the religious education web site), must be completed by the student. Submit forms directly to the Office of Faith Formation or through the child s catechist in order to have the spiritual exercise acknowledged on the progress report. Forms should be uploaded to the website once the service is complete. Do not wait to upload completed form. Please make a copy should you wish to retain proof. Non-completion of Service or spiritual requirements may result in non-progression to the next level. An example of the Spiritual Exercise Journal is contained in this handbook. See index for location. Intergenerational Events Parish Faith Formation students and their families must attend at least one of the many Intergenerational Gatherings (Parish Faith Festivals) held during the year. See the Family Tracking Sheet provided to assist you in keeping track of your fulfillment of your requirements. Substance Abuse Policy Substance abuse is defined as the possession of, use of, or trafficking in any unauthorized, illegal drug, chemical, alcohol, tobacco, or tobacco-like substance, which can be introduced into the body through ingestion, inhalation, injection or percutaneous means. Any student found in possession of an illegal or unauthorized substance on parish property will be immediately suspended from the program. Any student trafficking in illegal substances will be formally removed from the program and the matter turned over to the local authorities. Transport Substitute Should a parent or guardian wish to have someone other than themselves pick their child(ren) up from a religious education session or event, a form must be on file with the Office of Spiritual Formation stating this and the name of the person providing the alternate transportation for the child(ren). The person must report first to the office not to the child s classroom. A photo identification will be required of the person at that time and they will sign and receive a Pick-up Slip to give to the child s teacher in the classroom. The teacher will not release the child to the substitute person who does not have a Pick- up slip from the office. Should circumstances change in regard to a parent/guardian s choice of alternate, the Office of Faith Formation must be informed of this in writing. No child will be released to someone other than a custodial parent/guardian without written permission and a form of picture I.D. This transport form must be completed online and submitted to the Office of Faith Formation prior to the start of classes. Service and Spiritual Exercises are an element of the progress report and integral to promotion to the next level.
