1 2018 OLPH Parish Survey Please take a few moments to complete the OLPH Parish Survey. If you are a member of our parish, school, or community in any way, we want to know what you think. This is your opportunity to voice your opinion on all matters pertaining to OLPH. Tell us what you like, what you do not like, ways we can improve, and the areas where you think we excel as a faith community. The survey can be taken online at it is available on the school s Facebook page, it will be ed to all school families through SchoolSpeak, and there are also printed copies available in the back of church and in the parish office. If you would like a paper copy mailed to you, please call the parish office at and we will send one to your home. We want to hear from all parish members, so please take a few moments to share your thoughts. We would like to have all surveys completed by Monday, December 10 th. Results of the survey will be analyzed and reported back to the parish. Although we may not be able to address all suggestions, we certainly want to know the major concerns that are on the minds of those within our community. The survey is anonymous, but there is an opportunity to put your name and contact information if you choose to do so. Although some questions are very specific, there are several opportunities within the survey to share your thoughts on whatever topics are important to you. We thank you in advance for your time, suggestions, concerns, support, new ideas, and prayer for the continued success of Our Lady of Perpetual Help.
